When This Form Applies
Use the post-service form only for treatments already performed. If the dispute is over a treatment that has not happened yet — a decision point review or precertification denial — the pre-service appeal form applies instead. There is no hard deadline to file a post-service appeal, but the regulation requires that you submit it at least 45 days before initiating PIP arbitration or filing in Superior Court. A medical-necessity denial and a reimbursement-amount dispute count as separate appealable issues even when they involve the same service on the same date. Completing the Form
The form has four sections. Every field needs to match the insurer’s records exactly. A transposed digit in a claim number or date of birth gives the carrier a clean procedural bounce.
Claim Information
Enter the date you are submitting the appeal and the date you received the adverse decision. Then the insurance company’s name, the PIP claim number assigned to the accident, and the date of loss. The claim number appears on the Explanation of Benefits (EOB) that the carrier sent. Check it character by character.
Patient Information
Provide the patient’s last name, first name, middle initial, date of birth, and full mailing address, matching what the insurer has on file. If the name on the policy includes a suffix or hyphenation, reproduce it exactly.
Provider and Facility Information
Enter the treating provider’s name, facility or office name, specialty, Tax ID, and National Provider Identifier (NPI), along with your full address, phone, fax, and email. Post-Service Appeal Issues
This is the substantive grid. For each disputed line, enter the EOB ID, total billed amount, expected amount, dates of service, CPT/HCPCS/NDC codes, and line-level reimbursement amounts. Two sets of appeal reason codes are required: bill-level codes numbered 1 through 10 and line-level codes lettered A through S. The key on the back of the form defines each code (medical necessity, fee schedule dispute, coding disagreement, and so on). Sign and date the provider statement at the bottom. The form carries a fraud warning: knowingly filing false or misleading information exposes you to criminal and civil penalties.
Attachments

- The original bill, meaning the CMS-1500 (HCFA) or UB-04 claim form you first submitted to the carrier.
- The Explanation of Benefits or Payment showing the denial or reduction you are challenging.
- An appeal rationale narrative explaining why the denial or reduction was wrong, with clinical reasoning when medical necessity is at issue.
Include the following when they are relevant to the specific dispute:
- The APTP (Automobile Personal Treatment Plan) decision or response, if the dispute involves one.
- The Independent Medical Exam report, if the carrier relied on an IME.
- The peer review report, if the denial was based on peer review, together with a written rebuttal in your narrative.
- An audit report for billing and coding disputes.
- A network termination document, if the issue is tied to a terminated provider agreement.
- The PPO contract, if the reimbursement dispute turns on a contracted rate.
Cross-reference each medical record and document to specific line items and dates of service. A disorganized packet forces the reviewer to guess which record supports which line, and guesses rarely help the appellant.
Where and How to Submit
Send the completed form and attachments to the address or fax number that the insurer designates for appeals in its Decision Point Review Plan. That is not necessarily the same address you use for regular claims. Check the DPR Plan materials or call the carrier’s PIP department to confirm. Some insurers also allow electronic filing when their DPR Plan sets out a process for it.1Legal Information Institute. If you mail the appeal, use certified mail with return receipt. The delivery date starts the 30-day clock, and you will want proof if the carrier later claims late or missing delivery.
After You File
The insurer has 30 days from the date it receives the appeal and supporting documents to issue a written decision. The response will uphold the original denial, offer partial payment, or reverse the denial and pay in full. Insurers can only require one level of internal appeal per issue, so a denial at this stage exhausts the internal process for that issue and opens the door to PIP arbitration through Forthright, the organization designated by the state to administer no-fault dispute resolution.1Legal Information Institute.
