The IDPH EMS System Plan Modification Form is a one-page Illinois Department of Public Health document that a Resource Hospital uses to request changes to an already-approved EMS System Program Plan. You can download it from the IDPH EMS Provider and Vehicle Licensing page, where it is listed as “EMS System Plan Modification.”1Illinois Department of Public Health. EMS Provider and Vehicle Licensing Fill in the system and provider identification, check the type of change you are requesting, attach the required supporting documents, collect both signatures, and send the packet to your Regional EMS Coordinator — not directly to Springfield.
When You Have to File One
Any time your system’s day-to-day operations no longer match the plan IDPH approved, the Resource Hospital files a modification. The form itself covers these categories:2Illinois Department of Public Health. Emergency Medical Services (EMS) Systems – Request to Modify / Amend Approved System Plan
- Provider upgrade or downgrade between care levels (for example, BLS to ILS, or ALS to ILS).
- Vehicle additions or replacements. IDPH inspection is required before the vehicle can operate.
- Response area changes — adjusting the geographic territory a provider covers.
- Dispatch procedure changes — revising the access and dispatch mechanisms the system uses.
- Other modifications that alter the approved plan but do not fit the categories above.
The form warns at the top that incomplete applications will be returned to the Resource Hospital, so pulling everything together before you start saves a round trip.
Filling Out the Form
The form is short, but it pulls information from several people. Work through it in order.
System and Provider Identification
Start with the administrative block at the top: the EMS Medical Director’s printed name, the Resource Hospital name, and the EMS System Number assigned by IDPH. Add the Resource Hospital’s street address, city, state, and ZIP code. Then identify the specific provider involved in the modification by printed name, provider number, and city/state.2Illinois Department of Public Health. Emergency Medical Services (EMS) Systems – Request to Modify / Amend Approved System Plan The system number and provider number must match what IDPH has on file; if you are unsure, cross-check against the IDPH EMS System Listing.3Illinois Department of Public Health. Modification Type
Check the boxes that describe the request. For a change in a provider’s or vehicle’s care level, mark upgrade or downgrade and specify both the current and the requested level. The form lists First Responder, BLS, ILS, ALS, B/D (basic with defibrillation), and CCT (critical care transport).2Illinois Department of Public Health. For a response-area change, check the box and attach a separate sheet describing the new boundaries. For a dispatch procedure change, check the box and attach a written description of the revised procedures. The “Other” box covers anything else; describe it and attach supporting details.
Signatures
The applicant — typically the EMS System Coordinator or Resource Hospital administrator — signs and dates the request section. A separate signature block below is reserved for the EMS Medical Director or EMS System Coordinator, who must verify that the provider meets vehicle, equipment, and staffing requirements under current regulations and the system’s own plan before recommending approval.2Illinois Department of Public Health. What to Attach
Required attachments depend on which box you checked.
- Response area changes: a written description of the new boundaries, a map showing each vehicle’s response territory, the area’s square miles and population, and the locations of the resource or associate hospital and vehicle stations.
- Dispatch procedure changes: a written description of the revised access and dispatch mechanisms.
- Vehicle additions or replacements: plan for an IDPH vehicle inspection before the change takes effect. Filing the form does not waive it.
- Other modifications: a written description and any supporting documentation that explains the change.
If the modification adds a new hospital or provider to the system, the underlying Program Plan requires a letter of commitment from that entity, and you will likely need to supply a fresh one to keep the plan record current.5Cornell Law Institute. Where to Send It
The form does not go directly to the IDPH Central Office. It includes a “REMSC Review” block where your Regional EMS Coordinator signs off with “Recommended,” “Not Recommended,” or “Discuss” before the packet moves to Springfield for processing.2Illinois Department of Public Health. Emergency Medical Services (EMS) Systems – Request to Modify / Amend Approved System Plan Your first submission goes to the Regional EMS Coordinator assigned to your area.
Illinois has eleven EMS regions. Contact your Regional EMS Coordinator to confirm the preferred delivery method; some accept electronic submission by email, while others want a physical copy mailed or hand-delivered to the regional office. If you are not sure which region your system falls under, the IDPH EMS System Listing organizes systems by region.3Illinois Department of Public Health. EMS System Listing
After You Submit
Once the Regional EMS Coordinator has the packet, they review it, mark their recommendation, and send it along with your form to the IDPH Central Office. Central Office staff complete the processing block, which records the date processed, the license number, VIN (where a vehicle is involved), current care level, and requested care level.2Illinois Department of Public Health. Emergency Medical Services (EMS) Systems – Request to Modify / Amend Approved System Plan
IDPH does not publish a guaranteed turnaround time. In practice, a single vehicle addition moves faster than a wholesale response-area redraw. If the application is incomplete, the form instructions make clear it will be sent back to the Resource Hospital, so the fastest route to approval is making sure every required attachment and signature is in the packet before it reaches your coordinator.
The modification is not effective until IDPH processes it. Operating under the changed plan before you have authorization puts the system out of compliance with the approved Program Plan, which the Department is statutorily required to monitor.6Illinois General Assembly. 210 ILCS 50 – Emergency Medical Services (EMS) Systems Act
A separate note on scope: this form amends an existing system plan. If a Resource Hospital wants to break away from an existing system and have an entirely new EMS System approved, that is a different and more involved process under 77 Ill. Adm. Code 515.300, not a modification request.7Cornell Law Institute. Illinois Administrative Code 77 Section 515.300 – Approval of New EMS Systems
Mistakes That Get the Form Returned
Most returned applications fail on the basics. Check for these before you send:
- Missing the Medical Director signature. The form has two signature lines and both must be completed; the applicant’s signature alone is not enough.
- Wrong system or provider number. One transposed digit means IDPH cannot match the request to an existing record.
- No map for response-area changes. A narrative description without the map, square miles, and population figures is incomplete.
- Forgetting the vehicle inspection. IDPH must inspect added or replacement vehicles separately.
- Sending the form straight to Springfield. The REMSC Review block has to be signed first, or the form is incomplete when it arrives at Central Office.
