New Hampshire’s Form DSMV 30 is the Division of Motor Vehicles’ all-purpose record change request, and one submission updates your driver’s license, vehicle registration, and title records together. Use it to change your address, legal name, date of birth, physical description, organ donor status, or veteran indicator. Under RSA 263:9, you have 30 days to notify the DMV in writing after a legal name or address change.1New Hampshire General Court. New Hampshire Code 263:9 – Notice of Change of Licensees Name or Address Some changes you can drop off or mail; others require an in-person appointment.
One change the form does not handle is a gender marker update. That runs through the Application for Duplicate (DSMV 637) and also requires an in-person appointment.2NH Division of Motor Vehicles. Important Appointment Information/Required Documents
What Each Section of DSMV 30 Does
The form has five numbered sections, and you only complete the ones that apply.3New Hampshire Division of Motor Vehicles. New Hampshire DMV Record Change Request Form DSMV 30
- Section 2 handles address changes. You can update your mailing address, legal (residential) address, or both, check a box if they’re the same, and request that your legal address be printed on the back of your license. A separate checkbox in this section lets honorably discharged veterans add a veteran designation.
- Section 3 is for a legal name change. Name changes cannot go through a drop box or the mail; you have to appear at a DMV office.
- Section 4 covers other personal identifiers: date of birth, height, weight, eye color, and hair color. A date of birth correction, like a name change, requires an in-person visit.
- Section 5 is organ donor status. One box consents to donation under RSA 263:41 and RSA 291-A; a separate box revokes a prior consent.
Filling Out the Form
Download DSMV 30 from the NH DMV website or pick one up at any DMV office. Read the header first: every change you enter will apply across all your DMV records, so only fill in fields you intend to change permanently.
Section 1 is required no matter what else you’re doing. Enter your current first, middle, and last name exactly as they appear on your license, your date of birth, your driver’s license or non-driver ID number, a phone number, and an email. This is how the DMV locates your existing record.
Then move to the single section that matches your change. For an address update, fill in your new mailing and legal addresses in Section 2, or check the box indicating they match. For a name change, print your new legal name in Section 3. For a physical description correction or birthdate fix, use Section 4. Sign and date the certification at the bottom; you’re signing under penalty of unsworn falsification.
What to Bring, by Change Type
The documents you need vary sharply depending on what you’re updating.

The National Voter Registration Act Of 1993 (NVRA) Submitting DSMV 30 does not change your voter record. New Hampshire allows same-day registration at polling places, but to update beforehand, contact your town or city clerk.
