How to Fill Out Connecticut Form H-6B: Replacement Title Application

Connecticut Form H-6B is the DMV’s Application for Replacement Certificate of Title, used when your original title is lost, stolen, damaged, or destroyed. The form is a free download from the Connecticut DMV website, costs $25 to file, and can be submitted online, in person at a DMV office, or by mail to the Specialized Registry Services unit in Wethersfield.¹

When to Use Form H-6B

Use it whenever you no longer have a usable original title for a vehicle or vessel registered in Connecticut. Typical situations: a title misplaced during a move, damaged in a flood, or never received after a transaction.

You’ll also need Form H-6B when you have to complete an ownership transfer but can’t find the original title. In that case, submit a Q-1 (Supplemental Assignment of Ownership Form) along with the H-6B, or the DMV will reject and return your application.²

Who Can File

Only the vehicle’s owner or a legal representative with proper identification can apply. If an active lien is on the vehicle, the owner cannot apply directly. The lienholder has to submit the application with a power of attorney, and it must go by mail; the online and in-person paths aren’t available for active-lien applications.¹

If the registered owner has died, the executor or administrator of the estate can apply on the estate’s behalf, signing in place of the deceased owner and attaching a certified probate document (see below).

Filling Out the Form

The form must be printed in ink or typed. Strikeovers and erasures aren’t accepted. If you make a mistake, start over with a clean copy.²

Section 1: Vehicle and Owner Information

Enter the registration plate number (or vessel number), the Vehicle Identification Number or Hull Identification Number, the make, and the model year. Then provide the owner’s name exactly as it appeared on the original title (last name first, then first name and middle initial) along with the full mailing address. If the vehicle has multiple owners, all names must appear here, and all owners must sign in Section 3.²

Section 2: Old Liens You Can’t Get Released

Most people skip this section. You complete it only when a lien appears on the title record, the lien is more than 10 years old, and you can’t obtain a release letter from the original lender (for example, because the company went out of business or changed its name). Signing certifies under penalty of false statement that you have no knowledge of any outstanding liens or encumbrances on the vehicle. A false statement can trigger prosecution under Connecticut General Statutes Sections 14-110 and 53a-157b, and the DMV commissioner can revoke any title issued based on it.²

Section 3: Signature

Sign and date the form. Every owner listed in Section 1 must sign. A legal representative holding power of attorney may sign for the owner but must present proper identification when the application is submitted.²

Handling Liens

Liens are where replacement title applications get tricky, and where most rejections happen. What you need depends on the lien’s status.

Lien paid off: Include a lien release letter from the lender on institutional letterhead, showing the owner’s name, vehicle description (year, make, VIN), a statement that the loan has been paid in full, and an authorized signature.³
Lien over 10 years old, no release available: Complete Section 2 of the H-6B. You may also submit a Motor Vehicle Ownership Affidavit (Form H-115) in place of the lien release letter.³
Lien still active: The owner cannot apply. The lienholder mails the H-6B with a power of attorney.¹

If you’re using the H-6B in place of the current title and a lien has to be released as part of a title change, the $25 replacement fee is added to the $45 title service fee, for $70 total.³

Replacement Title After the Owner’s Death

An executor or administrator can apply using Form H-6B, signing in place of the deceased owner. The application must include a certified copy of a probate court document establishing that authority. Connecticut DMV accepts:

PC-160: Decree from probate court.
PC-212B: Petitioner’s probate certificate (must state that transfer of ownership is allowed).
PC-260: Decree granting administration of a probate will.
PC-264 or PC-264S: Decree or transfer of personal property without probate proceedings.
PC-450: Fiduciary’s probate certificate.

A lien release is still required if a lien appears on the title record.¹

How to Submit

Online

The fastest option. Through the Connecticut DMV’s online services portal, you enter the same information the paper form asks for and pay the $25 fee by credit or debit card. No paper H-6B is needed. The replacement title arrives by mail within 20 business days.¹

In Person

Bring the completed Form H-6B, any required lien documentation, valid photo ID, and $25 to a DMV office. Schedule an appointment beforehand; walk-ins are not available at Cheshire, Putnam, or Stamford, and no location offers walk-in service on Saturdays.³

By Mail

Send the completed H-6B, a $25 check or money order payable to “DMV,” a lien release letter if applicable, and any other supporting documents to:

Fees

The replacement title fee is $25. Mail payments must be by check or money order payable to “DMV.” Online applicants pay by credit or debit card. Combining the replacement with another title service, such as adding or removing a lienholder, adds fees on top of the $25.²

What Happens Next

Online applicants should receive the replacement title in the mail within 20 business days.¹ Mail and in-person applications generally take about 30 days from the date the DMV processes the transaction.³

If the title never arrives, you have 90 days from the transaction date to reapply at no additional cost by submitting a statement of non-receipt to the same Specialized Registry Services address in Wethersfield. Once received, the replacement title carries the same legal weight as the original.
