How to Renew Your NC CNA Registration: Form, Hours, and Status

To handle your NC CNA renewal, you and a Registered Nurse each submit a free online form through the North Carolina Division of Health Service Regulation (DHSR) portal at ncnarforms.ncdhhs.gov. Your Nurse Aide I Registry listing updates within three to five business days after both forms are approved.¹ Paper renewal forms have not been accepted since August 31, 2021.

Who Has to Renew and How Often

Federal and state law require anyone working as a nurse aide in a North Carolina nursing home to hold an active listing on the Nurse Aide I Registry.² Each listing lasts 24 consecutive months. The clock is measured from the last date of qualified nurse aide work reported on your Employment Verification Form, with the expiration extended to the last day of that month.³

The registry mails reminders about three months before a listing expires. Not receiving one does not excuse a late renewal. Tracking your own expiration date is your responsibility.

The Eight-Hour Work Requirement

To qualify to renew, you must have worked at least eight hours as a paid nurse aide during your current 24-month listing period, performing duties delegated and supervised by a Registered Nurse.⁴ The work does not have to happen in a nursing home. Home health, hospitals, and assisted living can all count, provided the duties fall within the Nurse Aide I task list maintained by the N.C. Board of Nursing.²

Volunteer hours do not count. Neither does work an RN cannot back up with employment records or direct knowledge.⁵

What to Have Ready Before You Log In

The online form will not let you continue with blank required fields. Gather this first:

Your first and last name exactly as they appear on the registry
The last four digits of your Social Security Number (the system does not ask for the full number)
Your date of birth
Your six-digit Nurse Aide Listing Number, assigned when you were first added to the registry
The name and contact information of the RN who will verify your employment

The verifying RN does not have to be your direct supervisor. Any RN with access to your facility’s employment records and the ability to confirm your work dates and duties can complete the verification.⁵ Line up that person before you start. The registry will not process your renewal until the RN submits their separate form.

Filling Out Your Online Renewal Form

Go to ncnarforms.ncdhhs.gov and open the Nurse Aide Online Renewal Form. Enter your identifying information exactly as it appears on your current registry listing. Even a small name mismatch can block the system from pulling up your record.

Once the system finds you, the form walks through confirming your personal and employment details. There is no fee.⁵ After you submit, the portal generates a confirmation. Save or print it. That is your only proof of submission if something goes wrong later.

Your part is done. The renewal is not.

The RN Employment Verification Form

This is the step people miss. You cannot submit the employment verification yourself. You have to ask a Registered Nurse to complete it separately. The RN logs into their own page on the same portal, the Nurse Aide I Employment Verification by a Registered Nurse form, and confirms that you performed nurse aide duties for pay under RN supervision during your listing period.¹

The RN needs the last four digits of your SSN and your six-digit Nurse Aide Listing Number to find your record. Their signature confirms that paid, supervised nurse aide work happened. It is not a clinical competency endorsement.²

If you work outside a nursing home, check well before your renewal window whether an RN on staff has the records and willingness to complete this form. Aides in home health and assisted living sometimes find out too late that no one at their workplace can verify their hours.

Processing Time and Checking Your Status

Once both forms are in, DHSR reviews and approves the renewal, and your listing updates within three to five business days.¹ Your new 24-month period runs from the last date of qualified work on the verification form, extended to the end of that month.³

Verify it yourself once you expect processing is complete. The public registry search tool at ncnar.ncdhhs.gov lets anyone look up a nurse aide’s status using the last four digits of the SSN and the name.⁶ Employers use the same tool, so the updated listing doubles as your proof of active certification. Confirm the new expiration date before you assume the renewal went through. Working with an expired listing can lead to termination and regulatory trouble for the facility that employs you.

What Happens If Your Listing Lapses

If you do not complete at least eight hours of qualified work during any 24-month period, your listing is removed from the registry. Federal regulations require the state to remove any nurse aide who has not performed nursing or nursing-related services for 24 consecutive months.⁷ Removal is not a temporary hold.

Getting back on the registry after a lapse means starting from scratch: a new state-approved Nurse Aide I training program and another passing score on the competency evaluation.² That costs money and weeks of time, so an accidental lapse is expensive.

The Training Waiver Option

There is one narrow way out if you are close to expiration and have not logged qualifying work. You can submit a Training Waiver Application at least 45 days before your listing expires and pass the competency exam before the expiration date. No extensions to the 45-day window are granted.³ The waiver skips retraining but not the exam, so it only helps if you act early and are confident in your skills.

Updating Your Name or Address

The registry can only send reminders and correspondence to the address it has on file. If you have moved or changed your name, submit a Registry Verification and Change Request Form through the DHSR portal before your renewal comes due.⁸ A name change typically requires supporting legal documentation. A missed renewal notice due to an outdated address leads directly to the lapse scenario above.

Coming From Another State

If you hold an active nurse aide certification in another state and want to work in North Carolina, this is not a renewal. You apply for reciprocity through DHSR instead, which is a separate process with its own eligibility rules and application form.⁹
