How to Fill Out the Texas Child Support Direct Deposit Authorization Form

The Texas Child Support Direct Deposit Authorization Form (OAG Form 6A002e) is what you file to have your child support paid straight into a checking or savings account instead of arriving by paper check or on the smiONE Visa Prepaid Card. You can download it from the Office of the Attorney General’s child support forms page or request a copy at 1-800-252-8014.¹ The form is one page, and once you have your court order and a recent bank statement in front of you, it takes only a few minutes to complete.

What to Have on Hand Before You Start

Missing or mismatched details are the most common reason forms come back unprocessed. Pull these together first:

The county-assigned cause number from your child support court order. This is not the same as your OAG case number.
Your Social Security number.
The payor’s full name, exactly as it appears on the court order.
Your account number and the nine-digit ACH routing number, plus the bank or credit union’s name, address, and phone number.

The routing number has to be the one your bank uses for ACH transfers, not a wire routing number. The number printed on your personal checks is almost always the ACH number, but if you are unsure, confirm on a recent statement or with the bank directly.²

How to Complete Each Section

At the top of the form, choose Start, Change, or Stop. For first-time enrollment, check Start.²

In the financial institution section, mark whether the account is checking or savings, then enter the account number, the nine-digit routing number, and the bank’s name, address, and phone number. Verify every digit against your statement. A single wrong number in either the routing or account field will cause the deposit to fail and force you to resubmit.

The payee section asks for your full name (last, first, middle), the payor’s full name, the cause number, your mailing address, work and home phone numbers, and your Social Security number. Your name must match the name on your child support case exactly.

Sign and date the bottom. Your signature confirms several things, including that you authorize the OAG to recover any overpayments from future deposits and that the authorization remains in effect until you cancel it in writing. The form also states that providing false information violates Texas Penal Code Section 37.10.²

Where to Send the Completed Form

The completed form goes to the Texas Child Support Disbursement Unit (TXCSDU) by one of three channels:³

The PO Box for the direct deposit form (659400) is not the same as the PO Box where child support payments are mailed (659791).⁴ Sending the form to the payment box will delay it. If you fax or email, save the confirmation page or sent-mail record.

How Long It Takes to Take Effect

Allow 30 days from the date TXCSDU receives your form for direct deposit to start.² Payments do not stop during that window. They keep arriving through whatever method you use now, whether that is a paper check or the smiONE Visa Prepaid Card, until the new deposit instructions go live.⁵

If the bank rejects the account, whether because of a wrong routing number, a closed account, or a name mismatch, TXCSDU will notify you. You will need to correct the form and send it in again, and the 30-day clock restarts. Verifying every field against a recent statement before submitting is the easiest way to avoid this.

Changing Banks or Stopping Direct Deposit

If you switch banks, close the account you are using, or want to go back to paper checks or the smiONE card, submit a new Form 6A002e with the Change or Stop box checked. A phone call is not enough. The OAG requires a written request for any change or cancellation.²

Download a fresh copy from the OAG website or the Customer Self-Service Portal, or call 1-800-252-8014 to have one sent. Fill in your updated information, or leave the bank fields blank if you are stopping direct deposit entirely and want checks mailed, and return it to TXCSDU using the same mail, fax, or email options above. Skipping the written step can send payments to a closed account, creating a gap that takes time to trace and fix.
