If you want something outside those combinations, such as an entirely new surname unrelated to either spouse, the marriage certificate is not enough. You’d need to file a formal name change petition through the Utah courts instead.
Step 1: Get Certified Copies of Your Marriage Certificate
Every agency you deal with will want to see a certified copy, so this document is the starting point. Step 2: Update Your Social Security Card
Social Security has to come first, because almost every other agency will want confirmation that the federal record already reflects your new name. Utah is not one of the states where the SSA currently allows a fully online marriage name change, so you’ll either visit a local Social Security office or start the application online and then bring the documents to an in-person appointment.5Social Security Administration. There is no fee. Your new card arrives by mail in 5 to 10 business days, and the agency returns your marriage certificate separately.7Social Security Administration. Change Name With Social Security The SSA gives you a receipt at the appointment, which you can use as proof at the Driver License Division while you wait for the card.
Step 3: Update Your Utah Driver License
You have to appear in person at a Utah Driver License Division office to change the name on your license or ID card.8Utah Driver License Division. If you hold a REAL ID compliant license, bring every original document you used to obtain it. Each document in the chain has to connect logically, and the marriage certificate is what fills the gap between the name on your birth certificate and your new one.
Step 4: Update Your U.S. Passport
The right form depends on the timing between your passport issue date and your name change.
If less than a year has passed since your passport was issued and since your name was legally changed, use Form DS-5504. If more than a year has passed since either the passport was issued or the name changed, and you meet renewal-by-mail eligibility, use Form DS-82. Step 5: Update Everything Else
Vehicle Title and Registration
If you own a car, update the title through the Utah Division of Motor Vehicles. Banks, Credit, and Voter Registration
Banks and credit card issuers usually want a certified marriage certificate plus your updated driver license before they’ll change account names.
