Idaho Medicaid Prior Authorization Form: Qualitrac Submission and Appeals

To get a service covered by Idaho Medicaid that requires prior approval, submit the Idaho Medicaid prior authorization form and supporting records through the Telligen Qualitrac portal at idmedicaid.telligen.com before the service is provided. Since August 2023, faxed, mailed, and phoned-in medical PA requests are no longer accepted.1Telligen. Changes to Prior Auth and Medical Record Submission Approval on the first try comes down to two things: filling in every required identifier, and attaching clinical documentation that clearly supports medical necessity.

Where to Send the Request

Medical services, procedures, and DMEPOS requests go through the Telligen Qualitrac Provider Portal. New users can register for portal access in about ten minutes, and the Qualitrac Provider Portal User Guide in Telligen’s document library walks through each submission screen.2Telligen. Two other paths exist and should not be mixed up with the Qualitrac route:

  • Pharmacy prior authorizations go to Prime Therapeutics, the state’s pharmacy benefit administrator. Fax requests to 800-327-5541 or call the Pharmacy Support Center at 800-922-3987, staffed 24 hours a day.3Idaho Department of Health and Welfare. Idaho Medicaid Pharmacy Program
  • If the patient is enrolled in a managed care organization, follow that MCO’s own prior authorization process, not the fee-for-service route described here.

What Information the Form Requires

Every request needs a core set of identifiers so the system can match the patient, provider, and service. Include all of the following up front:5Idaho Medicaid. General Information and Requirements for Providers

  • Participant’s full name and Idaho Medicaid ID number, exactly as they appear in the state database.
  • Pay-to or billing NPI, plus the provider’s name and contact information.
  • CPT or HCPCS procedure codes for the requested service, with ICD-10 diagnosis codes for the condition.
  • The PA request date and the specific date or date range for the proposed service.

For durable medical equipment, prosthetics, orthotics, and supplies, add the manufacturer’s suggested retail price or an invoice for any manually priced item. Without that pricing, Gainwell Technologies enters the price as zero and the claim will not pay correctly.6Telligen. Idaho Medicaid DMEPOS FAQ

Clinical Documentation That Supports Approval

Administrative fields get the request into the system. Clinical records are what get it approved. Idaho Medicaid reviewers work from a layered set of criteria: the Provider Handbook first, Milliman Care Guidelines for services Telligen reviews without specific handbook guidance, and Medicare’s local or national coverage determinations for anything the first two don’t address.5Idaho Medicaid. General Information and Requirements for Providers

At a minimum, attach recent physician progress notes describing the patient’s condition and explaining why the requested service fits. Lab results, imaging reports, and specialty consultation notes should go in when they add to the clinical picture. Each document should line up with the diagnosis and procedure codes on the form. Reviewers look for a coherent story, and gaps between the codes and the records are one of the fastest routes to a denial or a request for more information.

DMEPOS requests carry an additional layer. Each one must include the medical-necessity documentation required under IDAPA 16.03.09, Section 753.7Legal Information Institute. Idaho Admin. Code r. 16.03.09.752 – Durable Medical Equipment and Supplies Coverage and Limitations In practice that often means a certificate of medical necessity or a specialized assessment signed by the treating physician. Keep digital copies of everything you submit. You’ll need them if the request is denied and you appeal, or if the state audits the claim later.

How to Submit Through Qualitrac

Log into the Qualitrac Provider Portal at idmedicaid.telligen.com. The portal walks you through uploading the completed form and all supporting documents as PDFs. After you confirm the submission, the system assigns a tracking number for checking status. Save that number. It is your proof of receipt.

One rule drives everything else: the prior authorization must be submitted and approved before the service is provided. Idaho Medicaid will deny payment for services rendered before a valid PA request was submitted, with narrow case-by-case exceptions for circumstances genuinely beyond the provider’s control. Providers cannot bill the patient for a service that was denied because the authorization was not obtained in time.8Idaho Department of Health and Welfare. How Long the Decision Takes

Timing depends on the service type and whether the request is urgent. For Medicaid managed care plans, federal rules effective January 1, 2026, cap standard authorization decisions at seven calendar days from receipt. Expedited requests, where delay could seriously jeopardize the patient’s life, health, or ability to function, must be decided within 72 hours.9eCFR. 42 CFR 438.210 Either timeframe can be extended up to 14 additional calendar days if the enrollee or provider requests it, or if the plan needs more information and can show the extension serves the patient’s interest.

Pharmacy prior authorizations move faster. Idaho’s administrative code requires the Department to respond within 24 hours to a request for prior authorization of a covered outpatient prescription drug.10Legal Information Institute. Idaho Admin. Code r. 16.03.09.663 – Prescription Drugs

When review is complete, the Department issues a Notice of Decision to the participant. An approval includes an authorization number and a date range during which the service must be performed. Schedule within that window; services delivered outside it will not be covered. If the reviewer needs more information, you’ll receive a request for additional documentation, which resets part of the clock. A denial spells out the specific reasons and explains the patient’s appeal rights.

Appealing a Denial

A denial is not the end of the road. The participant has 28 days from the date on the Notice of Decision to request a fair hearing on the denial of a Medicaid service.8Idaho Department of Health and Welfare. IDAPA 16.03.26 – Idaho Department of Health and Welfare For denials tied to Medicaid eligibility rather than a specific service, the deadline is 30 days.11Idaho Department of Health and Welfare. Appeals and Fair Hearings

To file, complete the Fair Hearing Request form and mail it in, submit a written request by email or fax, or call the Department directly. A phone filing may still require a follow-up in writing. The appeal can be filed by the participant or by someone acting on their behalf, including a provider, family member, attorney, or other representative.11Idaho Department of Health and Welfare. Appeals and Fair Hearings

In some situations the participant can keep receiving benefits while the appeal is pending. To preserve that option, the participant must notify the Department within 10 days of the notice date. If the appeal ultimately fails, the participant may have to repay any benefits received during that period.11Idaho Department of Health and Welfare. Appeals and Fair Hearings

Most fair hearings take place by phone. An independent hearing officer reviews the evidence from both sides, including the participant’s documentation and the Department’s rationale for the denial. This is where saved copies of the original submission matter. Bring the clinical records, the denial letter, and any new documentation that strengthens the medical-necessity argument. The hearing officer decides based on the evidence presented and the applicable program rules.
