How to Fill Out Connecticut Form B-225: Handicap Parking Permit Application

To get a Connecticut disability parking placard application, download Form B-225P for a permanent or long-term condition or Form B-225T for a temporary one from the CT DMV website, or pick either up at a branch office. You complete Part A with your personal details, a licensed medical professional completes Part B certifying your condition, and you submit the finished form online, by mail, or in person by appointment.¹

Which Form Do You Need

The version depends on how long your condition will affect your mobility.

Form B-225P is for permanent or long-term disabilities. The placard is renewable, free, and valid until your Connecticut driver’s license or non-driver ID expires (up to eight years).¹

Form B-225T is for temporary conditions lasting six months or less. It costs $5 and cannot be renewed. If your condition continues past six months, you file a new application.²

Filling Out Part A

Part A is your section. You’ll need your full legal name, current residential address, date of birth, and a valid Connecticut driver’s license or non-driver ID number.¹ A parent or legal guardian can complete Part A for a minor or for someone unable to fill out the form themselves.³

Sign and date Part A to certify the information is accurate. False information can result in revocation of the permit.

Getting Part B Certified by Your Provider

Your healthcare provider fills out Part B. Licensed physicians, physician assistants, and advanced practice registered nurses can certify the form. For blindness, an optometrist, ophthalmologist, or the Connecticut Board of Education and Services for the Blind can complete Part B, or you can submit a copy of a certificate of blindness instead. Veterans applying based on PTSD need certification from a psychiatrist with the U.S. Department of Veterans Affairs.¹

Your provider must check the specific qualifying condition, include a state license number, and sign. If the license number or signature is missing or illegible, the DMV returns the form unprocessed.

The qualifying conditions are:

You cannot walk 200 feet without stopping to rest.
You cannot walk without a brace, cane, crutch, prosthetic device, wheelchair, or help from another person.
You are legally blind under Connecticut law.
You require portable oxygen.
You have lung disease meeting a defined clinical threshold (FEV1 below one liter on spirometry, or arterial oxygen tension below 60 mm/Hg on room air at rest).
You have a cardiac condition classified as Class III or Class IV under the American Heart Association’s functional limitation standards.²

On Form B-225T, the provider also writes in the expected duration of the disability in weeks or months, up to six months.

How to Submit the Application

You have three options:

Online through the DMV portal at dmv.service.ct.gov.⁴
By mail to CT DMV, Over the Counter Sales Unit, 60 State Street, Wethersfield, CT 06161-5052. Send the original form; photocopies and faxes are not accepted.¹
In person at any DMV branch, by appointment only.²

For a temporary placard, include the $5 fee. Mail-in payments should be a check or money order; online submissions accept credit cards. Renewable placards are free.

Processing Time and What You Receive

Approved placards are mailed to the address on your application. Mail-in applications generally take two to four weeks; in-person submissions may arrive a bit faster. You’ll receive both the placard and an identification card. Carry the ID card whenever the placard is in use.

Renewing or Replacing Your Placard

A permanent placard expires on the same date as your Connecticut driver’s license or non-driver ID. The DMV mails a renewal notice about 45 days before expiration. You can submit a renewal as early as six months before the expiration date and as late as 60 days after it lapses.⁵ Every renewal requires a fresh Part B medical certification.

If your placard is lost or stolen, report the loss to the DMV or local law enforcement, then file a new application with current medical certification through any of the standard channels. A police report can help if the placard was stolen.

Using the Placard Correctly

The placard is issued to you, not to a vehicle. You can use it in any car you’re driving or riding in.³ It cannot be displayed on a vehicle that isn’t carrying you at the time; lending it to someone who parks without you is illegal.

When parked in a designated accessible space, hang the placard from the rearview mirror, or place it face-up on the dashboard if the vehicle has no rearview mirror.³ Remove it before driving.

Misuse carries real penalties. A first violation under the placard statute is a $250 fine, and a second or subsequent violation is $500. Using a placard that belonged to a deceased person is a flat $500 fine. The DMV commissioner can also suspend or revoke the placard after a hearing if the holder allowed someone else to misuse it.³
