How to Fill Out the Florida Medicaid Group Membership Authorization Form

Florida Medicaid’s Group Membership Authorization form (AHCA Form 5000-1061) is what you file with the Agency for Health Care Administration to link an individual practitioner to a group provider for billing, or to remove that link when the practitioner leaves. Download it from the Florida Medicaid Web Portal at portal.flmmis.com under Enrollment Forms, then submit the signed form through your secure portal account or by contacting the fiscal agent, Gainwell Technologies, at 1-800-289-7799 (Option 4).1Florida Medicaid Web Portal. When You File It

Florida Administrative Code Rule 59G-1.060 requires group membership authorization whenever a practitioner forms, joins, or separates from a group.2Agency for Health Care Administration. Florida Medicaid Provider Enrollment Policy In practice, that means a new hire who needs to bill under the group’s Medicaid ID, a practitioner moving from one group to another, or a group dissolving and its members returning to individual billing. The portal’s enrollment forms page also lists this form as required when enrolling additional service locations under a group.3Florida Medicaid Web Portal. Both parties must already be enrolled in Florida Medicaid before the linkage can be created. The form does not enroll anyone; it connects two existing Medicaid provider records.

What to Gather Before You Start

Most rejections come from mismatched identifiers, and a rejection sends you back to the beginning with a new submission. Have these ready:

  • The individual practitioner’s National Provider Identifier (NPI), the 10-digit number from CMS. If the practitioner doesn’t have one, apply at nppes.cms.hhs.gov first.
  • The individual practitioner’s Florida Medicaid Provider ID, which appears on the enrollment confirmation from AHCA.
  • The group’s Tax Identification Number. The group name on the form must match the name associated with this TIN at the IRS exactly. Even minor discrepancies, such as an ampersand versus the word “and,” can trigger a rejection.
  • The group’s legal name as registered with the state and used on its Medicaid enrollment.
  • The group’s Medicaid Provider ID.
  • The requested effective date, which controls when the group can start or must stop billing for the practitioner’s services.

Completing the Form

Form 5000-1061 is a single page. In the individual provider section, enter the practitioner’s NPI, Florida Medicaid Provider ID, and full legal name. In the group provider section, enter the group’s TIN, Medicaid Provider ID, and legal name. Indicate whether you are adding the practitioner to the group or removing them, and enter the effective date.

Both the individual practitioner and the group’s authorized representative must sign. A form missing either signature is incomplete and will not be processed. The signatures serve as each party’s attestation that the information is accurate and that both consent to the billing arrangement being created or ended.

How to Submit

Through the Secure Web Portal

Electronic submission is the standard route. The Florida Medicaid Provider Enrollment Application Guide directs providers to manage group linkage information through their secure Web Portal accounts at home.flmmis.com. Log in, go to the Trade Files or File Upload panel, and upload the signed form.4Home Care Office. Florida Medicaid Provider Enrollment Application Guide Confirm the upload with the receipt screen. Electronic submissions create an immediate record and process faster than paper because they bypass manual data entry.

By Phone or Mail

If you cannot use the portal, call Gainwell Technologies at 1-800-289-7799, Option 4, for guidance on alternative submission methods.1Florida Medicaid Web Portal. After You Submit

Check status through the secure Web Portal. Once the update is finalized, open the practitioner’s provider file and confirm the group affiliation and effective date match what you requested. Catching a mismatch early prevents claim denials later.

If the enrollment unit finds a problem with your submission, you’ll get a written notice. You have 21 days from the date of that notice to correct the deficiency. Miss that window and the request is denied, meaning a fresh submission.2Agency for Health Care Administration. Once the linkage is active, any changes to enrollment information must be reported in writing within 30 days. AHCA can take action up to and including termination against providers who fail to report changes.2Agency for Health Care Administration. Florida Medicaid Provider Enrollment Policy When a practitioner leaves the group, file a new Group Membership Authorization form promptly to delink them; claims submitted under the group’s identifier after departure create billing problems for both sides.

Managed Care Plans Are Separate

This form updates your status in Florida Medicaid’s fee-for-service system only. It does not enroll or credential you with the state’s Medicaid managed care plans, such as Sunshine Health. Each managed care plan runs its own credentialing process, so if you treat patients enrolled in one of those plans, apply directly with the plan in addition to filing Form 5000-1061 with AHCA.5Sunshine Health.
