How to Fill Out the Maryland Emissions Waiver Form (EP-001A)

The Maryland emissions waiver form, officially Form EP-001A, lets you request an exemption from VEIP testing if your vehicle failed inspection and you meet one of three sets of conditions set by state regulation. You complete the form, attach the right supporting documents, and submit it online through myMVA, by mail to the MVA at 6601 Ritchie Highway NE, Glen Burnie, MD 21062, or in person at a VEIP station.¹

Confirm You Qualify Before You Fill Anything Out

Waivers fall into three categories under COMAR 11.14.08.07, and you must meet every element of at least one of them.²

Repair Waiver

Your vehicle has failed its emissions inspection and you have spent at least $450 on emissions-related repairs performed by a recognized repair technician.³ The $450 combines parts and labor, but only for repairs directly tied to the reason your vehicle failed. Routine maintenance like oil changes, tires, or brakes doesn’t count, even if you had it done on the same visit. A receipt for a catalytic converter won’t help if the failure was an evaporative system leak.²

Senior Citizen Waiver

Every owner listed on the registration is 70 or older at the time of the scheduled inspection, and the vehicle is driven 5,000 miles or fewer per year. If you co-own with a spouse or family member under 70, the waiver will be denied.¹

Disability Waiver

The vehicle is already registered with disability plates issued under Maryland Transportation Article §13-616 and driven 5,000 miles or fewer per year.² All owners on the registration must meet the disability requirements.⁴ If the vehicle doesn’t already carry disability plates, you need to apply for them through the MVA before requesting this waiver.

What Goes on Form EP-001A

The form is short, but the MVA is strict about accuracy. It warns that an inexact odometer reading alone can result in a denial.¹ You’ll enter:

Title number from your Maryland certificate of title. The MVA uses this to identify the vehicle in the waiver system, not the VIN or the tag number.
Current odometer reading, taken directly from the dashboard when you sign. Rounding or estimating is a common cause of denial.
Signatures of every owner listed on the registration. For senior and disability waivers, each of those owners must individually qualify.
The date you sign.
An email address and phone number so the MVA can reach you for follow-up.

The form includes a certification statement. By signing, you confirm under penalties of law that everything you provided is true and correct.

Documents to Attach

The form by itself is rarely enough. Missing paperwork is the quickest way to a denial.

For a repair waiver, attach original itemized invoices or receipts for every emissions-related repair. Each one should show the repair facility’s name, the date of service, the parts used, and the labor performed. The total across all receipts must reach $450, and the work must have been done by a recognized repair technician.² Photocopy everything before you submit; the MVA typically keeps the originals during review.

For senior and disability waivers, the odometer reading on the form is the key piece of evidence. The MVA reserves the right to verify what you provide, which can include comparing your reading against previous inspections or registration data.² Disability plates are confirmed through the MVA’s own records.

How to Submit the Waiver

There are three routes for the completed EP-001A and supporting documents:

Online through the myMVA portal at mymva.maryland.gov, which is the fastest.¹
By mail to VEIP, Motor Vehicle Administration, 6601 Ritchie Highway NE, Glen Burnie, MD 21062.¹
In person at a VEIP station, where you hand the form and original receipts to a supervisor. For a repair waiver, you can also complete the request printed on the back of your emissions inspection certificate at the station itself.⁵

Online is worth using if you can scan your receipts clearly. Mailed applications take longer because of postal transit on top of the review period. Make copies of everything before it leaves your hands, whichever method you pick.

Active-Duty Military Stationed Out of State

If you’re active-duty military stationed outside Maryland, the MVA may accept another state’s emissions test result instead of a VEIP test. To request a waiver on that basis, include a completed Certified Statement (Form EP-009) along with your EP-001A.⁶

After You Submit

Your registration status sits in a pending state while the application is reviewed. The MVA sends a formal notification by mail with the decision. If something is missing, the letter tells you what to resubmit.

A denial letter includes the reason. Common grounds are odometer readings that don’t match previous records, repair receipts that don’t add up to $450, repairs done by someone who isn’t a recognized technician, or a co-owner who doesn’t meet the age or disability requirements. If the problem is fixable, such as a missing receipt, you can gather the documentation and resubmit.

How Long the Waiver Lasts

An approved waiver covers the rest of your current test cycle, which in Maryland runs every two years.⁵ When that cycle ends, the waiver expires and the vehicle must go through a new inspection.⁶

The waiver is tied to the specific vehicle and its current ownership. If you sell or transfer the vehicle, the waiver does not carry over. Senior and disability waiver holders reapply each cycle, confirming the mileage cap and that all owners still qualify. For a repair waiver, if the vehicle fails again in the next cycle, the $450 threshold resets and previous repair costs don’t carry forward.
