Every absence from your LAUSD schedule has to be documented on one of two certification forms: Form 60.ILL if the absence involved illness, injury, or a new child, and Form 60.NON-ILL for every other reason. Both are governed by Policy Bulletin BUL-6307.5, and both must be completed, signed, and approved before your time can be reported to payroll.1Los Angeles Unified School District. Certification of Absence Forms (BUL-6307.5) You sign the LAUSD absence form under penalty of perjury, so the details matter.
Which Form You Need
Pick the form that matches the reason you were out. Choose the wrong one and your time reporter will send it back.
Use Form 60.ILL for your own illness or injury, a family member’s illness, or the birth, adoption, or foster placement of a child.1Los Angeles Unified School District. Certification of Absence Forms (BUL-6307.5)
Use Form 60.NON-ILL for any of the following:
- Accident or imminent danger to your personal property, or damage to a family member’s property
- Automobile breakdown when you need your car for that day’s work duties (bargaining units A, B, C, D, and S only)
- Registration or a final exam for higher education courses (bargaining units A, C, and S only)
- A religious holiday of your faith
- Court appearance or jury duty under court order
- Attending your child’s school activity
- Bereavement
- A district-approved conference
- Vacation, including regular accrued hours or 1994 Vacation Bank hours
- Paid Vacation Parental Leave for a new child, drawn from accrued vacation or the 1994 Vacation Bank
- Other absences provided for in your collective bargaining agreement or Personnel Commission Rules
Some reasons are restricted to particular bargaining units, so check your own agreement if you aren’t sure a category applies to you.1Los Angeles Unified School District. Certification of Absence Forms (BUL-6307.5)
How to Complete the Form
Both forms share the same three-part layout: employee information at the top, reason for absence in the middle, and administrator approval at the bottom. The only real difference is the printed list of reasons.
Employee Information
Fill in your name, employee number, work location, job title, and telephone number. Your employee number appears on your pay stubs and in the MyLAUSD portal. Enter the work location where you were actually scheduled on the day you missed, not your home school if you were on temporary assignment somewhere else.1Los Angeles Unified School District. Certification of Absence Forms (BUL-6307.5)
Reason for Absence
Enter the first date of the absence and the last date (or expected return date if you’re submitting the form before coming back), along with the total time missed in days or hours. Then select the specific reason from the options printed on the form. You don’t need to look up any time or pay codes; your site’s time reporter translates your selection into the district’s official codes using the Payroll Concepts Manual.1Los Angeles Unified School District. Certification of Absence Forms (BUL-6307.5)
Some reasons require advance filing. Religious holidays, court appearances, and school visitations must be submitted on the form at least five working days ahead, and your administrator may take reasonable steps to confirm that the stated reason actually existed.2United Teachers Los Angeles. Signature and Date
Sign and date the form at the bottom. This isn’t a formality. BUL-6307.5 states that your signature is made under penalty of perjury, confirming that everything you’ve written is truthful. The same signature also authorizes the district to recover any unearned wages from your next paycheck if you don’t have enough accrued leave to cover the absence.1Los Angeles Unified School District. Certification of Absence Forms (BUL-6307.5) A missing signature or date is the most common reason forms get sent back.
When You Need Medical Documentation
A short illness doesn’t require a doctor’s note. Longer absences do.
If you’re out for more than five consecutive working days because of illness, injury, or disability, you have to submit either a Form 60.ILL completed by your attending physician or a physician’s statement on letterhead attached to Form 60.ILL. Your own signature on the form is still required.2United Teachers Los Angeles. UTLA Proposal Article XII Leaves and Absences
If the absence stretches past 20 consecutive working days, you’ll also need to file a formal leave request and a separate Attending Physician Statement form.2United Teachers Los Angeles. UTLA Proposal Article XII Leaves and Absences Even for shorter absences, the district reserves the right to verify any claimed illness before authorizing pay, so a supervisor can ask for documentation whenever questions come up.
Family medical leave carries its own paperwork. When you’re caring for a child, spouse, or parent with a serious health condition, you need a health care provider’s certification that includes the date the condition began, its expected duration, and a statement that your participation in care is warranted.2United Teachers Los Angeles. UTLA Proposal Article XII Leaves and Absences The administrator section of Form 60.ILL has a field confirming that FMLA supporting documentation is on file, and getting that box checked matters: during FMLA-qualified unpaid leave your district health benefits continue, whereas going into unpaid status without FMLA protection can end district contributions for that pay period.3Los Angeles Unified School District. Where to Get the Form
Both forms are available through the MyLAUSD portal. Log in at my.lausd.net with your district email and password, which lands you on the e-Library Documents page. Search “60.ILL” or “60.NON-ILL,” check the box next to the document, and download it. Print and complete the form before you submit.1Los Angeles Unified School District. Certification of Absence Forms (BUL-6307.5) Your school’s main office usually keeps printed copies as well. Use the current version only; BUL-6307.5 directed every time reporter to destroy earlier editions when the revised forms took effect.
How to Submit It
Turn the completed form in to your site administrator or the time reporter at your work location. BUL-6307.5 requires a signed and approved form on file before absence time can be reported to payroll, so submit promptly after returning.1Los Angeles Unified School District. Certification of Absence Forms (BUL-6307.5) Your administrator reviews the form and signs the approval section, then the time reporter uses the information to process payroll.
One important boundary: reporting your absence through SmartFind Express is a separate step from filing the Form 60.4Los Angeles Unified School District. Classified Employment Services – Help For Current Employees – Create an Absence SmartFind Express tells the district you’ll be out and triggers substitute coverage; it does not replace the signed certification. You need to do both: log the absence through SmartFind Express and submit the signed Form 60 when you return.
What Your Signature Commits You To
The authorization line on the form is easy to skim past, and it has real paycheck consequences. When you sign, you agree that if you don’t have enough accrued benefit time to cover the absence, the district will deduct the unearned wages from your next paycheck.1Los Angeles Unified School District. Certification of Absence Forms (BUL-6307.5) The district doesn’t warn you before it happens. The adjustment is automatic once the time reporter processes the form and the system sees insufficient accruals.
Before submitting a Form 60 for an extended absence, check your current leave balances. Your time reporter can tell you what you have available, and balances are often visible through MyLAUSD. If the math doesn’t work, talk to your administrator about whether FMLA-protected unpaid leave or a formal leave of absence would be a better route than draining accruals and triggering a payroll deduction.
