To apply for IHSS in Los Angeles, call the county’s IHSS intake line at (888) 944-4477 (or the local number, (213) 744-4477) and ask a worker to open a case, or mail a completed Application for In-Home Supportive Services (Form SOC 295) to the DPSS regional office that serves your address.1Los Angeles County Department of Public Social Services. The program is funded through Medi-Cal, so active Medi-Cal coverage is part of qualifying.
Who Can Apply
IHSS is limited to Californians who are 65 or older, legally blind, or have a disability expected to last at least 12 months, and who could not safely remain at home without help.2California Legislative Information. California Code WIC 12300 – In-Home Supportive Services You have to live in your own home or apartment. People in a hospital, nursing facility, or board-and-care home don’t qualify.
Financial eligibility runs through Medi-Cal. If you already have full-scope Medi-Cal, you meet the income side of the test. If you don’t, you’ll need to apply for Medi-Cal at the same time or before the IHSS application; the statute ties eligibility for services directly to Medi-Cal eligibility.2California Legislative Information. Some applicants have income slightly too high for free Medi-Cal but still qualify through a “Share of Cost” arrangement, which works like a monthly medical deductible. For a single adult, the maintenance need level used in that calculation is $600 per month. If your income is modest but above the free Medi-Cal threshold, ask your eligibility worker about the Aged and Disabled Federal Poverty Level program, which offers free full-scope Medi-Cal to individuals with countable monthly income below $1,800 (or $2,433 for a couple).
How to Submit the Application
Most people in LA County apply by phone. Calling (888) 944-4477 puts you through to an intake worker who takes your information and opens the case that day. If you’d rather submit paper, mail or fax a completed SOC 295 to the DPSS regional office that covers your ZIP code; the intake line can tell you which office and provide its mailing address and fax number.1Los Angeles County Department of Public Social Services. The date the county receives your SOC 295 becomes the official start date of your application.3California Department of Social Services. SOC 295 – Application for In-Home Supportive Services That date matters because approved hours can be backdated to it. The county will mail you a confirmation with your case number within a few business days. Keep it. It’s your proof the clock started, and it can matter if there are later disputes about processing delays.
Have your Medi-Cal case number ready when you call. If you’re applying for Medi-Cal at the same time, gather proof of income (Social Security statements, pay stubs) and recent bank statements. The Health Care Certification and Its 45-Day Deadline
Once your case is open, the county sends the Health Care Certification (Form SOC 873) for your doctor, nurse practitioner, or other licensed health care professional to complete.5California Department of Social Services. SOC 873 – IHSS Program Health Care Certification Form The form confirms that you need help with daily activities and would be at risk of out-of-home placement without services.
This form has a hard deadline. The completed SOC 873 must come back within 45 calendar days of the county’s request. Miss that window and the county denies your application on procedural grounds. You can request a 45-day extension for good cause, giving you up to 90 days total, but you have to ask for it rather than letting the deadline slide.6California Department of Social Services. All-County Letter No. 11-76 – SOC 873 Health Care Certification Because doctors’ offices can be slow, get the form to your provider the day you receive it and follow up in person or by phone within a week.
The Home Visit and Functional Assessment
A county social worker comes to your home to evaluate how much help you actually need. This visit is the single most important step in the process, because it directly determines your authorized hours. The worker watches you perform (or attempt) tasks like standing, walking, reaching, and gripping, and asks detailed questions about what you can and can’t do on your own.
The assessment covers every IHSS service category: housecleaning, laundry, meal preparation, shopping, personal care such as bathing and dressing, help with walking, accompaniment to medical appointments, and doctor-ordered paramedical tasks like injections or wound care. Each category gets a functional index ranking based on your level of impairment, and those rankings translate into a specific number of authorized minutes per task. A few things help during the visit. Don’t downplay your limitations. Many people instinctively try to seem more capable than they are in front of a stranger in their home. Describe your worst days, not your best. If you fall, wander, or burn food, say so. If a family member has been filling in the gaps, explain what would happen if that person weren’t there. The worker also reviews the home environment for safety issues, so don’t stage-clean the house beforehand. A messy home can actually support your case that you need domestic help.
If the applicant has dementia or a serious mental illness and cannot recognize danger, ask the worker about protective supervision, which authorizes round-the-clock observation. Qualifying requires that the person be “non-self-directing” and likely to engage in behavior that puts them at risk of harm, such as wandering into traffic or leaving the stove on.8California Department of Social Services. Documentation from a treating psychiatrist, neurologist, or psychologist, brought to the home visit, strengthens the claim considerably.
Your Notice of Action
After the assessment, the county mails a Notice of Action on Form NA 1250 listing every approved service category, the authorized hours and minutes for each, and your total monthly hours.9California Department of Social Services. NA 1250 – IHSS Approval Read it carefully and compare the authorized hours against what you discussed during the home visit. If the county later changes your hours after a reassessment, you’ll receive Form NA 1253 showing the old and new allocations side by side.10California Department of Social Services. If the county denies your application, approves fewer hours than you need, or later cuts your hours, you can request a state fair hearing. The deadline is 90 days from the date on the Notice of Action.11California Department of Social Services. One trap catches people repeatedly. If you’re already receiving IHSS and the county is reducing or terminating your hours, filing your hearing request before the effective date on the notice keeps your current services running during the appeal. This is called “aid paid pending.” Wait past that date and your hours drop while you wait for a hearing, which can take weeks. Aid paid pending only applies to reductions or terminations of existing services, not to new services you’ve never received.
Enrolling Your Caregiver
Once approved, you choose your own caregiver. It can be a family member, a friend, or someone you hire. The program doesn’t assign providers to you. Whoever you pick has to complete the county’s enrollment process before they can be paid.

Certain criminal convictions within the past 10 years can affect eligibility to serve as a provider. Some serious offenses are absolute bars, while others can be waived if the recipient acknowledges the conviction and still wants that person as their caregiver.
