How to Report Changes in Circumstances to Medi-Cal

To report a change to Medi-Cal, you have 10 calendar days from the date it happens to notify your county social services office, and you can do it through the BenefitsCal online portal, by calling the county, or by mail. The changes that must be reported fall into four categories: income or property, household composition, address, and other health coverage.

What You Have to Report

California regulation 22 CCR ยง 50185 spells out four categories of changes that affect your Medi-Cal eligibility or share of cost:1Cornell Law School. California Code 22 CCR 50185 – Applicants and Beneficiaries General Responsibilities

  • Income or property changes. A new job, a raise, a job loss, changes to Social Security payments, unemployment benefits starting or stopping, or gaining or losing any other source of money.
  • Household changes. A birth, a death, a marriage, a divorce, or someone moving into or out of your home.
  • Address changes. Any move. If you move to a new county, you also need to contact that county and ask for a redetermination of eligibility there.
  • Other health coverage. Getting or losing private insurance, employer coverage, Medicare, or any other health plan.

That last category catches people off guard. The regulation also requires you to report any available option for other health coverage, even if you haven’t signed up. If your employer begins offering a plan or your spouse’s job provides coverage, the county needs to know.1Cornell Law School. The 10-Day Clock

The 10 days are calendar days, not business days, and they start the day the change happens. If you start a new job on March 3, the report is due by March 13.1Cornell Law School. California Code 22 CCR 50185 – Applicants and Beneficiaries General Responsibilities

Missing that window costs money. If you don’t report in time and Medi-Cal pays for services you weren’t eligible for, or pays more than it should have because your share of cost was wrong, every month from that point forward is treated as an overpayment period until the error is corrected.2Department of Health Care Services. How to File the Report

Online Through BenefitsCal

The fastest route is BenefitsCal.com. After you log in, link your case, open the “Report a Change” section, enter the details, and upload scanned or photographed documents. The system issues a confirmation when you submit.3BenefitsCal. Reporting Changes

One quirk: once you link your case, the report feature can take up to 48 hours to appear in your “Things to Do” list. Don’t wait until day nine to begin.

By Phone

Calling your county social services office works too. A worker can take the information over the phone, though they may ask you to mail or upload documents afterward. Phone reporting is useful if you don’t have a scanner or steady internet, and it lets you ask what the county needs from you.

By Mail

You can mail a written report with supporting documents to your county office. The Department of Health Care Services publishes a directory of county offices and their addresses.4Department of Health Care Services. County Contact List

Send it certified mail. The receipt with the postmark is your proof you met the 10-day deadline, and in a dispute over timing that record can be the difference between a clean file and an overpayment claim.

Documents the County May Ask For

The regulation requires you to make available to the county “all documents needed to determine eligibility and share of cost.” What that means in practice depends on the change:1Cornell Law School. California Code 22 CCR 50185 – Applicants and Beneficiaries General Responsibilities

  • Income: recent pay stubs, an offer letter with your new salary, an award letter for Social Security or unemployment, or a termination notice.
  • Household: a birth certificate, marriage certificate, or divorce decree, along with the full name and Social Security number of anyone joining your household.
  • Address: a copy of your new lease, a recent utility bill, or similar proof of residence.
  • Other health coverage: insurance cards, enrollment confirmations, or an employer letter describing available coverage.

California checks electronic data sources before asking you for paperwork. If what you reported matches what the state can verify through its databases, you may never be asked for physical documents. Requests usually come only when the electronic data doesn’t match, or isn’t available for that type of change.5Medicaid.gov. Financial Eligibility Verification Requirements and Flexibilities

What Happens After You Report

Once the county processes your report, it mails you a Notice of Action. The letter explains the decision: whether your Medi-Cal continues unchanged, whether you’ve moved to a different coverage group, whether your share of cost changed, or whether you’ve been found ineligible.6Department of Health Care Services. Medi-Cal Notice of Action – Frequently Asked Questions

For adverse actions like a benefit reduction or coverage termination, the county must mail the notice at least 10 calendar days before the first of the month the change takes effect. For other actions, the notice goes out by the effective date.7New York Codes, Rules and Regulations. If the county decides your income is above the limit for free Medi-Cal, you may be assigned a share of cost rather than losing coverage. A share of cost works like a monthly deductible: you pay a set amount toward medical expenses each month, and once you hit it, Medi-Cal covers the rest for that month. Months with no medical care cost you nothing. Income changes are the most common reason a share of cost appears or shifts, which is why reporting income promptly keeps the number accurate.

If You Don’t Report

The state has real tools to recover money spent on someone who should have been ineligible or paying more. For periods when you were fully ineligible, the entire cost of services Medi-Cal paid becomes the overpayment.2Department of Health Care Services. Medi-Cal Eligibility Procedures Manual – Overpayment and Fraud

Collection starts with demand letters and phone calls asking for voluntary repayment. If that doesn’t work, the state can intercept your California income tax refund or lottery winnings. In some cases, the Department of Health Care Services refers the debt to the Attorney General to pursue a judgment, which can lead to liens on real property or levies against other assets.2Department of Health Care Services. Intentional misrepresentation is treated separately through the criminal and civil courts rather than administrative fines. States can impose administrative penalties for “eligibility abuse,” including non-fraudulent failures to report, but those penalties must be in the state’s approved Medicaid plan and cannot exceed the value of services provided. A state also cannot lock you out of Medi-Cal as punishment unless you’ve been convicted of fraud in federal court.8Medicaid.gov. If the Change Ends Your Medi-Cal

A reported income increase or household change can push you above the Medi-Cal limits. If that happens, you have options, but the enrollment windows are tight and nobody is going to remind you.

Losing Medi-Cal opens a 90-day special enrollment period for Covered California marketplace plans, during which you can shop for a subsidized plan without waiting for annual open enrollment.9Covered California. Special Enrollment

If your employer offers insurance, losing Medi-Cal also opens a separate 60-day window to enroll in that employer plan. Your employer won’t be told automatically that you lost Medi-Cal, so contact HR yourself and request enrollment. Miss the 60 days and you’ll usually have to wait for the employer’s next open enrollment.10HealthCare.gov. Special Enrollment Period

When the county finds you no longer eligible, federal rules require it to assess your eligibility for marketplace coverage and electronically transfer your account to the marketplace. The transfer isn’t instant and doesn’t enroll you automatically. You still need to pick a plan and finish enrollment within your special enrollment window.

Reporting Changes Doesn’t Replace Annual Renewal

Reporting changes throughout the year is not the same as the annual renewal. Once a year, the state reviews your eligibility. If it needs information beyond what’s already in its electronic records, it mails a renewal form in a yellow envelope with a due date. Return that form on time or your Medi-Cal can be discontinued, even if nothing about your situation changed. Reporting changes as they happen keeps your file current, which usually makes the annual renewal straightforward.
