To fill out Form DE 2501 for California Disability Insurance benefits, complete Part A yourself with your personal, employment, and wage information, have your licensed health professional complete Part B with the medical certification, and submit both parts to the Employment Development Department (EDD) within 49 days of the date your disability began. You can file online through SDI Online or on paper by mail.
Before You Open the Form
EDD won’t process an incomplete application, so gather everything first. You’ll need your Social Security number, California driver license or state ID number, and current mailing address and phone number. If you don’t have a valid SSN or California ID, you have to file on paper; the online system won’t let you through.
On the employment side, pull out your most recent pay stub or W-2 and note your employer’s exact business name, phone number, and mailing address. Write down the last date you worked your normal duties, or the date you switched to modified or reduced duties. EDD uses that date to set the start of your claim and to identify your base period, so get it right.
Also list any wages you have received or expect to receive from your employer after you stopped working: sick leave, paid time off, vacation pay, or annual leave. If you have a pending Workers’ Compensation claim, keep those details close. Leaving this information out can trigger overpayment penalties later.
Filling Out Part A
Part A is the Claimant’s Statement, your section of the form. Whether you’re on paper or in SDI Online, the fields are the same.
Enter your personal identifiers, then your employment details, then the other income sources listed above. Double-check the last-day-worked date before you move on. On paper, complete Part A entirely before handing the form to your doctor; don’t mail anything until Part B is also done.
Online, you submit Part A through the SDI Online portal and receive a receipt number. Write that number down. You’ll pass it to your health provider so they can locate your claim and attach their certification. If your provider doesn’t use SDI Online, they can submit a paper certification with your receipt number written on it, and EDD will match it to your digital claim.
To file online in the first place, create a myEDD account at edd.ca.gov and verify your identity through ID.me. That verification can clear in minutes or take a few days if you have to upload documents. If you’re close to the 49-day deadline already, paper is the safer route.
Getting Part B Completed by Your Provider
Part B is filled out by a licensed health professional, not by you, but you’re the one who has to make sure it happens. Your provider records the clinical diagnosis, the ICD (International Classification of Diseases) code for your condition, the date the disability began, and an estimated return-to-work date. They also list their state license number, business address, and phone number.
Inaccurate coding or missing credentials are among the fastest routes to a denial or a long delay. Schedule your appointment early in your 49-day window rather than at the end of it, because the medical certification has to reach EDD inside that same window.
How to Get a Paper Form
If you’re filing on paper, you can’t print DE 2501 yourself. Request one of these ways:
- Order it through EDD’s online forms page and they’ll mail it to you.
- Call 1-800-480-3287, say “Claim form” at the prompt, and follow the recorded instructions.
- Pick one up at any SDI office, or ask your doctor or employer, since many keep copies on hand.
When and Where to Submit
File your completed DE 2501 no earlier than nine days after your disability begins and no later than 49 days after it starts. Filing too early can cause delays, loss of benefits, or disqualification. Filing too late has the same effect on the other end.
For online claims, submit through the confirmation screen in SDI Online. For paper claims, use the pre-addressed envelope included with the form, or mail it to:

Who Can File This Form
DE 2501 is for workers whose non-work-related illness, injury, pregnancy, or surgery keeps them from doing their regular job. Work-related conditions go through Workers’ Compensation instead, not this form. To qualify for DI, you must:
- Be unable to do your regular work for at least eight days.
- Be losing wages because of the disability.
- Have been employed or actively looking for work when the disability started.
- Have earned at least $300 in your base period from a job where SDI was withheld.
Your base period is roughly the 12 months between 17 months and 5 months before your disability began. If you’re not sure whether SDI was withheld, check a recent pay stub for a “CASDI” or “SDI” line.
What Happens After You File
EDD typically processes a complete claim within about two weeks. If anything is missing or unclear, that timeline stretches.
Even after approval, there’s a seven-day unpaid waiting period measured in calendar days. Your first day of paid benefits is the eighth day of your claim. EDD then sends a Notice of Computation showing your weekly benefit amount. Payment options are direct deposit, an EDD debit card, or a mailed check; direct deposit is fastest, and if bank information is wrong or the deposit fails, EDD falls back to debit card or check.
Filing DE 2501 starts your claim, but it doesn’t keep benefits flowing on its own. Depending on how your claim is set up:
- On automatic payment, after 10 weeks of payments EDD sends a Disability Claim Continuing Eligibility Certification (DE 2593). Complete and return it.
- On non-automatic payment, EDD sends a Claim for Continued Disability Benefits (DE 2500A) every two weeks. Sign, date, and return it.
If you don’t return whichever form applies, benefits stop, even if your disability is ongoing.
If You’re Self-Employed or an Independent Contractor
DE 2501 isn’t your form. Standard W-2 employees have SDI deducted automatically; self-employed workers and independent contractors aren’t covered unless they opt in through EDD’s Disability Insurance Elective Coverage (DIEC) program. The application for that program is DE 1378DI, available on EDD’s website or by calling 1-916-554-7104. DIEC has its own eligibility rules, a two-year minimum participation, and a six-month waiting period after approval before you can claim.
