Who Can Order

You qualify if you are:

  • The person named on the certificate.
  • An immediate family member or in-law in the direct line of descent, up to but not including cousins. That covers parents, grandparents, siblings, children, and their spouses.
  • A guardian or foster parent, with your guardianship or custody paperwork.
  • A stepparent, who can receive a copy of a legitimate birth certificate by stating the relationship.
  • An authorized representative (an attorney, physician, or other agent) acting with a signed statement from the registrant or a family member.

Others may qualify by showing the record is needed to determine or protect a personal or property right. The state registrar can ask for additional ID or a notarized sworn statement in that case.

What to Have Ready Before You Start

The online form asks for the same core information as the paper application: the full name on the certificate (plus any alternate names the birth might have been recorded under), date of birth, and the city and county of birth. One warning worth taking seriously: KBA fails more often than people expect. Thin credit histories, recent moves, and frozen credit reports are common reasons the system can’t match you to its questions.
