How to Fill Out Oregon DMV Forms: Title, License, and Placards

Oregon’s Driver and Motor Vehicle Services division runs most of its common transactions through a handful of numbered Oregon DMV forms, each posted as a fillable PDF on the official DMV site and accepted at any field office. The four you are most likely to need are Form 735-268 for titling and registering a vehicle, Form 735-173 for a driver license or ID card, Form 735-6890 for reporting a vehicle sale, and Form 735-265 for a disabled parking placard. Several of these transactions can also be completed online at DMV2U.Oregon.gov without printing anything.

Form 735-268: Title and Registration

Use the Application for Title and Registration (Form 735-268) whenever you buy, inherit, or otherwise acquire a vehicle and need to put it in your name. The form asks for your full legal name, date of birth, Oregon driver license or customer number, residence and mailing addresses, county of residence, and the vehicle’s year, make, body style, fuel type, and Vehicle Identification Number. You also enter your insurance company name and policy number — the company itself, not the agent. There is no field on this form for a Social Security number or an odometer reading.

Odometer disclosure is handled on a separate federal document that travels with the title transfer. Under National Highway Traffic Safety Administration rules, vehicles of model year 2011 or newer need an odometer disclosure when the title changes hands, and vehicles more than 20 years old are exempt. In 2026, that puts the exemption cutoff at model year 2005 and earlier.

Timing matters. If you wait more than 30 days to transfer the title, Oregon charges a late fee of $25 for transfers between 31 and 60 days and $50 after 60 days.1Oregon Driver & Motor Vehicle Services. Titling and Registering Your Vehicle A false statement on this application is a Class A misdemeanor under ORS 803.375, carrying up to one year in jail, a fine of up to $6,250, or both.

Registration Fees

Passenger vehicle registration is billed for a two-year period and scaled to fuel efficiency:2Oregon Driver & Motor Vehicle Services. Vehicle Title, Registration and Permit Fees

  • 0–19 MPG: $126
  • 20–39 MPG: $136
  • 40+ MPG (not enrolled in OReGO): $216
  • All-electric (not enrolled in OReGO): $376
  • Motorcycle or moped: $88

If you enroll in OReGO, Oregon’s per-mile road usage charge program, the two-year registration drops to $86 regardless of fuel type.2Oregon Driver & Motor Vehicle Services. Vehicle Title, Registration and Permit Fees

Out-of-State Dealer Purchases

If you bought the vehicle from a dealer outside Oregon, you may owe a Vehicle Use Tax of one-half of one percent (0.5%) of the retail price before the DMV will process your title. The tax applies only when all of the following are true: the vehicle was purchased from a dealer, has a purchase date of January 1, 2018 or later, was driven 7,500 miles or fewer (or sold with a manufacturer’s certificate of origin), has a gross vehicle weight rating of 26,000 pounds or less, and has never been registered or titled in Oregon.3Oregon Department of Revenue. Vehicle Privilege and Use Taxes You need a certificate of Vehicle Use Tax payment from the Oregon Department of Revenue in hand when you submit the title application.

Form 735-173: Driver License or ID Card

Form 735-173 is shorter than most people expect. The paper itself captures your legal name, date of birth, and current Oregon driver license or permit number if you have one. For applicants under 18, a parent or legal guardian signs a certification on the form confirming school enrollment or completion.4Oregon Department of Transportation. Oregon DMV Form 735-173 – Driver License/ID Card Application The real work is the documents you bring with it, and the requirements differ depending on whether you want a standard card or a REAL ID.

Documents for a Standard License

For a standard Oregon license or ID card, bring one primary identity document and two proofs of your Oregon residential address. Acceptable identity documents include a U.S. passport, a U.S. state or territory birth certificate, a certificate of naturalization, a tribal ID card from a federally recognized tribe in Oregon, a foreign passport not expired more than five years, a military ID, or an out-of-state driver license expired no more than one year. If your current legal name does not match the identity document, you also need a document showing the name change, such as a marriage certificate or court decree.5Oregon Department of Transportation. Acceptable Proof of Identity, Date of Birth, Full Legal Name, and Residence Address

The two address proofs must come from different sources and show your current name and Oregon address. Utility bills, bank statements, rental agreements with the landlord’s original signature, pay stubs, insurance documents, and mail from a government agency or verifiable business all qualify.5Oregon Department of Transportation. Documents for a REAL ID

REAL ID enforcement began May 7, 2025, so you now need a REAL ID-compliant card or a valid U.S. passport to board domestic flights and enter certain federal facilities.6Transportation Security Administration. REAL ID Frequently Asked Questions For a REAL ID, bring one unexpired identity document (U.S. passport, U.S. birth certificate, certificate of naturalization, permanent resident card, or employment authorization card, among others), proof of your Social Security number (the card itself, or a W-2 or SSA-1099 showing your name and SSN), and two proofs of Oregon residence from different sources, following the same categories as the standard license.7Oregon Department of Transportation. Form 735-6890: Notice of Sale

Oregon DMV Form 735-6890 – Notice of Sale or Transfer of a Vehicle Filing it protects you from liability for parking tickets, towing fees, or other problems tied to the vehicle after the sale.

For the buyer, you enter their name, phone number, Oregon driver license or customer number if available, date of birth, address, and the date of purchase. The seller section mirrors that information. The form does not ask for the selling price, because it is a change-of-ownership notice, not a bill of sale.10Oregon Department of Transportation. Oregon DMV Form 735-6890 – Notice of Sale or Transfer of a Vehicle Filing it also does not transfer the title; the buyer still has to apply for a new title in their name on Form 735-268.

Form 735-265: Disabled Parking Placard

The Application for Individual Disabled Person Parking Permit Placard (Form 735-265) comes in two halves. You complete the top section with your personal information and sign a certification that you meet the definition of a person with a disability under ORS 801.387. A licensed physician completes the Certificate of Disability section at the bottom.12Oregon Department of Transportation. Oregon DMV – Application for Individual Disabled Person Parking Permit Placard

The physician’s portion requires their printed name, license number, office address, phone number, and signature, and the doctor indicates whether the disability is temporary or permanent, which sets how long the placard remains valid.13Oregon Public Law. OAR 735-080-0020 – Issuance of Disabled Person Parking Permits Incomplete applications are not processed; a missing license number or signature sends the form back.

Submit the completed form by mail to DMV Driver Transactions Unit, 1905 Lana Ave NE, Salem, Oregon 97314, or take it to any field office. • Response letters for missing requirements: 1 week

Watch your mail during the processing window so you can respond quickly and keep the clock from restarting.

Voter Registration Goes With the License

When you apply for or renew an Oregon driver license, the application doubles as a voter registration form unless you decline, and any address change you submit to the DMV also updates your voter registration address unless you opt out.17Department of Justice. The National Voter Registration Act of 1993 This applies to in-person, mail, and online transactions, so no separate form is needed to register to vote as long as you do not decline that portion of your license application.
