Utah Form TC-69C is the notice you file with the Utah State Tax Commission to change or close an existing business tax account — updating your address, business name, officers or owners, or shutting down one or more tax accounts entirely. Download the current form at tax.utah.gov, complete the sections that apply, attach any required Department of Commerce paperwork, sign it, and send it to Master Records by mail or fax. When To Use TC-69C
TC-69C is only for changes to a business that is already registered with the Tax Commission. If you are registering a new business, use Form TC-69 through tap.utah.gov. One carve-out to know about: you cannot use TC-69C to change the address of a cigarette, tobacco, e-cigarette, or cannabinoid products location. What the Form Covers
The form has two parts. Part A handles changes that affect the whole business. Part B handles changes to a single sales location, such as closing one storefront while others stay open. Through Part A, you can report:
- A new physical address, mailing address, or both, applied to the account types you specify (sales and use tax, withholding, corporate, mineral production withholding, or motor fuel).
- A change to your DBA or legal business name.
- Officers, partners, or members added or removed, with Social Security numbers for anyone added.
- Closure of one or more tax accounts, including sales tax, withholding, transient room tax, motor vehicle rental tax, restaurant tax, waste tire recycling fee, mineral production withholding, and fuels tax.
- Any other business change not listed elsewhere on the form, in Section A7.
Filling Out the Form
Identifying Your Account
At the top of page one, enter the information the Tax Commission already has on file so it can match your submission to your account: your Utah tax account number, your Federal Employer Identification Number, and your current legal business name exactly as it appears in state records. Include a phone number. If the Tax Commission can’t locate your account from what you submitted, a representative will call.1Utah State Tax Commission. Marking the Affected Accounts
Check the box for every tax account type the change applies to — sales and use tax, corporate or partnership, withholding, mineral production withholding, motor fuel. An address change for a business that holds both a sales tax and a withholding account needs both boxes checked, or the update won’t carry across.1Utah State Tax Commission. Completing the Change Sections
In Part A, fill in only the sections that apply. For an address change (Section A2), enter the full street address, city, state, and ZIP, and indicate whether it is your physical location, mailing address, or both. For a name change (Section A5), write the new DBA or business name. For officer or owner changes (Section A6), give the full name, address, and Social Security number of anyone being added, and list anyone being removed.1Utah State Tax Commission. For closures (Section A1), mark each tax type you want to close. Closure isn’t complete until you have filed every outstanding return for every period the account was open, so treat the TC-69C as the notice, not the finish line. Signing
An authorized person — a president, partner, member, or owner — has to sign and date the form. An unsigned TC-69C gets rejected, and this is the most common reason a submission comes back.1Utah State Tax Commission. TC-69C Notice of Change for a Business and/or Tax Account
Documents You May Need To Attach
Several changes require paperwork from the Utah Department of Commerce along with the TC-69C:
- For a name change, attach the Department of Commerce Articles of Incorporation, unless you are a sole proprietor.
- For an officer or owner change, attach the Department of Commerce Registration Information Change Form matching your entity type; there are separate versions for corporations, LLCs, limited partnerships, and other structures.
- For a corporate account closure, attach a copy of your Articles of Dissolution from the Department of Commerce.

Online Through TAP
Address changes and account closures can go through the Taxpayer Access Point at tap.utah.gov instead of the paper form. Address changes submitted through TAP update immediately.3Utah State Tax Commission. TAP FAQ – About TAP Log into your account and look for the option to update account information. Name changes and officer changes still need the paper form because of the required Commerce attachments.
Update the IRS Separately
Filing TC-69C updates only your Utah records. The IRS keeps its own database tied to your EIN, and a change to your business address, location, or responsible party needs a separate IRS Form 8822-B. Responsible-party changes have to reach the IRS within 60 days.4Internal Revenue Service.
