What the Seller Fills Out
The seller signs the back of the current certificate of title. That block also needs the buyer’s full legal name, current address, and the exact date of sale. Spelling errors and mismatched dates cause delays at the counter, so check the entries against the buyer’s ID and the bill of sale before you sign.
A written bill of sale isn’t a state-issued form, but it’s worth doing. List both parties, the sale price, the vehicle description, the VIN, and signatures from buyer and seller. If a dispute comes up later, that document is the proof of the transaction.
What the Buyer Fills Out

Odometer Disclosure
Iowa follows the federal odometer rules under 49 CFR Part 580. For transfers happening through December 31, 2030, any vehicle that is model year 2011 or newer requires an Odometer Disclosure Statement signed by both the buyer and the seller.4Iowa Legislature. Damage Disclosure
Iowa Code ยง 321.69 requires a Damage Disclosure Statement for most vehicles that are seven model years old or newer.5Justia. Filing at the County Treasurer’s Office

- The original title, signed over by the seller, with the buyer’s name and sale date filled in.
- Completed Form 411007.
- Odometer Disclosure Statement, if the vehicle is model year 2011 or newer.
- Damage Disclosure Statement, if the vehicle is seven model years old or newer.
- Bill of sale for a purchase, or the applicable affidavit for a gift or inheritance.
- Payment for the title fee, registration fee, and any lien fee.
Fees

The 30-Day Deadline and Late Penalties
You have 30 days from the date of sale to file. Miss that window and penalties start automatically: a $10 late fee on the title plus 5% of the registration amount, with a minimum registration penalty of $5.2Woodbury County, Iowa. Gifts and Inherited Vehicles
When a vehicle changes hands as a gift with no money involved, the parties fill out an Affidavit in Lieu of Bill of Sale rather than a standard bill of sale. The affidavit declares the transfer as a genuine gift so the county can categorize the transaction correctly for tax purposes.
Inheritance depends on whether there is a will. With no will, the heir files an Affidavit of Death Intestate (Form 411088), which establishes entitlement under Iowa’s laws of descent and distribution.8Department of Transportation. Either way, the new owner still files Form 411007 and pays the standard title fee.
Vehicles With a Loan on Them
If a loan is still outstanding, the lender holds a lien on the title, and the seller can’t hand over a clean document until the debt is paid. Iowa uses an Electronic Lien and Title system, so many lienholders release their interest electronically once the loan is satisfied. If a seller claims the loan is paid off but doesn’t have the physical title yet, ask for documentation of the lien release before handing over money.
If the Title Is Lost
If the original title has been lost, destroyed, or stolen, the current owner on record has to apply for a replacement before the transfer can happen. The replacement title fee is $35, and every owner listed on the face of the original title must sign the application, even if the names are joined by “or.” Replacements are typically printed and mailed within five business days of the paperwork being received. If a lien was on the original title and has since been paid off, the owner needs a notarized lien release from the lender before a clean replacement can issue.
A buyer who is told the title is coming “later” is better off waiting until the replacement is in hand before paying. Handing over money without a signed title leaves you with no way to register the vehicle and no legal proof of ownership.
The License Plates Come Off
In Iowa, license plates belong to the seller, not the vehicle. The seller removes them before handing the car over.11Department of Transportation. Otherwise, the plates go back to the county treasurer’s office. The buyer gets new plates at registration, so plan the trip to the treasurer accordingly or arrange for a tow, because the vehicle can’t legally be driven on public roads without plates.
