How to Fill Out Pennsylvania Form MV-46: Illegible Plate Replacement

Pennsylvania Form MV-46 is the Application for Replacement of Illegible Registration Plate, and PennDOT issues the new plate at no charge. If your plate has faded, peeled, or worn to the point where the characters are hard to read, you get an authorized official to verify its condition, fill out the one-page form, and mail it to the Bureau of Motor Vehicles in Harrisburg.

When a Plate Qualifies

PennDOT treats a plate as illegible when one or more letters or numbers can’t be recognized from 50 feet away, or when the plate shows blistering, peeling, discoloration, or loss of reflectivity.¹ The damage has to be from gradual wear and weather. A plate bent in a collision or damaged by vandalism is a different situation and not what this form covers.

To check yourself, have someone stand about 50 feet from the vehicle and try to read every character. If they miss even one, the plate qualifies. Reflectivity is harder to gauge in daylight, but visible flaking or fading is a strong sign.

Who Can Verify the Plate

You cannot self-certify. An authorized official must physically inspect the plate and sign the verification section before PennDOT will accept the application. Three types of officials qualify:²

A certified safety inspector at an official Pennsylvania inspection station. This is usually the easiest route, since you can combine it with your annual inspection.
A local or state police officer.
A PennDOT Online Messenger. These are private businesses contracted to handle registration services, and they may charge a market-driven service fee even though the plate itself is free.³

An illegible plate on its own will not cause a vehicle to fail its annual safety inspection. If the mechanic spots the problem, they should hand you a completed MV-46 so you can request the replacement.

Filling Out the Form

You can download the form from PennDOT’s Driver and Vehicle Services website or pick one up at an inspection station or Online Messenger location.⁴ It has three sections, each completed by a different person.

Section A: Vehicle and Owner Information

Fill this in exactly as it appears on your current registration card: full name (or full business name), street address, telephone number, plate number, vehicle year, make, and VIN.² Individual owners list a Pennsylvania driver’s license or Photo ID number and date of birth. Businesses enter their Business ID, which is the Employer Identification Number. Mismatched details, especially on the plate number or name spelling, are the usual cause of delay.

Section B: Owner Certification

The owner (or an authorized representative for a business) signs here, certifying that the information is accurate. A false statement on the form is punishable as a misdemeanor under Pennsylvania’s unsworn falsification statute.⁵

Section C: Official Verification

The authorized official who inspected the plate completes this section with the plate number, vehicle year and make, their printed name, signature, date, and phone number. A police officer adds badge number and department; an inspection mechanic adds Inspector ID and station name; an Online Messenger adds their DIN or Business Partner ID and messenger name.² PennDOT will reject the application if this section is incomplete or unsigned.

The replacement plate is free. No check, no money order, nothing to enclose but the form itself. PennDOT covers the cost of replacing plates that have deteriorated through normal use.⁶

How Long It Takes

A standard-issue plate typically arrives within about 15 days from the time PennDOT receives and processes your form. Personalized plates take six to eight weeks, because they have to be manufactured with your specific character sequence.⁶

One detail worth pausing on: once PennDOT processes your form, the vehicle record updates immediately to show the new plate number as the valid registration. But you must keep the old plate on the vehicle until the new one physically arrives in the mail.² Do not remove it in the meantime. If you’re stopped during the interim, the officer should be able to confirm through PennDOT’s system that a replacement is in transit.

Personalized Plates

If your illegible plate is a personalized one, PennDOT will replace it with the same style and the same character configuration at no cost.² You keep your custom characters. Expect the six-to-eight-week window.

MV-46 does not cover lost or stolen plates. A personalized plate that has gone missing is replaced in the same style, but you have to pick a new character configuration through a separate process. This form is only for plates that have become illegible through gradual wear.

Penalty for Driving With an Illegible Plate

Displaying an illegible plate is a summary offense under Pennsylvania’s Vehicle Code and carries a $100 fine on conviction.⁷ The statute targets plates illegible in ways that interfere with automated enforcement (red light cameras, electronic tolling) as well as plates where the issuing jurisdiction can’t be identified at a reasonable distance. Since the replacement itself costs nothing, a citation costs more than the postage to send the form.
