How to Fill Out the Pennsylvania DL-180R Driver’s License Application

The Pennsylvania DL-180R form is the application you use to transfer a non-commercial driver’s license from another state to Pennsylvania. You complete it and bring it in person to a PennDOT Driver License Center, along with proof of identity, two documents showing your Pennsylvania address, your current out-of-state license (which PennDOT keeps), and payment. There is no mail-in option.1Commonwealth of Pennsylvania. Moving to Pennsylvania Commercial drivers use a different form, the DL-180RCD.2Commonwealth of Pennsylvania. Who Can Use the DL-180R

You qualify if you already hold a valid driver’s license from another U.S. state, or one that expired within the last six months, and you are establishing residency in Pennsylvania. You must be at least 16½ years old. If your previous license has been expired longer than six months, PennDOT treats you as a first-time applicant instead, which brings in a knowledge test and possibly a road test.3Pennsylvania Department of Transportation. What to Bring to the Driver License Center

A missing document means a wasted trip. Gather everything below before you go.

Proof of Identity

If you are a U.S. citizen, bring your original Social Security card (it cannot be laminated) plus one of the following: a birth certificate with a raised seal, a Certificate of U.S. Citizenship (USCIS Form N-560), a Certificate of Naturalization (USCIS Form N-550 or N-570), or a valid U.S. passport.

Non-U.S. citizens must bring all three of these: original USCIS or immigration documents showing current lawful status, a valid passport, and either a Social Security card or an SSA ineligibility letter.3Pennsylvania Department of Transportation. Two Proofs of Pennsylvania Residency

If you are 18 or older, you need two documents showing your name and your current Pennsylvania street address. P.O. boxes do not count. PennDOT accepts options such as:

  • A current lease agreement or mortgage document
  • A W-2 form or recent pay stub
  • Official tax records
  • A computer-generated utility bill
  • A Pennsylvania vehicle registration card or auto insurance card
  • Postmarked mail or package labels

Your Out-of-State License

Bring the physical license. PennDOT will take it during the visit, and no Pennsylvania non-commercial license will be issued until you surrender all valid licenses from any other state.3Pennsylvania Department of Transportation. Name Change Documents, If Applicable

If your current legal name does not match the name on your identity documents, bring the original (not a photocopy) of one of these: a certified marriage certificate issued by the county court where the marriage license was processed, a court order granting a name change, adoption records, or an amended certified birth certificate with a raised or embossed seal.4Commonwealth of Pennsylvania. Filling Out the Form

Download the current DL-180R from the PennDOT Driver and Vehicle Services website or pick one up at any Driver License Center. You can complete most of it ahead of time.

Section A: Personal Information

Enter your full legal name, date of birth, Social Security Number, and current Pennsylvania mailing address. These entries must match your identity documents exactly. A misspelling or an outdated address is the most common reason applications stall. If you have ever been issued a Pennsylvania driver’s license or photo ID before, include that number too.3Pennsylvania Department of Transportation. Section B: Exam Report

Leave this one blank. A PennDOT examiner fills it in at the center, recording your vision screening results and certifying the license class and any endorsements. You do not take a written or road test when transferring, unless PennDOT decides your driving record warrants one.

Section C: Parental or Guardian Consent

This section applies only to applicants under 18. A parent, legal guardian, person in loco parentis, or spouse who is at least 18 must sign it to authorize the application.3Pennsylvania Department of Transportation. Section D: Health and Medical Certification

Answer the questions about neurological, circulatory, cardiac, and other health conditions that could affect safe driving. This section also lets you request a veterans designation on your license. Answer honestly. PennDOT may require a medical evaluation if your responses show a condition that affects driving.

Signature, Organ Donor, and Gender Designation

Sign and date the authorization at the bottom. You are certifying under Section 3709 of the Pennsylvania Vehicle Code (unsworn falsification to authorities) that everything you provided is true. The form also includes optional sections to register as an organ donor and to select your gender designation.3Pennsylvania Department of Transportation. At the Counter

The examiner reviews your paperwork, runs the vision screening, takes your out-of-state license, and processes payment. If everything checks out, PennDOT issues your Pennsylvania license at the center that day.

Things That Can Block or Delay Your Transfer

An Out-of-State Suspension or Revocation

Before issuing a license, PennDOT checks the National Driver Register, a federal database that flags people whose driving privileges have been suspended, revoked, or denied in any state.7National Highway Traffic Safety Administration. National Driver Register (NDR) If you have an unresolved suspension or revocation elsewhere, PennDOT will not issue your Pennsylvania license until the originating state clears your record. Resolve those issues before you visit a Driver License Center. This is where people waste the most time in the transfer.

A License Expired More Than Six Months

Past the six-month mark, the DL-180R transfer path closes and you apply as a new driver, with the testing that entails.3Pennsylvania Department of Transportation. Application for Pennsylvania Non-Commercial Driver’s License by Out-of-State Non-CDL Driver (DL-180R)

Applicants Under 18

You must be at least 16½, and a parent or guardian needs to sign Section C in person. Pennsylvania’s graduated licensing restrictions may also apply, including limits on nighttime driving and the number of passengers, depending on the type of license you held in the previous state.

REAL ID

If you want your new Pennsylvania license to be REAL ID–compliant (marked with a gold star), your identity and name change documents need to meet the federal REAL ID standards. The documents listed above generally satisfy both, but a certified marriage certificate specifically must come from the county court where the marriage license was processed.4Commonwealth of Pennsylvania.
