To fill out the Georgia WIC Request for Medical Formulas (RMF) form, download the fillable PDF from the Georgia Department of Public Health’s WIC Formula Resources page, have a licensed healthcare provider complete every field and sign it, and then fax or hand-deliver the signed form to your local WIC clinic.1Georgia Department of Public Health. WIC Formula Resources The form is one page and looks simple, but a missing field or a formula that isn’t on Georgia’s approved list will send it back.
Who Completes the Form
The RMF is designed for your healthcare provider to fill out, not you. A physician, physician assistant, or nurse practitioner completes it based on their clinical assessment of the participant. The PDF can be filled in on a computer, saved, and printed for signature, and page two of the document itself carries instructions and resources for the provider.1Georgia Department of Public Health. Your job is to get the form to the right provider, make sure they have Georgia’s approved formula list in front of them, and check that nothing is left blank before it leaves the office.
What Each Field Requires
- Participant information. Full name and date of birth, used by the clinic to match the request to the correct case file.
- Medical diagnosis. The specific qualifying condition and its ICD-10 code. The code removes ambiguity about the diagnosis — R62.51 for failure to thrive in a child, or E70.0 for PKU, for example.
- Prescribed formula or medical food. The exact brand name and product type from Georgia’s approved list, plus the amount needed per day in ounces or containers.2eCFR. 7 CFR 246.10 – Supplemental Foods
- Duration. How long the participant needs the prescribed product. This sets the authorization window; once it expires, a new form is required.
- Standard food modifications. If the condition also calls for removing or substituting standard WIC foods such as juice, cereal, or whole wheat bread, the provider marks those changes so the clinic can adjust the full food package.
- Signature, date, and provider contact information. Without all three, the form cannot be processed.
A note on acceptable diagnoses: personal preference, general fussiness, gas, spitting up, and colic are not qualifying conditions. The diagnosis must be one where the standard food package would be medically insufficient, such as premature birth, low birth weight, failure to thrive, severe food allergies, or metabolic disorders like PKU or galactosemia.
Pick a Formula From Georgia’s Approved List
This is where most RMFs get rejected. Georgia WIC maintains a specific list of approved formulas and nutritional products, and a product that isn’t on it will not be issued no matter how clearly it’s prescribed.3Georgia Department of Public Health. Georgia WIC Approved Formulas and Nutritionals
A handful of contract brand infant formulas — currently Enfamil Infant, Enfamil Gentlease, Enfamil A.R., Enfamil Reguline, and Enfamil ProSobee — can be issued without an RMF at all. Everything else on the list, from amino acid–based formulas like EleCare and Alfamino to calorie supplements like Duocal and Benecalorie, requires a completed RMF. Pull the current approved list from the WIC Formula Resources page and bring it to the appointment so the provider prescribes a product the clinic can actually fulfill.
Who Can Sign
Federal WIC regulations require the form to be signed by a healthcare professional licensed to write prescriptions under state law.2eCFR. 7 CFR 246.10 – Supplemental Foods In practice, that means a physician (MD or DO), a physician assistant, or an advanced practice registered nurse. A form signed by someone outside that group cannot be processed.
The signature date also establishes when the authorization period begins, so an undated signature creates a real problem even if everything else is correct.
How to Submit the Form
The completed form goes to your local WIC clinic, not the state office. You have two practical options:
- Fax from the provider’s office. Fastest, and avoids an extra trip.
- Hand-deliver at your WIC appointment. Lets you confirm receipt on the spot.
Mailing is possible but adds unnecessary delay. To find your local clinic’s fax number and address, use the Georgia WIC Clinic and Store Locator on the Department of Public Health website, or call the Georgia WIC hotline at (800) 228-9173.4Georgia Department of Public Health. WIC
What Happens After You Submit
Clinic staff check that every required field is complete, that the prescribed formula is on Georgia’s approved list, that the diagnosis supports the requested product, and that the signature and date are present. If anything is missing or unclear, the clinic contacts the provider or you to correct it before moving forward. That back-and-forth is the single most common source of delay.
Once approved, the special formula or medical food is loaded onto your eWIC card along with any modified standard food items. You then buy the prescribed products at an authorized WIC vendor; the same locator that finds your clinic lists authorized retail locations.5Georgia Department of Public Health. Renewals
The authorization lasts only as long as the duration your provider wrote on the form. When it ends, the clinic cannot keep issuing the special formula without a new, freshly signed RMF, even if the condition hasn’t changed. Schedule the follow-up with the provider before the current authorization expires so there’s no gap in benefits. Lifelong conditions like PKU still require the paperwork cycle at each WIC certification period.
If the Request Is Denied
Denials fall into two categories. Paperwork problems — a missing signature, an unlisted formula, an incomplete diagnosis — are fixable: ask the clinic exactly what’s wrong, have the provider correct and resubmit, and it can often clear within days.
If the denial is based on the clinic finding that the medical documentation does not support the requested product, you have the right to appeal. Federal WIC regulations require written notice of any decision that makes a participant ineligible or changes benefits, including the reasons and the right to request a fair hearing.6eCFR. 7 CFR 246.7 – Certification of Participants Contact your local WIC clinic or the Georgia WIC hotline at (800) 228-9173 to ask how to file a fair hearing request and what deadlines apply.4Georgia Department of Public Health.
