When the DMV Asks for This Form

The most common trigger is failing the vision screening at a DMV counter. If you can’t read the 20/40 line in at least one eye, or your peripheral vision measures below 70 degrees, you’ll be handed an MV3030V and told to see a specialist.¹ The Medical Review & Fitness Unit may also mail you a request after a report from a law enforcement officer, physician, or family member raises concerns about your ability to see while driving.²

The form is also the tool for removing a vision-related restriction already on your license. The top of the form is yours. Complete it before your appointment so your provider only has the clinical portion to worry about. What the Provider Records

Everything below the patient section is clinical. Your provider will document:

Visual acuity for each eye, both corrected and uncorrected, in Snellen 20/ notation.
Temporal (peripheral) field of vision in degrees, for each eye.
Whether you can distinguish standard red, amber, and green traffic signal colors.
Any eye condition affecting one or both eyes that is expected to worsen, and what it is.
Recommendations for driving: corrective lenses, no freeway or interstate driving, driving limited to a set radius (with a mileage figure), daylight only, and whether a DMV driving evaluation is advised.
An opinion on whether you’re safe to operate a non-commercial vehicle, commercial vehicle, or school and passenger bus.³

Those recommendations are advisory. The final licensing decision rests with the Secretary of Transportation.³

One timing rule matters: the exam must have been conducted within the past 90 days, or since a date the DMV specified in your request letter. A report based on an older exam will be rejected.³

Who Can Sign It

Only five categories of licensed provider can sign the MV3030V:

Doctor of osteopathic medicine (DO)
Optometrist (OD)
Certified physician assistant (PA-C)
Advanced practice nurse prescriber (APNP)

The Standards Your Numbers Are Measured Against

Regular Class D license: at least 20/100 acuity (corrected or uncorrected) in at least one eye, and at least a 20-degree horizontal temporal field in at least one eye.⁵
Commercial driver license: at least 20/60 acuity in at least one eye and at least a 70-degree field in at least one eye.⁵
School bus or passenger endorsement: at least 20/40 acuity in each eye and at least a 70-degree field in each eye.⁵

Bioptic telescopic lenses are allowed to help meet the acuity number, but they can’t be used to reach the field-of-vision minimum for your license class.⁶ A Class D applicant at 20/100 in one eye can qualify, but restrictions such as daytime-only driving are all but certain at that level.

What the DMV Does With Your Form

The DMV mails its decision. If your vision clears the unrestricted standard, nothing changes. If you meet the minimum but fall short of the unrestricted mark, your license comes back with one or more restrictions. Common ones include corrective lenses required, daylight driving only, no freeway or interstate driving, a set-radius limit from home, and outside mirrors required. These mirror the checkboxes on the form, though the DMV makes the final call regardless of what the provider recommended.¹ The DMV may also require you to pass a knowledge, signs, or road test before restoring full privileges.

If your vision falls below the absolute minimum for your license class, or if the form doesn’t come back within 30 days, the DMV cancels the license.¹

If You’re Denied or Canceled

A cancellation isn’t the end of the road. Wisconsin has a three-step appeal path, and your cancellation letter will spell out what you need to do to get reinstated.⁷

The first step is submitting updated medical reports. If you’ve had corrective surgery since the original exam, or your provider believes the earlier findings didn’t capture your true capability, the Medical Review & Fitness Unit will reconsider with the new information.⁷
