The California DOJ firearms qualification form is the document that proves you passed the live-fire portion of your concealed carry training, and it travels with your CCW application to your local licensing authority. A Department of Justice-certified instructor completes and signs most of it after your range session, recording the specifics of each firearm you qualified with. You fill in your personal identification, then submit the form alongside Form BOF 4012 and the rest of your packet to the county sheriff or city police chief who handles CCW licensing where you live.1State of California – Department of Justice – Office of the Attorney General. Who Fills Out Which Parts
The form has two sides to it. The training and qualification sections belong to the instructor. Only a firearms instructor certified by the California Department of Justice under Penal Code section 31635 is authorized to complete and sign them.2State of California – Department of Justice – Office of the Attorney General. Becoming a Carry Concealed Weapon Program DOJ Certified Instructor The instructor records:
- Their full name, DOJ certification number, and contact information
- The dates of training, total hours completed, and confirmation that the course covered all required topics
- Live-fire results for each firearm: rounds fired, firing distances, and score
- Firearm information for every pistol or revolver you qualified with: manufacturer, model, caliber, and serial number
The instructor’s signature certifies that you met the licensing authority’s proficiency standards. A false certification exposes the instructor to serious professional and legal consequences, which is why they fill in these sections personally rather than letting you self-report.
Before the instructor signs, check the serial numbers against the firearms themselves. A single transposed digit can create problems when the licensing authority cross-references the form against your DOJ registration records, and every firearm listed must already be registered in your name.
The Applicant Section
Your portion is basic identifying information, but it has to match your current records exactly:
- Full legal name as it appears on your California driver’s license or state ID
- Current residential address. Licensing authorities generally require a physical address rather than a P.O. box3San Diego County Sheriff. Carry Concealed Weapons (CCW) License
- California driver’s license or ID number, which links you to the background check
- Date of birth, used to verify the age-21 minimum under Penal Code section 261504California Legislative Information. California Penal Code 26150 – License to Carry a Pistol, Revolver, or Other Firearm Capable of Being Concealed Upon the Person
If you recently changed your name or moved, update your driver’s license first. Mismatches between the form and your identification are one of the most common reasons applications stall.
What Goes on the Form Comes From the Live-Fire Session
The round counts, distances, and scores the instructor writes down come from the course of fire your licensing authority has published. Penal Code section 26165 requires you to demonstrate safe handling and shooting proficiency with every firearm you want listed on your license. You cannot qualify with one pistol and carry a different one.5California Legislative Information. California Penal Code 26165 The same statute directs each licensing authority to set its own specific standards, so the numbers that end up on the form depend on where you apply.
Sacramento County, for instance, requires 35 rounds per firearm across five stages at distances from 1.5 yards to 10 yards, with a minimum passing score of 80 percent on a 12-by-16-inch scorable target.6Sacramento County Sheriff’s Office. Compliance with California Legislation in Senate Bill 2 San Diego County uses a 15-round course from 3 to 7 yards with specific one-hand and two-hand grip requirements at each stage.3San Diego County Sheriff. If you fail the live-fire portion, the instructor cannot sign off on the qualification form at all. Most training providers allow a retest. Retake policies and fees vary by provider.
Submitting the Form
The completed qualification form does not go to the DOJ. You submit it to your local licensing authority: the sheriff of your county or the chief of police in your city. It is one piece of a packet built around the standard CCW application, Form BOF 4012.1State of California – Department of Justice – Office of the Attorney General. Forms and Publications Along with the qualification form and BOF 4012, a typical packet includes:
- Proof of residency, such as utility bills or bank statements. Some counties require two proofs for initial applications3San Diego County Sheriff. Carry Concealed Weapons (CCW) License
- LiveScan fingerprints for the DOJ and FBI background check, usually completed at a separate appointment
- Character references. Many counties require three for initial applicants, with specific rules about including a cohabitant and a current or former spouse or dating partner3San Diego County Sheriff. Carry Concealed Weapons (CCW) License
- A passport-style photograph
Submission methods differ by agency. Some counties use online portals where you upload documents; others require in-person delivery during a scheduled interview. Check your licensing authority’s website for the current process, and keep copies of everything you submit. If a document is lost in processing, you want to replace it from your own file rather than schedule another qualification session.
How Fresh the Qualification Has to Be
Timing matters. The LAPD, for example, will not accept proof of training dated more than six months before your initial application interview.7Los Angeles Police Department. CCW Carry Concealed Weapon License Other agencies impose similar windows. Qualifying too far ahead of submission can force you to retake the course and generate a new form.
Renewal applicants go through this cycle every two years, since California CCW licenses are valid for two-year terms.8Sacramento County Sheriff’s Office. Concealed Weapons Permit Information Renewal training is shorter — eight hours instead of the 16 required for an initial applicant — but it still includes a written exam and live-fire exercises that produce a new qualification form.5California Legislative Information. California Penal Code 26165 DOJ-certified instructors are not required to take a renewal course to renew their own licenses.
If the Application Is Denied
A denial based on a “disqualified person” determination under Penal Code section 26202 can be challenged. Disqualifying factors include being found likely to be a danger to yourself or others, certain prior convictions or dismissed charges within the past ten years, being subject to a restraining order within the past five years, and a history of unlawful firearm use.9California Legislative Information. California Penal Code 26202
If you are denied on these grounds, the licensing authority must give you written notice explaining why and provide a “Request for Hearing to Challenge Disqualified Person Determination” form.10State of California – Department of Justice – Office of the Attorney General. Frequently Asked Questions You have 30 days from receipt of that notice to request a hearing from the superior court of your county of residence. Some licensing authorities require you to exhaust an internal appeal first; if so, the agency must resolve the appeal within 60 days, and you get another 30-day window after an unsuccessful internal appeal to request the court hearing.11California Legislative Information. California Penal Code 26206 Mark the 30-day deadline the day you receive the notice. Missing it forfeits your right to judicial review of that denial.
