To get a medical marijuana card in Oklahoma, you need a recommendation from an Oklahoma-licensed physician, proof that you live in the state, a photo ID, and an online application submitted to the Oklahoma Medical Marijuana Authority (OMMA) with a $104.30 fee. OMMA has 14 business days to approve or deny you, and everything is handled through its online portal.
Who Qualifies
Oklahoma is unusual among medical marijuana states in that it does not publish a list of qualifying conditions. The decision belongs to your physician. In practice, recommendations are common for chronic pain, epilepsy, cancer-related symptoms, anxiety, PTSD, multiple sclerosis, and severe nausea, but nothing in the law restricts a physician to those categories.

Step 1: Get a Physician Recommendation
The physician recommendation is the step that gates everything else. Your doctor must hold a valid, unrestricted license with one of three Oklahoma boards: the State Board of Medical Licensure and Supervision, the State Board of Osteopathic Examiners, or the State Board of Podiatric Medical Examiners.3Oklahoma Medical Marijuana Authority. Telehealth is allowed, so you can complete the visit by video from home. One rule to be aware of: as of January 1, 2026, physicians cannot conduct these appointments, including virtual ones, from a location at the same physical address as a dispensary.4Oklahoma Medical Marijuana Authority (OMMA). After the evaluation, the physician completes and signs the Adult Patient Physician Recommendation Form. Pay attention to the timing: your OMMA application must be submitted within 30 days of the date the physician signs the form. Physician evaluation fees are separate from the OMMA fee. Most providers charge somewhere between $75 and $200, and the price does not correlate with the quality of the evaluation. Shopping around is reasonable.
Step 2: Gather Your Documents
Before you open the online application, put together clear, legible color copies of everything you will upload. Missing or unreadable documents are the most common reason applications get delayed.

- An Oklahoma driver’s license or state ID card, front and back
- A utility bill from the previous calendar month (cell phone and internet bills are not accepted)
- A residential property deed in Oklahoma
- A current rental agreement for an Oklahoma residence
You also need proof of identity, which can be an Oklahoma driver’s license, Oklahoma ID card, U.S. passport, U.S. Bureau of Indian Affairs ID card, or an approved tribal identification card. An Oklahoma driver’s license can serve as both proof of residency and proof of identity.
Round out the file with your signed physician recommendation form, a clear color full-face digital photograph for your license card, and, if you qualify for the reduced fee, documentation showing your Medicaid (SoonerSelect), Medicare, or 100% disabled veteran status.
Step 3: Apply Online and Pay the Fee

- Standard: $100 plus a $4.30 processing fee, for a total of $104.30.
- Reduced: $20 plus a $2.50 processing fee, for a total of $22.50, if you are enrolled in Medicaid (SoonerSelect) or Medicare, or you have 100% disabled veteran status.
All fees are nonrefundable, even if OMMA denies your application.
How Long Approval Takes
OMMA is required by law to process patient applications within 14 business days. You receive an email letting you know whether you have been approved or denied. How Long Your Card Lasts
A standard adult patient license is valid for up to two years. Oklahoma also offers a 60-day license for patients whose treatment plan calls for marijuana over a shorter period. The 60-day license carries the same application fee and requirements as a two-year license.1Oklahoma Medical Marijuana Authority. Patient Licenses
Renewal is not automatic. You will need a fresh physician recommendation and a new application with the same fee. Do not let your card lapse if you rely on it, because possessing marijuana without a valid license is a criminal offense even if you previously held one.
Minor Patients
Children under 18 can be licensed, but the rules are tighter. A minor’s application must include recommendations from two separate Oklahoma-licensed physicians, not one, and a parent or legal guardian has to sign the application and submit their own proof of identity and Oklahoma residency.2Justia. Visiting from Another State
If you live elsewhere and want to buy medical marijuana while in Oklahoma, OMMA issues a 30-day temporary license. You must already hold a valid state-government-issued medical marijuana license from your home state; a doctor’s recommendation letter or prescription is not enough on its own.1Oklahoma Medical Marijuana Authority. The fee is $104.30, the same as the standard in-state rate, and no reduced rate is available. You upload a color copy of your home-state medical marijuana license, a photo ID, and a digital photograph. Out-of-state applicants can apply for a new temporary license starting one week before their current one expires.
Designating a Caregiver
If you cannot easily purchase or administer marijuana yourself, you can designate a caregiver to buy, transport, possess, and administer it on your behalf. A caregiver must be an Oklahoma resident at least 18 years old. Each adult patient can have one designated caregiver; minor patients may have up to two.1Oklahoma Medical Marijuana Authority. Your physician must certify on the recommendation form that a caregiver is necessary, and both you and the caregiver sign a Caregiver Designation Form. The caregiver then submits their own application with proof of identity, residency, and a photo. There is no application fee for a caregiver license, and it lasts until the patient’s license expires, up to two years.
What Your Card Does Not Cover
An Oklahoma medical marijuana card does not override federal law, and two areas trip people up often enough to flag before you apply.
Firearms
Federal law makes it illegal for any user of a controlled substance to possess firearms or ammunition, and marijuana is still a Schedule I controlled substance federally. ATF Form 4473, which you complete when buying a firearm from a licensed dealer, asks directly whether you are “an unlawful user of, or addicted to, marijuana or any depressant, stimulant, narcotic drug, or any other controlled substance,” and warns that marijuana use remains unlawful under federal law regardless of state legalization.7US Bureau of Alcohol, Tobacco, Firearms and Explosives. DOT-Regulated Jobs
If you hold a commercial driver’s license or work in another position subject to Department of Transportation drug testing, a medical card will not protect you. The DOT has stated that state medical marijuana laws have no bearing on federal drug testing requirements, and a physician’s marijuana recommendation is not a valid medical explanation for a positive test.9US Department of Transportation.
