To order an Oregon birth certificate, submit an application, a copy of your ID, and payment to Oregon Vital Records through one of five channels: online or by phone through VitalChek, by mail, through a secure drop box in Portland, or at an in-person appointment. A certified copy costs $25 at the base rate, with online and phone orders running up to $49.20 because of vendor and expedite fees. Same-day pickup is possible in person; mail and drop box orders take 8 to 10 weeks.
Who Can Order
Oregon restricts birth records for 100 years after the date of birth, so you have to prove you’re eligible before the state will release a certified copy. You must be at least 18 to order, except that a registrant ordering their own certificate only needs to be 15.1Oregon Public Law. ORS 432.380 – Issuance of Certified Copies of Records
The following people can request a certified copy:2Oregon Health Authority. Order a Birth Certificate
- The person named on the certificate (the registrant), if at least 15
- A parent, stepparent, spouse, registered domestic partner, child, grandchild, sibling, or grandparent of the registrant
- A legal guardian, with a copy of court guardianship papers
- A licensed attorney representing the registrant or another eligible person, with documentation of that authority
- Federal, state, local, or tribal agencies acting officially, with a letter on official letterhead
If your name doesn’t appear on the certificate and your relationship isn’t clear from the paperwork, plan to prove that relationship. A notarized permission note from an eligible person can sometimes work when you don’t qualify on your own.
What to Gather Before You Apply
The application asks for the full name on the birth record, the date of birth, the city and county of birth, and both parents’ full names including the mother’s maiden name. You’ll also give your own name, mailing address, phone number, and relationship to the registrant. The official form lives on the Oregon Health Authority website.
Every applicant must include a copy of valid, unexpired ID. Acceptable photo IDs include a current driver’s license, permit, or state ID card; a U.S. passport or passport card; a tribal photo ID; a U.S. Armed Services ID; or a permanent resident card.3Oregon Health Authority. No government photo ID? Submit copies of three different documents instead, at least one showing your current address. A recent utility bill, a bank statement, and a voter registration card would satisfy this. Expired IDs are never accepted.4Oregon Health Authority. If You Need the Certificate for Use Abroad
An ordinary certified copy may not be enough for use in another country. Countries that participate in the Hague Apostille Convention require an apostille from the Oregon Secretary of State. You’ll need an original certified copy (not a photocopy), a completed Request for Authentication Form from the Secretary of State’s website, and a $10 fee per apostille. If you’re ordering through VitalChek specifically for this, you can have the certificate shipped directly to the Secretary of State’s Corporation Division. The office also accepts documents via FedEx or UPS for faster turnaround.8Oregon Secretary of State. How to Get an Authentication (or Apostille)
