Pennsylvania Form DL-742 is the report you send to PennDOT’s Bureau of Driver Licensing to notify the state that another driver or vehicle owner did not appear to have liability insurance at the time of a motor vehicle accident. Once PennDOT receives the form, it investigates whether the reported party met Pennsylvania’s financial responsibility requirement. When to Use the Form
DL-742 is meant for accidents that also required notice to a police department. If your crash generated a police report and you have reason to believe the other party was driving without insurance, this is the form that puts that suspicion in front of PennDOT. It applies whether the accident involved property damage, personal injury, or death.
Pennsylvania law requires every owner of a currently registered vehicle to maintain liability insurance, so a lapse at the moment of a crash is a reportable violation.2Commonwealth of Pennsylvania. Information to Gather Before You Start
Most of what the form asks for is on the police report or the information exchange you completed at the scene. Pull those documents out before you sit down with the form so you can copy details directly rather than from memory.
- The exact date, time, and location of the accident.
- Full legal names, current mailing addresses, and driver’s license numbers for every driver and vehicle owner involved.
- Year, make, body style, license plate number, state of registration, and Vehicle Identification Number for each vehicle, especially the one you’re reporting as uninsured.
- Your own insurance carrier and policy number, along with any insurance information you obtained from the other party.
Accuracy is worth more attention here than it might seem. If a VIN or driver’s license number doesn’t match PennDOT’s records, the report can be kicked back or routed to the wrong person. Cross-check every number against the police report before writing it on the form.
How to Complete and Submit DL-742

Near the end of the form is a verification section where you attest that the information is true and correct. Pennsylvania takes this seriously. False statements on official PennDOT forms can carry penalties under 18 Pa. C.S. ยง 4904, which covers unsworn falsification to authorities. Report only what you know firsthand or can back up with documentation from the accident.
Mail the completed form to the PennDOT Bureau of Driver Licensing in Harrisburg at the address printed on the form. Send it by certified mail or another method that produces a tracking receipt, and keep a copy of everything you submit.
What PennDOT Does With Your Report
PennDOT reviews the form and cross-references it against insurance records from carriers authorized to write policies in Pennsylvania. The department then contacts the reported party and gives them a chance to show they had active coverage on the date of the accident. If that driver produces a valid insurance card or policy confirmation for the crash date, the matter closes with no penalty.
If the reported party cannot prove coverage, two things happen. What Filing DL-742 Will Not Do
This form starts an administrative process. It is not a substitute for a civil lawsuit or an insurance claim. PennDOT’s role is limited to verifying insurance status and imposing license and registration consequences. The department will not award you money, negotiate a settlement, or order the other driver to pay for your repairs or medical bills.
If you suffered property damage or injuries from an uninsured driver, your paths to recovery run separately from DL-742. You would typically file a claim under your own uninsured motorist coverage if you carry it, or pursue the other driver directly in civil court. Filing DL-742 does create an official record that the other party was uninsured, which can be useful evidence if you end up in court, but the form itself will not get you compensated.
