Three situations account for almost every request:

How to Get a Copy and Who Fills It Out
You do not fill out Form 040.013 yourself. Pick up a blank copy at your Property Management Office or download one from the NYCHA Self-Service Portal, then deliver it to your employer’s HR or payroll department. Your employer completes the employment details, signs, and dates the form. You bring the completed form back to your Management Office or upload it to the portal as part of your recertification package.
If you work for more than one employer, each one needs to complete a separate form. Give payroll enough lead time; at a larger company the paperwork can sit a week or more before it comes back to you. Check the signature and date before you submit, because an unsigned form gets rejected and you start over.
What the Employer Confirms
The employer verifies your current rate of pay so NYCHA can calculate your gross employment income. If your income includes tips or commissions that move around from paycheck to paycheck, the employer usually enters the base hourly or salary rate. When earnings vary significantly period to period, NYCHA may ask for supplemental records to capture the variable portion accurately.
How the Verified Pay Feeds Into Rent
Income-based rent at NYCHA is 30 percent of the household’s adjusted net income. Not every dollar earned in your household counts. Earnings by children under 18, including foster children, are excluded entirely.5HUD Exchange. NYCHA must process an interim review within 30 calendar days from the date you report the change. If the new figure raises or lowers your rent, you receive written notice of the adjustment.
Using the Form When Adding Someone to Your Household

The full package for a permanent addition includes Form 040.297D, a Consent for Criminal Records form, proof of the new member’s identity and relationship to you, and income verification for that member. When the income verification is Form 040.013 rather than pay stubs, include the completed, signed form with the rest of the package and submit everything to your Property Management Office or through the portal.6New York City Housing Authority. Practical Tips Before You Submit
Deliver the form to your employer well before your recertification appointment. Confirm the employer signed and dated it. Keep a copy for yourself. If your pay has a variable component like tips or commissions, ask whether NYCHA will want supplemental documentation and gather it in advance so a second request does not delay your recertification. If you hold more than one job, do not consolidate the information on a single form; each employer completes its own.
