How to Fill Out the NJ BME-MEV Medical Education Verification Form

The NJ BME-MEV form is the medical education verification that every physician applicant files with the New Jersey State Board of Medical Examiners. You complete a short applicant section at the top, then send the form to the registrar at every medical school you have ever attended. The registrar fills in your enrollment and graduation details, attaches an official transcript, and returns both directly to the Board.

What You Fill In at the Top

The applicant portion is brief. You provide your full name, the name of the medical school, the school’s complete mailing address (street, city, state, zip code, and country), and the school’s telephone number with area code.1New Jersey Office of the Attorney General. Medical Education Verification Form That’s all you fill in. The MEV itself doesn’t ask for a Social Security number or date of birth; your main license application collects those separately.

Write your name exactly as it appears on your license application. If a school holds your records under a former name, call the registrar before sending the form so staff can find your file. A mismatch slows the Board’s ability to match the returned verification to your application.

What the Registrar Completes

The rest of the form belongs to your school’s registrar. The registrar confirms your enrollment dates (month and year started, month and year finished), your graduation date in month-and-year format, and the degree earned.1New Jersey Office of the Attorney General. Medical Education Verification Form The form uses month and year only, not exact dates.

The registrar also has to report whether you took any leaves of absence, were placed on probation, faced disciplinary action, were subject to any investigation, received negative reports from instructors, or had special requirements imposed during your enrollment. The registrar then signs the form and affixes the school’s official seal. Without both the signature and the seal, the Board will not accept the verification as authentic.

One Form for Every School You Attended

A common mistake is sending the MEV only to the school that granted your degree. The Board requires a separate form for every medical school in which you were ever enrolled, even if you received no credit or did not seek credit for the study.2Rutgers New Jersey Medical School. Instructions for Completing the License Application for a New Jersey Medical License If you transferred, sat in on coursework, or started at one institution before finishing at another, each school needs its own copy. Write your name at the top of each one and send it to the right registrar.

Official Transcript Must Come With It

The form alone is not enough. The registrar must return the completed MEV together with an official transcript of your medical school record.1New Jersey Office of the Attorney General. Medical Education Verification Form When you contact each registrar, make this clear so the office doesn’t send the form without the transcript or send the two separately. Both should arrive at the Board as a single package.

How the Form Reaches the Board

The registrar sends the completed form and transcript directly to the Board. You cannot submit the form yourself. If the Board receives the MEV from you rather than from the school, it will not be accepted.2Rutgers New Jersey Medical School. Instructions for Completing the License Application for a New Jersey Medical License That direct-from-the-source requirement is what gives the verification its credibility.

The current form instructs registrars to return the completed MEV and transcript by email to BMEApp@dca.njoag.gov.1New Jersey Office of the Attorney General. Some registrars are used to mailing paper verifications and may need a reminder that the Board now accepts electronic delivery.

If You Trained Outside the United States

Foreign medical graduates still need a BME-MEV completed by each foreign school attended. The form’s address block includes a country line for that purpose.1New Jersey Office of the Attorney General. Medical Education Verification Form Getting a foreign registrar to complete and return the form often takes longer than a domestic school, so start early.

You also need certification from the Educational Commission for Foreign Medical Graduates (ECFMG), sent directly to the New Jersey Board.2Rutgers New Jersey Medical School. Instructions for Completing the License Application for a New Jersey Medical License The ECFMG certification is a separate request to ECFMG and does not substitute for the MEV.

Don’t Confuse It With the BME-VPT

The BME-MEV covers your medical school education. A separate form, the BME-VPT (Verification of Postgraduate Training), covers residency, fellowship, internship, and any other postgraduate training you took part in after earning your degree.2Rutgers New Jersey Medical School. Instructions for Completing the License Application for a New Jersey Medical License You need both. The VPT goes to every training program you participated in, whether you completed it or not, and the program sends it directly to the Board. Unlike the MEV, the VPT does not require an official transcript.

Confirming the Board Received It

After you submit your main license application, you can track which verification documents the Board has received through the MyLicense portal at newjersey.mylicense.com. Log in and use the “View Checklist” link to see what has been checked off and what remains outstanding.3MyLicense.
