Your Part of the Form
Your section is short: full legal name, Social Security Number, phone number, and current mailing address. These fields link the form to your existing claim. A wrong digit in the SSN or an outdated address is enough to delay processing, so double-check before handing it off.
What Your Doctor Certifies
The medical portion is where the substance lives. Your treating physician or licensed practitioner completes it based on a recent examination and provides:
- A diagnosis
- Clinical findings from exams or lab results that support continued disability
- A new estimated recovery date
- Functional limitations that prevent you from performing your regular job duties, such as lifting restrictions, limits on sitting or standing, or inability to use certain body parts
The estimated recovery date drives how long your extended benefits last and when the EDD may review your claim again. The functional limitations are compared against your occupation to confirm you still qualify.
Formatting Rules That Get Paper Forms Rejected

- Black ink only
- One letter or number per box, staying inside the lines so scanning equipment can read it
- No correction tape or liquid; request a new form if you make a mistake
- No address labels, stamps, or signature stamps — the physician must sign by hand
- No strikeouts or strikethroughs
- Leave inapplicable fields blank; do not write “N/A”
- The physician enters a state license number, not an NPI, with all letters and digits
- Dates go in MMDDYYYY format

Online Through SDI Online
The physician submits the medical certification electronically from their own SDI Online account. - Search for your claim using either your Claim ID or the last four digits of your SSN, plus your last name and date of birth
- Select “Submit Additional Medical Information” from the Action column in the search results
- Choose the 2525XX under “My Forms Available to Submit” and complete the required medical fields
The system generates a confirmation number after submission. Ask the office to save or screenshot it. Medical staff handle dozens of forms a day, and the confirmation is your proof of filing if the EDD later questions when the form arrived.
By Mail
The mailing address is pre-printed on the form. Have your doctor complete the medical certification, then send the entire form to the address shown. Certified mail or a certificate of mailing from the post office gives you proof of the submission date, which is worth the small cost when a late arrival can mean lost benefits.
Processing, Payment, and the 52-Week Cap
Allow 10 business days after the EDD receives the form. During that window, staff review the physician’s clinical findings to confirm you still qualify. California DI benefits are capped at 52 weeks for a non-work-related illness, injury, or pregnancy.5Employment Development Department. If you originally claimed 12 weeks and your doctor certifies another 10, you’re at 22 weeks. If you’re already at week 50 and your doctor certifies 8 more, you’ll only be paid through week 52.
Watch Out for Gap Days
If there’s a gap between your original certified end date and the start date on the new certification, those gap days may not be eligible for payment.6Employment Development Department. If the Extension Is Denied

Your appeal should explain in detail why you believe you’re still eligible and include any supporting medical documentation that wasn’t in the original submission. Mail the completed DE 1000A to the return address on your notice. If you no longer have the form, a signed letter works as long as it includes your full name, claim ID or EDD Customer Account Number, Social Security Number, address, phone number, and the reason for the appeal.
The EDD reviews the appeal internally first. If it reverses course, payments resume. If not, the case goes to the California Unemployment Insurance Appeals Board, where an Administrative Law Judge holds a hearing with evidence from you and an EDD representative.
