Illinois Medical License Renewal: 150-Hour CME and Deadlines

To complete an Illinois medical license renewal, you must submit your renewal through the Illinois Department of Financial and Professional Regulation (IDFPR) online portal before your license expires on July 31, 2026, after finishing 150 hours of continuing medical education during the prior 36 months and paying the $543 fee. The CME total must include several topic-specific hours that Illinois requires every cycle, and you self-certify compliance at the time of renewal.

When the License Expires

Illinois physician licenses run on a three-year cycle that ends July 31 of the renewal year. The current cycle expires July 31, 2026. The prerenewal period, meaning the window in which your CME hours must be earned, is the 36 months leading up to that expiration date.¹

IDFPR sends renewal notices before expiration, but the obligation to renew on time is yours whether the notice reaches you or not. Track the date independently.

The 150-Hour CME Requirement

You must complete 150 hours of continuing medical education during the three-year prerenewal period. Those hours split into two categories:

At least 60 hours from formal programs (Category 1)
Up to 90 hours from informal or self-directed activities (Category 2)

Formal programs must come from approved sponsors. For MDs, that means programs accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Illinois State Medical Society and its affiliates, or other organizations approved under the administrative code.¹ Informal hours can include teaching, publishing research, or independent study, provided you log them.

Required Topics Inside the 150 Hours

Several specific topics are mandatory each cycle. These hours count toward your 150-hour total rather than being added on top, but skipping any of them will block renewal.

Sexual Harassment Prevention

One hour each renewal cycle, covering legal definitions, reporting mechanisms, and workplace conduct standards. This applies to all Illinois licensed professionals subject to continuing education.

Implicit Bias Awareness

One hour on recognizing implicit bias in healthcare settings, focused on improving equity in patient outcomes. Physicians who work with children may find overlap with the implicit bias component of mandated reporter training.

Mandated Reporter Training

Illinois designates physicians as mandated reporters of suspected child abuse and neglect. Physicians who treat or interact with pediatric populations must complete mandated reporter training offered through the Department of Children and Family Services (DCFS), available online at no cost through the DCFS website.

Safe Opioid Prescribing

If you hold a controlled substance registration, you must complete three hours of continuing education on safe opioid prescribing practices each cycle. Those three hours count toward both your controlled substance registration renewal and your overall 150-hour CME total.²

How to Renew Online

Renewals go through the IDFPR online portal.³ Before you start, have your professional license number and the email address IDFPR has on file. If your contact information has changed, update it first so digital correspondence reaches you.⁴

CME compliance is handled through self-certification. You attest that you have completed the 150 hours and the mandatory topic training; you do not upload certificates. You are, however, legally required to keep those records in case of audit. The renewal also asks you to confirm tax compliance and disclose any disciplinary actions, criminal convictions, or malpractice matters since your last renewal.⁵

The renewal fee for an active three-year physician license is $543, paid by credit or debit card. Stay on the confirmation page until you receive a confirmation number. You should then get an automated email with a digital copy of your renewed license, and you can verify the update through IDFPR’s online License Lookup within two to four business days.³

Keep Your CME Records

IDFPR conducts random CME audits. If you are selected, you have to produce documentation proving you completed the required hours. Keep records for at least one full license cycle (three years) after the cycle in which the credits were earned.

For formal CME programs, retain the sponsor’s name, program title, dates of attendance, credit hours earned, and certificates of completion. For informal activities, keep a log of each activity type and the approximate hours spent.

If you cannot finish your CME before the deadline, IDFPR has a CME Waiver Request Form. It must be submitted before your license expires and asks the Department to consider specific circumstances. It is not a general extension.

If You Miss the Deadline

Once your license expires, getting back into good standing becomes harder the longer you wait. The administrative code addresses this under Section 1285.130, covering reinstatement from expired, inactive, or not-renewed status. A late application generally requires a written submission by mail rather than the online portal, higher fees than a standard renewal, proof of completed CME hours including any mandatory topic-specific training, and potentially additional review. If your controlled substance registration also lapsed, you must separately demonstrate the three hours of safe opioid prescribing education.² You cannot legally practice during the gap.

Penalties for Practicing on an Expired License

IDFPR can impose a civil penalty of up to $5,000 for each offense against anyone who practices, attempts to practice, or holds themselves out as a physician without a current license. That penalty is per incident.⁶

Criminally, a first-time violation of certain provisions of the Medical Practice Act is a Class 4 felony, carrying one to three years in prison. Repeat offenses are Class 3 felonies.⁶ Practicing on a lapsed license also creates malpractice exposure that your insurer may refuse to cover, since most policies require a valid license during the period of care.

IDFPR has separate authority to refuse to renew a license or impose discipline, including fines of up to $10,000 per violation, against any physician who commits fraud or misrepresentation during renewal. Falsely attesting to CME completion falls squarely within that authority.⁵
