The PSEO Notice of Student Registration form, better known as the NOSR, is Minnesota’s three-part registration document that links a high school student’s record to their college course enrollment so the state can reimburse the college for tuition, fees, and course materials. The student fills out the first section, a high school counselor fills out the second, and the college fills out the third and sends the completed form to the Minnesota Department of Education (MDE).1Minnesota Department of Education. Get the Right Version of the Form
MDE posts the current NOSR for each academic year on its PSEO finance page. For 2026–27, three versions are available: a fillable PDF, a Word document, and a version with a digital signature field.2Minnesota Department of Education. Check the header before you start writing on it. The academic year is printed at the top, and submitting a prior year’s form can delay processing at MDE.
Notify Your District First
Before the NOSR comes into the picture, state law requires you to tell your school district you intend to enroll in PSEO. There are two hard deadlines:
- May 30 for the following fall semester.
- October 30 for the following spring semester.
The statute puts it plainly: “A pupil is bound by notifying or not notifying the school district, charter school, or Tribal contract school by October 30 or May 30.”3Minnesota Office of the Revisor of Statutes. Minnesota Statutes Section 124D.09 Miss it and your district can deny PSEO for that term. Ask your counselor what format the district wants; some use an intent form, others accept an email.
Section 1: Student Information
You (or a parent, if you’re under 18) complete Section 1. The fields are straightforward, but a few trip people up.
- Legal name, birthdate, and home address.
- State Student ID. The form labels this field “SSID.” It’s a 13-character code assigned by your school district. If you don’t have it on hand, ask your counselor or check your school’s parent portal.1Minnesota Department of Education. Minnesota PSEO Notice of Student Registration Form
- School name and type: public, nonpublic, or home school.
- Term of attendance. Pick the semester or quarter. Quarter-system schools list Qtr 1, Qtr 2, and Qtr 3 as options.
- Signatures. The student signs. If the student is under 18, a parent or guardian signs too.
Section 2: High School Verification
A counselor or authorized staff member at your secondary school fills out Section 2. Their signature certifies that you meet PSEO eligibility requirements and that the grade level, expected graduation year, and related details you entered are accurate.1Minnesota Department of Education. Homeschool students don’t have a counselor to sign here. Coordinate with the college’s PSEO office on how this section gets handled in your case.
Section 3: Postsecondary Institution
The college completes Section 3, and only after you’ve actually registered for courses. It lists each course by number and credit count, with space for up to six courses per term. An authorized contact at the institution signs to confirm you’re enrolled in those specific courses for that term.1Minnesota Department of Education. How the Form Moves and Gets Submitted
A common mistake is for students to think they’re supposed to send the finished NOSR to MDE. They aren’t. Only the postsecondary institution submits the completed form to the Minnesota Department of Education.1Minnesota Department of Education. Minnesota PSEO Notice of Student Registration Form The workflow goes like this:
- Complete Section 1 and bring (or send) the form to your high school counselor.
- The counselor completes Section 2 and returns the form to you.
- You deliver the partially completed form to the college’s PSEO office or student services before registering for courses.4Normandale Community College. PSEO Form
- After you register, the college completes Section 3 and sends the form to MDE.
Watch your college student portal to confirm your registration posted. That’s the clearest sign the paperwork is moving on the funding side.
You Need a New Form Each Term
One NOSR covers one term. If you take PSEO courses in the fall and want to continue in the spring, start the process over with a fresh form and updated course listings. Public school students should also remember the May 30 and October 30 notifications are tied to each term you plan to enroll.
Who Can Use the Form
The NOSR is only useful if you’re PSEO-eligible in the first place. Eligibility is set by Minnesota Statutes section 124D.09 and depends on your grade and the type of course:3Minnesota Office of the Revisor of Statutes. Minnesota Statutes Section 124D.09
- 11th and 12th graders at a Minnesota public, charter, or tribal contract school may apply for any nonsectarian college course.
- 10th graders may enroll in a career and technical education (CTE) course at a Minnesota State college or university if they passed the 8th-grade MCA in reading. The first semester is limited to one CTE course; a C or higher opens additional courses for the rest of 10th grade.
- 9th and 10th graders can enroll in other situations only when both the high school and the postsecondary institution agree, or when the course is a world language course already available to 11th and 12th graders.
Foreign exchange students in a cultural exchange program are not eligible. Private-school and homeschool students can participate, but they must register annually with the Minnesota Commissioner of Education through MDE’s nonpublic and homeschool page before using the NOSR.5Minnesota Department of Education.
