How to Fill Out the Kentucky PAFS-700: Items, Wage Table, and Return

The Kentucky PAFS-700 form is a wage-verification request sent by the Department for Community Based Services (DCBS) to employers when a worker applies for or renews SNAP, K-TAP, or Medicaid. You fill in the employee’s pay, hours, and job status for the dates printed on the form, sign the certification, and return it to the caseworker. Speed matters: SNAP applications must be decided within 30 days of filing, and benefits cannot be finalized until your numbers arrive.1kynect. Pull These Records First

Before writing anything on the form, gather the payroll documents covering the date range DCBS printed on it. The agency pre-fills the employee’s name and Social Security number on the copy it sends you; check those against your own records and correct them if they’re wrong.2Cabinet for Health and Family Services. • Payroll register or pay stubs for every pay period in the requested range, showing gross wages, hours, and taxes withheld.
  • Most recent hire date and the date of the first paycheck.
  • Current pay rate, scheduled weekly hours, and any upcoming rate change with its effective date.
  • Retirement plan details if the employee participates: plan type, balance, and early-withdrawal penalty.
  • Leave or termination records if the employee is out or no longer on your payroll.

Use blue or black ink on the paper version. Don’t leave fields blank. If something doesn’t apply, write “N/A” so the caseworker can see you considered it rather than skipped it.

Working Through the Form

The PAFS-700 has 13 numbered items, a wage-reporting table, and an employer certification block at the bottom.

Items 1 Through 3: Identity and Employment Status

Item 1 confirms or corrects the employee’s name and SSN. Item 2 asks yes or no whether the person is currently on your payroll. Item 3 asks for the most recent hire date and the date of the first paycheck. For a rehire, use the most recent hire date, not the original one.2Cabinet for Health and Family Services. Items 4 Through 9: Pay Rate, Hours, and Frequency

Item 4 packs several details into one block. Check the box for how often you pay (weekly, biweekly, semimonthly, or monthly) and fill in the hourly rate, overtime rate, anticipated weekly hours, the day of the week the employee is paid, and any shift premium. For a salaried worker, convert to an hourly equivalent or write in the salary with a note.

Items 5 and 6 ask whether a rate or hours change is coming. If so, enter the new figure, the effective date, and the first paycheck that will reflect it. Item 7 asks whether the employee voluntarily cut hours and why. The reason matters because a voluntary reduction can affect benefit calculations differently than an employer-driven one.

Item 8 asks whether the employee’s share of taxes is deducted from gross wages; for standard payroll setups the answer is yes. Item 9 asks whether the wages come through a federally funded program such as Title V of the Older Americans Act or WIOA on-the-job training. Check the box only if it applies.2Cabinet for Health and Family Services. PAFS-700 Verification of Employment and Wages

Item 10: The Wage Table

This is the core of the form. List every paycheck issued during the months printed on the form. For each one, enter the date the employee received the check, hours worked, gross wages before deductions, tips (separately if not already in gross), and taxes withheld. The table has ten rows, which usually covers two to four months depending on pay frequency.2Cabinet for Health and Family Services. PAFS-700 Verification of Employment and Wages

If a pay period in the requested range had no hours or earnings, write zero rather than leaving the row empty. A blank row looks skipped; a zero shows you checked. Report gross wages, not net. The caseworker handles the deduction math.

Items 11 Through 13: Workers’ Comp, Retirement, and Leave

Item 11 asks whether the employee has ever filed a workers’ compensation claim, and if yes, the date. Item 12 covers retirement plans: list the plan type, current account balance, and whether an early-withdrawal penalty applies (and its amount).

Item 13 applies only if the employee is temporarily on leave. Fill in the date last worked, date last paid or expected to be paid, the gross amount of that paycheck, and the expected return date. If the employee was terminated, use the separate termination block at the bottom: check whether the person was fired, quit, or left for another reason, and enter the termination date, final paycheck date, and the gross final amount including any vacation or sick pay.2Cabinet for Health and Family Services. Certification

Sign and date the form at the bottom, print your name, and provide your title, phone number, and business address. The certification says the information is true and correct to the best of your knowledge. The person who actually pulled the payroll data should sign, rather than a manager who didn’t review the records.

Returning the Completed Form

You have three options for getting the PAFS-700 back to DCBS:

  • Mail it to the caseworker address printed on the form, or to DCBS Family Support, P.O. Box 2104, Frankfort, KY 40602.
  • Fax it to (502) 564-9810 and keep the transmission confirmation.
  • Upload it directly to the applicant’s case record through benefind.ky.gov.

If the form arrived with a return envelope or a local DCBS fax number, use that instead. Routing it to the caseworker who requested it is faster than going through the statewide mailbox.3Kentucky Cabinet for Health and Family Services. Division of Family Support

There is no specific statutory deadline for employers to return the form. Turnaround still matters: the applicant’s benefits can’t be finalized until the caseworker has your numbers, SNAP decisions are due within 30 days of filing, and low-income applicants may qualify for expedited processing within a few days.1kynect. Getting a Blank Form

Most employers receive the PAFS-700 already pre-filled with the employee’s identifying information, either by mail from DCBS or hand-delivered by the employee. A Note on Accuracy

The certification you sign is not a formality. Under KRS 194A.505, anyone who knowingly makes a false statement or hides a material fact to influence a public-assistance decision can face criminal charges, with the severity rising with the dollar amount of benefits wrongly obtained.5Kentucky Legislative Research Commission. Kentucky Revised Statutes 194A.505 – Prohibited Activities – Commencement of Proceedings for Enforcement The statute primarily targets applicants and providers, but an employer who knowingly falsifies wage data on the PAFS-700 could be prosecuted under it as well. Report the gross wages your payroll records actually show; if you’re uncertain about a figure, verify before signing.
