To get a Troy birth certificate, contact the City of Troy Vital Records office on the fifth floor of City Hall at 433 River Street. The office issues certified copies for anyone born within Troy city limits, charges $10 per copy, and accepts requests in person or by mail.1City of Troy. Birth Records There is no online ordering through the city itself.
Who Can Request a Copy
New York Public Health Law limits certified copies to a short list of people. Troy will issue one to the person named on the certificate if they are at least 18, to a parent listed on the certificate, or to someone presenting a New York State court order.1City of Troy. Birth Records The underlying statute also recognizes a lawful representative of the person on the record, which includes legal guardians, and permits state or federal agencies to obtain records directly.2New York State Senate. New York Public Health Law 4174
If you are requesting on behalf of someone else, bring documentation of the relationship: a current court-certified guardianship order, written authorization, or a power of attorney, depending on the situation.
What to Gather Before You Apply
In person or by mail, the office needs the same core details about the person on the record:
- Full name at birth, including maiden name if applicable
- Date of birth
- Place of birth (the city or town)
- Father’s name
- Mother’s maiden name
- Number of copies you want
Everything you provide must match what the registrar has on file. A small misspelling or a wrong parent name can stall the search, so check family records before submitting.1City of Troy. Birth Records
Identification
Every application needs one acceptable form of ID. Troy accepts any one of the following:
- Driver’s license
- State-issued non-driver photo ID card
- Passport
- U.S. military photo ID
- Employer photo ID card
- Police report documenting the loss or theft of your ID
If you don’t have any of those, Troy will accept two secondary documents instead: recent utility or telephone bills showing your name and address, or letters from a government agency dated within the last six months.1City of Troy. Birth Records Submit either one primary ID or two secondary documents, not a mix.
Payment
Each certified copy is $10. The office accepts cash, check, or money order, with checks made payable to “City of Troy.” Credit and debit cards are not accepted.3City of Troy. Vital Records – Fees For mailed requests, use a check or money order.

Birth Records Bring your ID, your payment, and the information listed above. Staff can usually search while you wait, which makes an in-person visit the fastest route to a certified copy.

No specific printed form is required. A signed written request works as long as it includes all the information above and the address where you want the certificate sent.1City of Troy. Birth Records Using the standard state form (DOH-296A, Application to Local Registrar for Copy of Birth Record) is a convenient way to make sure nothing is left out, and it includes a checkbox for the purpose of the request, such as a passport, school enrollment, or employment.4New York State Department of Health. Mail takes longer in both directions, and your request still has to wait in the registrar’s processing queue. If you’re working against a passport or enrollment deadline, go in person.
Online Ordering and the State Alternative
Troy does not take online requests for its birth records. In person or by mail are the only two options directly through the city.1City of Troy. Birth Records
The New York State Department of Health does offer online and phone ordering through VitalChek, and its statewide records include Troy births going back to 1881.5New York State. If You Weren’t Born in Troy
Troy can only issue certificates for births that happened inside the city. For a birth elsewhere in New York State, you can contact the local registrar for that city, town, or village, or order from the New York State Department of Health, which holds records for the state outside New York City going back to 1881.6New York State Department of Health. Fixing an Error on the Record
Corrections and amendments to a New York birth certificate don’t go through Troy. They are handled by the state Department of Health’s Vital Records Amendment Unit, which uses different forms depending on the type of change (spelling errors, missing first name, adding a father’s name after a marriage, or a legal name change).7New York State Department of Health. Amending a Birth Certificate One corrected copy is issued at no charge once the amendment is processed; additional copies are $30 each. If your child is under 12 months old and the hospital made the error, contact the hospital first, since they can often correct the record with the state before it becomes more complicated.6New York State Department of Health.
