What to Put on Form TC-123
Form TC-123 is a PDF on the Utah Tax Commission website.4Utah.gov. TC-123 Application for Utah Duplicate Title Fill it out in blue or black ink. Four details have to be exact, or the application comes back:
- The full 17-character VIN (or hull identification number for a boat). Altered or incorrect numbers get the application rejected.
- The license plate number currently on the vehicle.
- The full legal name and current residential address of every owner on the title record.
- Signatures from every owner shown. If the owners are joined by “and,” both must sign; if joined by “or,” either one can sign alone.
Bring a valid government-issued photo ID so staff can verify you. The fee is $6, set by statute.5Utah Legislature. Utah Code 41-1a-1210 – Fees for Original and Duplicate Certificates of Title Confirm the year, make, and model on your form match what the state has on file; small discrepancies will delay the application and can cause problems the next time you sell or transfer the vehicle.
Three Ways to Submit
Online
The fastest route is Utah’s Motor Vehicle Portal (MVP). It walks you through the application, takes the $6 fee electronically, and mails the new title to the address on record.1Utah State Tax Commission. Replace Title – DMV No office visit, no paper form.
In Person
Any local DMV office or participating county assessor’s location will take the completed TC-123, verify your ID, and collect the fee. The advantage is immediate feedback: if something doesn’t match state records, you fix it before leaving. The title itself is still printed centrally and mailed, so you won’t walk out with it.
By Mail

If a Lender Is Still on the Title
An active loan complicates things. Many Utah lenders use the Electronic Lien and Title system, meaning no paper title exists while the loan is open; the lender holds the electronic record and is the party that would request a duplicate.6American Association of Motor Vehicle Administrators (AAMVA). Electronic Lien and Title
If the loan is paid off but the lien still appears on your record, you need a formal release before Utah will issue a clean duplicate. The lienholder can sign the release section on TC-123, or provide a signed letter on official letterhead confirming the debt is satisfied.4Utah.gov. TC-123 Application for Utah Duplicate Title Lenders can take weeks to process a release, so start that request early.
If a buyer is already lined up and both the owner and lienholder provide proper releases, Utah can skip the duplicate step and title the vehicle directly to the new owner.3Utah Legislature. Utah Code 41-1a-518 – Duplicate Titles
If the Owner Can’t Sign
When the owner isn’t able to handle it personally, someone else can apply using a power of attorney. Form TC-737 (Power of Attorney and Declaration of Representative) authorizes a third party to act on the owner’s behalf; it must identify the specific vehicle and be signed by the owner.7Utah State Tax Commission. TC-737 Power of Attorney and Declaration of Representative The representative then signs TC-123 and submits everything with the $6 fee.
If the Owner Has Died
Utah lets a surviving family member transfer a vehicle without full probate by using Form TC-569C, the Survivorship Affidavit.8Utah State Tax Commission. TC-569C Survivorship Affidavit All of the following must be true:
- At least 30 days have passed since the death.
- No personal representative has been appointed or applied for.
- The decedent left no more than four boats, motor vehicles, trailers, or semi-trailers.
- The rest of the estate (excluding vehicles, minus debts) is worth $100,000 or less.9Utah Courts. Informal Probate
If the estate doesn’t meet those conditions, the vehicle has to go through probate, and the court-appointed personal representative applies for the duplicate.
What Arrives, and What Happens to the Old One
Expect the new certificate in about a week. It will have “duplicate” printed on its face.3Utah Legislature. Utah Code 41-1a-518 – Duplicate Titles Issuing a duplicate automatically invalidates every previously issued certificate for that vehicle, so if the original turns up later, it’s legally worthless. Shred it to avoid confusion the next time the vehicle changes hands.
