Gather Your Documents Before You Start
Several fields on the DMV-002 have to match what is printed on the documents you present, so it helps to have them in front of you while you fill out the form. Every document must be an original or a certified copy. Photocopies are not accepted.7Nevada Department of Motor Vehicles. Proof of Identity and Residency
- One proof of identity, such as a U.S. passport, U.S. birth certificate, or Certificate of Naturalization. The accepted list is narrower for a REAL ID than for a standard card.7Nevada Department of Motor Vehicles. Proof of Identity and Residency
- One proof of Social Security number, such as your Social Security card, employment records, or federal tax returns.8Nevada Legislature. Nevada Code 483.290 – Application for License or Instruction Permit

New residents transferring from another state must surrender their out-of-state license at the appointment. Nevada law prohibits holding more than one valid driver’s license, so the DMV collects your old card and returns it to the issuing state.10Nevada Legislature. Personal Information and Physical Description
The top section asks for your full legal name, date of birth, sex, home address, and mailing address if different. Enter your name exactly as it appears on your identity document or on the most recent name-change document you are bringing. NRS 483.290 requires the application to briefly describe the applicant, which on the form translates to fields for height, weight, hair color, and eye color.8Nevada Legislature. Nevada Code 483.290 – Application for License or Instruction Permit You also report whether you have previously held a license and, if so, whether it was ever suspended, revoked, or refused.
Medical Questions
The form asks two yes-or-no medical questions: whether you have a disability or missing extremity, and whether you have any illness or take medication that could affect your driving. If you answer yes to either, the DMV may require your physician to complete a separate Confidential Physician’s Report (Form DLD-7) before a license is issued.15Nevada Department of Motor Vehicles. Confidential Physicians Report There is also a field to note an identified communications need, which can be printed on your card for law enforcement awareness.
Optional Designations
Several optional selections carry real weight and are easy to miss:
- Veteran designation. U.S. veterans can have their status printed on the card by presenting a DD-214 or other evidence of an honorable discharge.16Nevada Department of Motor Vehicles. Nevada DMV New Resident Guide
- Organ donor. Checking the box adds a heart symbol to your card. The form also asks whether you want to contribute $1 or more to the state’s Anatomical Gift Account, also called the Gift of Life Education Fund, with proceeds going to the University of Nevada, Reno School of Medicine for organ donation education.17Nevada Department of Motor Vehicles. - Voter registration. Under the National Voter Registration Act, every state DMV license application doubles as a voter registration opportunity. If you are eligible and choose to register, the DMV transmits your registration to the appropriate election official.18U.S. Department of Justice. The National Voter Registration Act of 1993 (NVRA)
- Gender marker. You can select F, M, or X at any time. No medical certification is required.6Nevada Department of Motor Vehicles. Frequently Asked Questions
If the Applicant Is Under 18
A parent or legal guardian must sign the Consent for Minor’s License section at the office. That signature has legal consequences: the parent who signs takes on joint financial liability for damages caused by the minor’s negligent or willful driving.19Nevada Legislature. Nevada Code 483.300 – Signing and Verification of Application of Minor A parent can later cancel the minor’s permit or license by filing a cancellation request at the DMV, which releases them from further liability.1Nevada Department of Motor Vehicles. Application for Driving Privileges or ID Card (DMV-002)
Minors applying for a license also need to present:
- DMV-301, the certification of school attendance.
- A driver education certificate from a Nevada DMV-approved course.
- DLD-130, a beginning driver experience log certifying at least 50 hours of supervised behind-the-wheel practice.1Nevada Department of Motor Vehicles. Application for Driving Privileges or ID Card (DMV-002)
If the minor opts into organ donation, the parent or guardian also signs acknowledging donor status. Once the minor dies, the parent cannot amend or revoke the anatomical gift if the donor did not do so during their lifetime.

Driver License Testing If you are transferring a valid license from another state, the DMV decides which tests to waive based on your record. Everyone gets a vision check at the window, and any needed restriction, such as corrective lenses, is printed on the card.1Nevada Department of Motor Vehicles.
