The Virginia VSA 67 form is the DMV application you use to get a replacement or substitute vehicle title when your original is lost, damaged, or needs updated information. The fee is $15 by mail or in person, $14 online, and the new title arrives by mail within five business days.1Virginia Department of Motor Vehicles. Transaction: Title Replacement The form has six sections; most applicants complete four of them.
Replacement or Substitute: Pick the Right One
The VSA 67 covers two different requests, and the choice determines which section you sign. A replacement title is an exact reprint. The original was lost, stolen, mutilated, or illegible, and nothing on it needs to change. A substitute title is for when information on the existing title has changed: your legal name, your address, the lienholder, a beneficiary, trustee names, or even the VIN.2Virginia Department of Motor Vehicles. VSA 67 – Application for Replacement and Substitute Titles Both cost $15.3Virginia Department of Motor Vehicles. Substitute Titles
Section 4 is for replacements, Section 5 is for substitutes. You fill out one, not both. If you need a substitute title, you cannot use the DMV’s online portal; online ordering is limited to straight replacements with no information changes.1Virginia Department of Motor Vehicles. What to Have Ready
- The full 17-character VIN from the dashboard plaque or door jamb.
- Year, make, model, body type, plate number, plate type, and vehicle weight.
- The previous title number if you have it. The DMV can look it up if you don’t.
- Lienholder name, mailing address, and lien date, if a lender holds a lien.
- Your DMV customer number from your Virginia driver’s license or ID.
- Your registration card and a photo ID if you plan to apply in person.4Virginia Department of Motor Vehicles. Replacement Titles
- Supporting documents for a substitute title, such as a court-ordered name change, a lien release, or a corrected VIN assignment.
Filling Out the Form Section by Section
Everyone completes Sections 1, 2, and 3. Then you fill in Section 4 or Section 5. Section 6 is only for applicants using an authorized representative.2Virginia Department of Motor Vehicles. VSA 67 – Application for Replacement and Substitute Titles
Section 1: Owner Information
Enter your full legal name exactly as the DMV has it on record: last name first, then first name, middle name, and suffix. The second line is for a co-owner. Any mismatch with DMV records will delay the application or get it rejected. Add your DMV customer number (or Social Security number if you don’t have one), a phone number, and your current mailing address. The section also asks whether any owner listed is on active military duty.
Section 2: Vehicle Information
Fill in the VIN, year, make, model, body type, title number if known, plate number, plate type, and vehicle weight. Check the VIN character by character. One transposed digit means the DMV can’t match the application to your record.
Section 3: Current Lien Information
If a lender has a lien, enter the lien date, lienholder name, lienholder code if you have it, and the lienholder’s mailing address. There is space for a first and second lien. Check the box that tells the DMV whether you’re attaching a paper title or whether the current title is electronic with no paper attached.
Section 4: Replacement Title
Only use this section for an exact reprint with no changes. Sign and date under one of three stated reasons: the most recent title was lost, mutilated, or illegible. A lienholder may apply for a replacement title on its own without the owner’s signature.2Virginia Department of Motor Vehicles. VSA 67 – Application for Replacement and Substitute Titles
Section 5: Substitute Title
Use this section when something on the title needs to change. Check every box that applies. Options include legal name change, name change after a co-owner’s death, address change, adding or removing a beneficiary, clearing a satisfied lien, correcting or assigning a VIN, changing a lienholder’s name, and changing trustee names for a trust. An “Other” box with a line for explanation handles anything that doesn’t fit. Below the checkboxes, list the supporting documents you’re attaching to prove the change.
Section 6: Authorized Representative
If someone other than the recorded owner is submitting the form, the owner fills in the representative’s name and signs here. The representative must present valid photo ID when submitting the application or picking up the title. If they can’t show ID, the DMV mails the new title to the owner instead.2Virginia Department of Motor Vehicles. VSA 67 – Application for Replacement and Substitute Titles
If There’s a Lien on the Vehicle
When a lender holds a lien, the title is usually in the lienholder’s possession or stored electronically with the lien noted. You still complete Section 3 with the lien details. If the loan is paid off but the lien still shows on the record, you need a release from the lender before the DMV will issue a clean title. Virginia law requires a lienholder to execute a release and deliver the title to the owner within ten days after the debt is satisfied.5Virginia Code Commission. Code of Virginia 46.2 – Titling and Registration of Motor Vehicles
If the lender won’t cooperate, perhaps because the company went out of business or won’t respond, you can present whatever evidence you have that the debt is paid, along with a sworn statement to that effect. The DMV will review the documentation and, if satisfied, issue a new title or an endorsement removing the lien.6Virginia Code Commission. Virginia Code 46.2-642 – Release of Security Interest Shown on Certificate of Title Removing a lien changes information on the title, so this is a Section 5 substitute title request, not a Section 4 replacement.
How to Submit the Form
Online (Replacement Only)
If you own or co-own the vehicle, the title was lost, stolen, or mutilated, and no information needs to change, you can order the replacement through the DMV’s online portal for $14. If your address has changed, update it online at least 24 hours before placing the order, because the title gets mailed to the address of the first owner on the record.1Virginia Department of Motor Vehicles. Don’t send cash. If you’re including supporting documents for a substitute title, send originals or certified copies. The DMV will not return them unless you specifically ask.
What Happens After You Submit
The DMV mails the new title within five business days to the address of the first owner listed on the title record.1Virginia Department of Motor Vehicles. Transaction: Title Replacement If you applied in person, the title still comes by mail; the counter visit processes the application, but printing happens at a central facility.
Once a replacement or substitute title is issued, every previously issued title for that vehicle becomes invalid.4Virginia Department of Motor Vehicles. Replacement Titles If the old one turns up in a drawer a week later, shred it. Using a voided title to sell or transfer the vehicle creates legal problems for both parties.
When the Owner Has Died
The VSA 67 is not the right form if the vehicle’s owner has died. The surviving heir, executor, or estate administrator uses the VSA 66 (Application for Transfer and Supplemental Liens) to transfer ownership into their name, along with a certified or notarized death certificate and at least one of the following:4Virginia Department of Motor Vehicles. Replacement Titles
- A copy of the will and a completed Authority to Transfer Virginia Title Certification (VSA 24).
- A court-issued document or copy of the will naming the executor or administrator.
- A completed VSA 24 as proof of legal heir if there is no will and no court appointment.
- A small estate affidavit stating the total estate value is under $50,000.
Penalties for a False Application
Making a false statement on a title application is a Class 6 felony under Virginia law. The same statute covers altering or forging a title and conspiring with someone else to do any of the above.9Virginia Code Commission. Virginia Code 46.2-605 – Altering or Forging Certificate of Title, Salvage/Nonrepairable Certificate, or Registration Card; Penalty A Class 6 felony in Virginia carries one to five years in prison, or at the judge’s or jury’s discretion, up to 12 months in jail and a fine of up to $2,500.10Virginia Code Commission.
