How to Get a Driver Abstract in NY: Online, Mail, or In Person

To get a driving abstract in NY, order it through the MyDMV online portal for $7, or submit Form MV-15C by mail or in person for $10. The abstract shows your license status, traffic convictions, points, and accident history, and the DMV issues it in three versions: Standard, Lifetime, and Commercial Driver License (CDL).

Pick the Right Version First

Ordering the wrong version means paying again, so decide what you actually need before you start.

The Standard abstract is what most people want for insurance shopping or a routine employment check. It carries most traffic convictions and accidents until the end of the year they occurred plus three more years. Suspensions and revocations show for four years after they ended. Alcohol- and drug-related offenses stay longer.

The Lifetime abstract contains everything the DMV still has on file, including items that have already aged off the Standard version. It may not reach all the way back to your first license, but it goes further than the Standard. Order this one when a court or licensing authority asks for your full record.

The CDL abstract includes everything on a Standard abstract plus the extra data commercial drivers need: convictions and licensing actions from every state in any vehicle type, and medical certification status with examiner details, certification dates, and any vision or hearing variances.

All three versions are available online through MyDMV. At a DMV office, the MV-15C form covers a Standard abstract request.

Order Online Through MyDMV

MyDMV is the cheapest and fastest route at $7. After payment, you can download a PDF of your Standard, Lifetime, or CDL abstract immediately, and it stays in your account for five days.¹

Using MyDMV requires a NY.gov ID account. Setting one up asks for your name, email, and mailing address. Then you link the account to MyDMV using:

Your 9-digit DMV Client ID number
The 8- or 10-digit document number from your most recently issued New York license, learner permit, or non-driver ID
The last four digits of your Social Security number
Your date of birth
The ZIP code the DMV has on file for you

If you have never held a New York license, permit, or non-driver ID, you cannot register for MyDMV and will need to use one of the offline methods below.²

Order by Mail

The mail method costs $10 and uses Form MV-15C, available as a PDF on the DMV website.³ Complete the form and send it with a check or money order for $10 made payable to “Commissioner of Motor Vehicles.” The mailing address is printed on the form. Build in extra time for postal delivery and processing on top of what an online order would take.

Order in Person at a DMV Office

In person is also $10 and also uses Form MV-15C. Bring the completed form to any DMV office along with proof of identity. Acceptable ID includes a driver license or other government-issued photo ID, or six points of identification as described on the DMV’s ID-44 form. You can pay by cash, check, credit card, or money order.¹

What the Abstract Actually Shows

A New York abstract is divided into sections. The summary lists your license class, current status (valid, suspended, or revoked), expiration date, restrictions or endorsements, and suspension and revocation history.

The convictions section lists each traffic violation you were convicted of, with the type of violation, the date it happened, the conviction date, the location, the fine amount, and any points assessed. The accidents section lists each reported accident by date, whether there was a fatality, personal injury, or property damage, the county where it occurred, and the case number.¹

How Long Items Stay on the Record

Timing matters when you choose between Standard and Lifetime, and it matters if you are trying to time an insurance switch.

On a Standard abstract, most traffic convictions and accidents display until the end of the calendar year in which they occurred, plus three additional years. A conviction from June 2026 would drop off at the end of 2029.

Alcohol- and drug-related offenses run longer. A DWI conviction stays on a Standard abstract for 15 years from the conviction date. A DWAI conviction stays for 10 years. Suspensions and revocations generally appear for four years after they ended, and a suspension for refusing a chemical test shows for five years from the suspension date.¹

A Lifetime abstract keeps everything the DMV still has in its system. If you need to show a clean recent history, the Standard version helps you. If the request is for a full history, the Lifetime is the one to order.

Points and PIRP Do Not Erase From the Document

Points assessed for a violation appear on the abstract next to the conviction. Completing a DMV-approved Point and Insurance Reduction Program (PIRP) course reduces up to 4 points from the internal total the DMV uses to decide whether to suspend your license, but the points do not physically leave the document. They still show on the abstract; the DMV simply stops counting them toward the 11-point suspension threshold. The reduction only applies to violations from the 18 months before you completed the course, and you can use PIRP for point reduction once every 18 months.⁴

So a PIRP course is worth taking if you are near a suspension, but do not order a new abstract expecting the entries to be gone.

Check It Before You Hand It Over

Read the abstract carefully before you send it to an employer or insurer. If it contains something wrong, like a conviction that belongs to another driver or a suspension that was already resolved, contact the DMV Call Center to start a correction. Errors are easier to fix before they cost you a job offer or an insurance quote than after.¹

When Someone Else Wants to Pull Your Record

Third parties like employers, insurers, and attorneys use the same MV-15C form, but they must identify a “permissible use” under the federal Driver’s Privacy Protection Act. The form lists the allowable reasons, including use in a court or administrative proceeding, insurance claims investigation or underwriting, and employer verification for CDL holders.³

If none of the listed exemptions applies, the requester can still obtain the record with your written consent, and the DMV provides Form MV-15GC for that purpose. Without a permissible use or your written consent, the DMV will not release the record.⁵

Employers who use your driving record as part of a hiring decision through a background screening company must also follow the Fair Credit Reporting Act. They need to give you a separate written disclosure and get your written authorization before pulling the report, and they must notify you and give you a chance to respond if the report leads to an unfavorable decision.
