IHSS in Fresno County is California’s In-Home Supportive Services program run locally by the Fresno County Department of Social Services. It pays a caregiver of your choosing to help you with daily tasks at home so you can avoid moving into a nursing facility or board-and-care home. To receive it, you need active Medi-Cal, you need to be aged, blind, or disabled, and a county social worker needs to agree that without the help you would be at risk of out-of-home placement.1California Department of Social Services. Who Qualifies
Three things have to be true. You live in Fresno County in your own home or a place you chose yourself (hospitals, nursing homes, and licensed care facilities don’t count). You have Medi-Cal. And you’re aged, blind, or disabled under state and federal standards.2County of Fresno. In-Home Supportive Services
Beyond the baseline, the deciding question is whether you would end up in out-of-home care without the services. Your physical or mental condition has to limit your ability to handle daily activities safely on your own, and the county has to find that removing in-home support would put you at risk of institutional placement.3California Legislative Information. California Welfare and Institutions Code 12300 – In-Home Supportive Services Children can qualify when they need more care than a child of the same age without a disability would typically require.
The 2026 Medi-Cal Asset Limit
Because IHSS runs through Medi-Cal, you have to meet Medi-Cal’s financial rules. On January 1, 2026, California reinstated asset limits for non-MAGI Medi-Cal programs after a period when assets were not counted. The cap is $130,000 for an individual and $195,000 for a couple, plus $65,000 for each additional household member. Your primary home, one vehicle, and certain other assets are typically excluded. If you were receiving IHSS during the no-asset-test period, check with Fresno County to confirm your continued eligibility.
What IHSS Covers
The social worker decides which tasks you qualify for and how many monthly hours you get for each one, using statewide Hourly Task Guidelines to keep the numbers consistent across California.4California Department of Social Services. In-Home Supportive Services Fact Sheets Authorized services fall into these groups:
- Domestic tasks like cleaning, laundry, grocery shopping, and meal preparation.
- Personal care, including hands-on help with bathing, grooming, dressing, eating, and bowel or bladder care.
- Paramedical services such as injections, catheter care, or oxygen monitoring. These must be authorized by a doctor, who trains or instructs your provider.
- Accompaniment to and from medical appointments when you cannot travel alone.
- Protective supervision, which is around-the-clock monitoring for recipients whose mental impairment or mental illness makes them unable to direct their own care. A diagnosis like Alzheimer’s or autism does not automatically qualify you; the county evaluates your actual functional abilities and the safety risks you face when unsupervised.
Total monthly hours are capped at 283 for recipients classified as severely impaired and 195 for everyone else. Individual providers also have a separate 66-hour weekly cap across all the recipients they serve.5California Department of Social Services. IHSS New Program Requirements
How to Apply in Fresno County
Two forms drive the application. The IHSS Program Application (SOC 295) is the main form, where you give your personal information and describe your need for in-home support.6California Department of Social Services. Application for In-Home Supportive Services Before you start, pull together your Social Security number, Medi-Cal client index number, and your primary care doctor’s contact information.
Fresno County accepts the SOC 295 three ways:
- Online, through the county’s IHSS application page (available in English and Spanish) and a separate secure document upload portal.2County of Fresno. In-Home Supportive Services
- By mail to Department of Social Services, P.O. Box 1912, Fresno, CA 93718-1912.
- By fax to (559) 600-7762.
If you don’t receive confirmation within a couple of weeks, call the IHSS intake line at (559) 600-6666 to make sure the county has your file.2County of Fresno. In-Home Supportive Services
The second form is the Health Care Certification (SOC 873). State law requires it before IHSS can be authorized.7California Department of Social Services. In-Home Supportive Services Program Health Care Certification Form Your doctor, physician assistant, psychologist, physical therapist, or other licensed health care professional fills it out, describing your specific conditions, your functional limitations, and your risk of out-of-home placement without help. By law, they cannot charge you a fee to complete this form.8California Legislative Information. California Welfare and Institutions Code WIC 12309.1 In urgent situations, like a hospital discharge where you need services immediately to go home safely, the county can authorize services temporarily before receiving the SOC 873.
What Happens After You Apply
Once the county processes your paperwork, a social worker schedules an in-home visit. The visit usually runs about an hour, though complex cases take longer. The social worker observes your living situation, talks with you about what you can and cannot do, and evaluates your functional abilities across categories like meal preparation, bathing, mobility, and housekeeping. Those observations get translated into a monthly hour allocation for each authorized task.
A Notice of Action follows. It tells you whether you were approved or denied and, if approved, exactly how many hours you received for each task. Read it carefully. If the hours look too low for your real needs, you can appeal.
IHSS is not a one-and-done approval. A social worker must reassess your needs before the end of the twelfth month after your last assessment, though the county may extend that to 18 months case by case if your condition is stable and certain documentation requirements are met. You can also ask for a new assessment any time your condition changes significantly.9California Department of Social Services. Program Service Categories and Time Limits
Hiring and Paying a Provider
In IHSS, you are the employer. You hire, train, supervise, and if necessary fire your own provider.1California Department of Social Services. In-Home Supportive Services Program Many recipients hire a family member or someone they already know. If you need help finding a caregiver, the Fresno County Public Authority keeps a provider registry. Reach them at (559) 600-6666 or in person at 1617 W. Belmont Avenue, Fresno, CA 93728, Monday through Friday from 8:00 a.m. to 5:00 p.m.10County of Fresno. Join the IHSS Public Authority Provider Registry
Whoever you choose has to complete enrollment before they can be paid. That means attending an IHSS provider orientation, signing the Provider Enrollment Form (SOC 426) and the Provider Enrollment Agreement (SOC 846), and passing a fingerprint-based criminal background check through the California Department of Justice.11California Department of Social Services. IHSS Provider Orientation Enrollment can take several weeks, so start as soon as your hours are approved. Once your provider is enrolled, they can submit timesheets retroactive to their assignment date, so you won’t lose authorized hours during the wait.12California Department of Social Services. IHSS Provider Resources
Pay periods run from the 1st through the 15th and the 16th through the end of the month. Timesheets are signed electronically through the Electronic Services Portal (ESP), and providers can sign up for direct deposit.
Electronic Visit Verification
Federal law requires an electronic record of when and where IHSS services are performed. Providers who do not live with their recipient must check in at the start of each shift and check out at the end using the ESP website, the telephone timesheet system, or the IHSS EVV mobile app, selecting whether services are being provided in the home or in the community. Pay is based on the hours reported on the timesheet, not on the time between check-in and check-out, and a missed real-time check-in or check-out can be entered manually later.13California Department of Social Services. Electronic Visit Verification Help Providers who live in the same home as their recipient are exempt and claim the exemption by filing a Live-In Self-Certification form (SOC 2298) with the state.14California Department of Social Services. If You’re Denied or Your Hours Are Cut
Your Notice of Action includes hearing instructions. You have 90 days from the date of the notice to request a state hearing; after that, you need good cause for the delay.15California Department of Social Services. State Hearing Requests You can file three ways:
- By phone at (800) 743-8525.
- Online through the CDSS hearing portal.
- By mail, using the form on the back of your Notice of Action or a letter that includes your full name, address, phone number, the county that took the action, and why you disagree.
If you’re an existing recipient whose hours are being reduced or terminated, timing matters. File your hearing request before the effective date on the notice and you keep your current services while the appeal is pending. This is called aid paid pending. Wait until after the effective date and your services drop to the new level while the hearing is processed. Aid paid pending does not apply to new applicants who were denied entirely, because there’s no existing service level to maintain.
Tax Rule Worth Knowing for Live-In Providers
If your IHSS provider lives in the same home as you, their wages may be excluded from federal and state income tax. Under IRS Notice 2014-7, Medicaid waiver payments made to a caregiver who shares a home with the care recipient are treated as “difficulty of care” payments excludable from gross income under Section 131 of the Internal Revenue Code.16Internal Revenue Service. Certain Medicaid Waiver Payments May Be Excludable From Income The provider has to genuinely live there; keeping a separate primary residence somewhere else does not qualify. The SOC 2298 that triggers the income tax exclusion does not change Social Security (FICA) or Medicare withholdings, so those amounts still show up in Boxes 3 and 5 of the W-2.14California Department of Social Services. Live-In Provider Self-Certification Information Providers can also elect to include the excluded payments as earned income when claiming the Earned Income Tax Credit or Child Tax Credit, which sometimes produces a larger refund.
