How to Renew Your CDS Registration in Maryland: Steps and Fees

To renew your CDS registration in Maryland, first register with the state’s Prescription Drug Monitoring Program through CRISP, then submit your renewal application electronically at egov.maryland.gov/mdh/cds and pay the $120 fee. The Office of Controlled Substances Administration (OCSA) accepts renewals as early as 60 days before your certificate’s expiration date and up to 30 days after it.¹

When to Renew

A Maryland CDS certificate is valid for up to three years and expires on the date printed on the certificate.² OCSA sends a renewal notice at least 30 days out, but not receiving one doesn’t excuse a late filing.³

File early. If your application reaches OCSA at least two weeks before your expiration date, your current registration stays in effect while the renewal is processed. Miss that two-week buffer and you risk a gap in prescribing authority even if you technically applied on time.³

Register With the PDMP First

Before OCSA will process any renewal, you must be registered with Maryland’s Prescription Drug Monitoring Program.⁴ This is a hard prerequisite, and it catches many providers off guard. Access runs through CRISP (Chesapeake Regional Information System for our Patients), and you’ll need to submit your PDMP email confirmation, including its confirmation code, as part of the renewal.

PDMP registration is free and takes about half an hour. Register at the CRISP website using your personal email address, and have your DEA number, CDS number, NPI, and professional license number ready.⁵ Prescribers, pharmacists, and delegates each have their own registration category on the portal. If you prescribe opioids or benzodiazepines, Maryland law also requires you to query the PDMP before writing those prescriptions, so the account gets used either way.

What to Gather Before You Apply

Have these ready before you log in. Mismatches between your application and the underlying licensing databases will stall processing.

Your Maryland professional license number, active and in good standing with your board.
Your current DEA registration number.
Your existing CDS registration number from your current or most recent certificate.
The business address where you handle controlled substances. If it has changed, file an amendment ($50) rather than trying to change it through the renewal.⁶
Your PDMP confirmation email and code from CRISP.

Submitting the Renewal Online

All Maryland CDS applications, new and renewal, must be submitted electronically through egov.maryland.gov/mdh/cds.⁴ Log in with your existing credentials and go to the renewal section for your practice type.

The form asks for your professional license details, DEA number, and CDS number in the corresponding fields, then moves into disclosure questions about disciplinary actions, investigations, or changes in practice status since your last registration. Answer these carefully. Inaccurate or incomplete responses can trigger a review that delays approval or leads to a denial. Review everything before you submit, then save or print the confirmation and tracking number.

Fee and Exemptions

The renewal fee is $120 for the full three-year term, whether you’re a practitioner, pharmacy, hospital, or research institution.⁶ The portal accepts major credit cards and electronic checks.

You may owe nothing if you’re an employee (not a contractor) of a hospital, clinic, or other facility operated by a local or state government agency. Exemptions also apply to certain agencies where the state covers the fee with the Secretary’s approval, and to persons covered under 21 CFR § 1301.21.⁶ Amending a registration is $50, and reinstatement after a lapse adds $50 on top of the renewal fee.

How Long Processing Takes

OCSA commits to determining whether your application is complete within 3 days and issuing a final decision within 23 days.⁴ In practice, plan for three to four weeks for practitioner renewals, and four to five business weeks for establishment renewals once mailing time is factored in.⁷

If your current registration is still valid and you filed at least two weeks before expiration, you can keep prescribing during processing. Once approved, you’ll get an electronic notification and can download the updated certificate from your portal account. Keep the new certificate posted at your registered practice location.

If You Miss the Deadline

Maryland law requires an active registration to manufacture, distribute, or dispense a controlled dangerous substance in the state.⁸ Prescribing under an expired CDS registration violates state law even if your professional license and DEA registration are still active.

Reinstatement costs $50 in addition to the $120 renewal fee.⁶ During any lapse you have no authority to handle controlled substances, which creates immediate problems for patient continuity, pharmacy holds on pending prescriptions, and potential attention from your licensing board.

Your DEA Registration Is Separate

The Maryland CDS and federal DEA registrations are separate credentials on separate three-year cycles, and they don’t align automatically. A valid Maryland CDS means nothing without an active DEA number behind it, and if your DEA lapses you have only one calendar month to reinstate it before you have to apply for an entirely new registration. Federal law bars handling controlled substances during that grace period.⁹

When the two renewals fall close together, handle the DEA first, because your active DEA number is a prerequisite for the Maryland application. At DEA renewal you’ll also affirm completion of the one-time, eight-hour MATE Act training on treating patients with opioid and other substance use disorders, required of all DEA-registered practitioners except veterinarians.¹⁰ The DEA has confirmed that once you’ve attested to the training, it won’t appear on future renewals.¹¹
