What Form 411007 Is and When You Use It
Form 411007 is the buyer’s application. It tells the state who now owns the vehicle, what the vehicle is, what you paid for it, and whether a lender has a security interest in it. You use it any time ownership of a titled vehicle changes hands in Iowa: private sales, dealer purchases, gifts, and inheritances all run through the same form. Before you start writing anything on the form, make sure the seller has done their part on the back of the existing title: printed and signed their name in the assignment area, filled in the purchase price and date of sale, written your name as buyer, and recorded the odometer reading for any vehicle model year 2011 or newer (an odometer disclosure is required through December 31, 2030).4Iowa Administrative Rules. Owner Information
Write your full legal name and current residential address as they should appear on the new title. If you want more than one person on the title, list every owner. Indicate whether the names are connected by “and” or by “or.” That single word matters later: “and” means every listed owner must sign to sell the vehicle, while “or” lets any one of them sign alone. Choose deliberately.
Vehicle Description
Copy the year, make, model, body type, and 17-digit VIN directly from the title the seller handed you. Do not work from memory, the registration, or an online listing. A single wrong digit in the VIN stops the transfer, and if the VIN on your application doesn’t match the VIN on the surrendered title, the treasurer cannot process the file.
Purchase Price
Enter what you actually paid. Iowa’s county treasurers work with the Department of Revenue to verify purchase prices, so a suspiciously low number to shrink the 5% fee for new registration invites scrutiny.5Woodbury County, Iowa. Lien Information
If you financed the vehicle, list the lender’s full name and address in the lien section. The lien will be noted on the new title, and the physical title will be sent to the lender rather than to you.1Iowa Department of Transportation. Signatures
Every person listed as an owner must sign. If two spouses will both appear on the title, both sign the application. Signatures need to match the printed names above them; initials or nicknames create problems.
Documents You Bring With the Form
Form 411007 does not travel alone. Bring the following to the county treasurer:

- A completed Damage Disclosure Statement (Form 411108) for any vehicle seven model years old or newer. Gift and Other Exempt Transfers
If the vehicle is a gift, Form 411007 works the same way with one addition. Check the “Claim Exemption” box and complete the second page with the appropriate tax exemption code.9Linn County, IA. You still pay the $35 title fee and applicable registration fees; only the 5% fee for new registration is waived for qualifying transfers.
Cannot Appear in Person
If the buyer or seller cannot sign in person, Iowa allows a power of attorney. The DOT publishes Form 411021 (Power of Attorney Authorization) for exactly this purpose, and both the person granting authority and the person signing on their behalf should include copies of their photo identification.10Iowa Tax And Tags. Filing, Fees, and the 30-Day Deadline
Take the complete package to any county treasurer’s office in Iowa. You do not have to file in the county where you live. Many counties require an appointment for in-person visits; you can also submit by U.S. mail or through a drop box.8Polk County Iowa. Title Transfers

You have 30 days from the purchase date to file. A sale is considered complete when the vehicle is delivered to you, not when you sign paperwork or hand over payment. Miss the deadline and the treasurer adds a $10 penalty for filings 31 to 90 days late, or $20 after 90 days, on top of the regular fees.2Iowa Legislature. When the treasurer accepts your file, you receive a registration receipt on the spot. Keep it in the vehicle; it is your proof of ownership and registration until the printed title arrives by mail. If there is a lien, the title goes to the lender.
Common Reasons a 411007 Gets Rejected
Most rejections come down to a handful of avoidable mistakes:
- VIN on the application does not match the VIN on the surrendered title.
- An owner listed on the application did not sign.
- Signatures do not match the names as printed.

- Missing odometer entry on the title for a model year 2011 or newer vehicle.
- Purchase price on the application inconsistent with the price recorded on the title assignment.

Check every field against the assigned title before you leave the house. A single mismatch means a return trip, and returning after day 30 costs you the penalty on top of the wasted afternoon.
