How to Find and Fill Out CDSS Forms: Submission, SAR 7, and Appeals

The California Department of Social Services publishes every form it uses, for free, on its Forms and Brochures page, and most can be submitted online through BenefitsCal or dropped off at your county welfare office. CDSS forms carry a prefix that tells you which program they belong to, so once you know the prefix you can usually find the right document in a minute or two.1California Department of Social Services. Where to Find CDSS Forms

The CDSS Forms and Brochures page is the central source. You can browse an alphabetical list or filter by program.2California Department of Social Services. On-line Forms and Publications A – D The prefix on each form number identifies the program:

  • CF — CalFresh
  • CW — CalWORKs
  • SOC — social services programs, including IHSS
  • AR — reports of changes
  • DPA — hearing and disqualification matters

Many forms are available in Spanish, Chinese, Vietnamese, and other threshold languages.1California Department of Social Services. Forms and Brochures

BenefitsCal, the state’s online portal, is the other place to work with these forms. An account lets you apply for CalFresh, CalWORKs, and health coverage programs, upload supporting documents, track your case, and receive notices electronically.3BenefitsCal. Home | BenefitsCal. The Forms Most People Need

The right form depends on which benefit you want and whether you are starting out or already enrolled.

SAWS 2 Plus is the combined application for CalFresh, CalWORKs, and Medi-Cal. Most people start here because one form opens the door to all three programs. To begin a CalFresh application, you need only your name, address, and signature; CalWORKs requires questions one through five plus your signature.4California Department of Social Services. Application for CalFresh, Cash Aid, and/or Medi-Cal/Health Care Programs

CF 285 is the standalone CalFresh application for people who only want food assistance. It is available in accessible formats, including large print and braille.

SOC 295 is the application for In-Home Supportive Services. It goes to your county IHSS office, not through BenefitsCal. A county social worker will then schedule a home visit to assess what services you need and how many hours to authorize.5California Department of Social Services. In-Home Supportive Services (IHSS) Program If you apply for IHSS, a licensed health care professional also has to complete the SOC 873 certifying that you cannot safely perform certain daily living activities on your own.6California Department of Social Services. SAR 7 is not an application. It is the semi-annual eligibility status report that current CalFresh and CalWORKs recipients file to keep benefits going.7California Department of Social Services. How to Fill Out Your SAR 7 Eligibility Status Report More on it below.

What to Have Ready Before You Fill One Out

Whichever form you are using, the county will want a full picture of your household. List every person living with you, with full legal names, Social Security numbers, and dates of birth. Household size drives the income limits that apply and the size of any benefit you receive.

Report all income for everyone in the household. That means earned wages plus unearned income such as Social Security, unemployment insurance, veteran’s benefits, child support, and workers’ compensation. If you are self-employed, report both your income and your business expenses.8California Department of Social Services. Application for CalFresh, Cash Aid, and/or Medi-Cal/Health Care Programs

Monthly expenses count too. The form asks for your rent or mortgage, utilities, and childcare. Use exact figures. Numbers that do not match what the county can pull from state and federal databases will trigger a request for more documentation or a denial.

A finished form is only half the work. The county will not finalize your case until it sees documents backing up what you wrote. Gather these before you start:

  • Identity for each adult applicant — driver’s license, state ID, or passport.4California Department of Social Services. Application for CalFresh, Cash Aid, and/or Medi-Cal/Health Care Programs
  • Residency — rental agreement, utility bill, or official mail with your California address.
  • Income — pay stubs from the last 30 days, or a recent IRS Schedule C if self-employed. Award letters or benefit statements for unearned income.9California Department of Social Services. Request for Verification
  • Assets — recent bank statements for everyone in the household.
  • Immigration status, if applicable — Alien Registration Card, visa, or other proof of lawful status for non-citizen applicants.8California Department of Social Services. Application for CalFresh, Cash Aid, and/or Medi-Cal/Health Care Programs
  • Birth certificates for anyone applying for cash aid.

Label every document with your case number if one has already been assigned. If you are uploading through BenefitsCal, make sure your scans are legible. Blurry or cut-off images are a common reason counties ask for a resubmission.

How to Submit a Completed Form

You have three filing channels, and all three carry the same legal weight.

Online through BenefitsCal. Create an account at benefitscal.com, complete the application there, and upload your documents. The portal confirms receipt right away and lets you track the case.10BenefitsCal. Reporting Features Awareness Update

By mail. Send the packet to your local county welfare department. Use certified mail or a tracked service so you have proof of the mailing date. The filing date controls when your benefits start if the application is approved.

In person. Drop the forms off at the county office. Ask for a date-stamped receipt, or use the secure drop box if one is available.

One exception: the SOC 295 for IHSS goes directly to your county IHSS office rather than through BenefitsCal.5California Department of Social Services. In-Home Supportive Services (IHSS) Program

Faster CalFresh for Households in Crisis

If your household is in immediate financial distress, you may qualify for expedited CalFresh. The county must then issue benefits within seven calendar days of your application instead of the standard 30.11Food and Nutrition Service. SNAP Application Processing Timeliness You qualify if any one of the following applies:

  • Your household’s gross monthly income is below $150 and your liquid resources (cash and bank accounts) are under $100.12eCFR. 7 CFR 273.2
  • Your combined monthly gross income and liquid resources are less than your monthly rent or mortgage plus utilities.
  • You are a destitute migrant or seasonal farmworker with liquid resources under $100.12eCFR. 7 CFR 273.2

Tell the county at submission that you believe you qualify. The sooner they get your identity and income verification, the sooner benefits post to your EBT card.

After You File

Once your application is logged, the clock starts. CalFresh applications must be processed within 30 days of the filing date.13California Department of Social Services. All County Information Notice No. I-23-12 CalWORKs has a 45-day window.14California Department of Social Services. Application Processing Time Frame Requirements If CalFresh is approved, your benefits are prorated back to the date you submitted the application, not the date the county decided.15Food and Nutrition Service. SNAP Eligibility

Most applications include an interview. For CalFresh, every applicant is interviewed, usually by phone. A face-to-face interview is required only if you ask for one, if another program needs it, or if the county decides it is necessary to verify eligibility. For IHSS, the interview is an in-home assessment with a county social worker.5California Department of Social Services. In-Home Supportive Services (IHSS) Program

You will then receive a Notice of Action in the mail explaining whether benefits were approved, denied, or whether more information is needed. The notice also appears in your BenefitsCal account.3BenefitsCal. Home | BenefitsCal. Together, we benefit.

Keeping Benefits Going with the SAR 7

Once you are approved for CalFresh or CalWORKs, you have to file the SAR 7 Eligibility Status Report once a year, six months after your initial application or your annual renewal.7California Department of Social Services. How to Fill Out Your SAR 7 Eligibility Status Report Your county will tell you which month is yours.

The form asks about changes in household composition, address, income, and expenses since your last report. Sign and date it after the last day of the report month; signing earlier makes the form incomplete.16California Department of Social Services. SAR 7 Eligibility Status Report – For Cash Aid and CalFresh It is due by the 5th of the following month. If the county has not received it by the 11th, it is late. If a complete report has still not arrived by the first business day of the month after that, your case will be discontinued and you will have to reapply.

Missing the SAR 7 is one of the most common reasons people lose benefits they still qualify for. Set a reminder as soon as you learn your report month, and file through BenefitsCal if you can so you have a timestamped confirmation.

If the County Says No

If the county denies your application, cuts your benefits, or closes your case, the Notice of Action will explain the reason and your right to a state hearing. You can request one by contacting CDSS directly.17California Department of Social Services. State Hearing Requests You generally have 90 days from the date on the notice.

There is a shorter deadline worth knowing about. If you request the hearing within 10 days of the notice date, your benefits continue at their current level while you wait for the decision. This is called aid paid pending, and it applies as long as your certification period has not expired and the county has not issued a separate notice about a different change. If your benefits are stopping specifically because you did not submit a SAR 7, aid paid pending generally does not apply unless you can show the form was actually turned in.
