Indiana State Forms: Online Catalog, Agency Links, and Filing

Official Indiana state forms live in the State Forms Online Catalog maintained by the Indiana Archives and Records Administration at forms.in.gov, but several of the busiest agencies — the Department of Revenue, the Bureau of Motor Vehicles, the Secretary of State’s INBiz portal, and the Department of Health’s vital records division — keep their own form libraries that you should go to directly. Which door you use depends on what you’re trying to file.

The State Forms Online Catalog

The Indiana Archives and Records Administration (IARA) hosts the catalog at forms.in.gov.1Indiana Archives and Records Administration. About Us You can search by form number, title, description, agency name, or language. Documents are available in English, Spanish, Burmese, French, and Haitian Creole.2Indiana Archives and Records Administration. Search Forms

Every document carries a unique State Form number that tracks its revisions. Submitting an outdated version is one of the most common reasons a filing gets rejected, so check the number against the catalog before you send anything in. Not every state form appears here, though. Each form belongs to a specific agency, and some agencies publish their documents only on their own websites.3Indiana Archives and Records Administration. Where to Go by Agency

Tax Forms

The Indiana Department of Revenue (DOR) keeps its own forms collection. Individual filers typically need the IT-40 for resident income tax returns, or the IT-40PNR for part-year and nonresident filers. The filing deadline mirrors the federal one: April 15, moved to the next business day when that date falls on a weekend or holiday.4Indiana Department of Revenue. Tax Filing Deadlines

For businesses, the DOR publishes forms for sales tax, withholding, business closure, and more. The WH-1 withholding tax form must be filed by every Indiana employer remitting payroll taxes and reports withholdings by county.5Indiana Department of Revenue. Filing a WH-1 Other common ones include the BT-1 business tax application, the BC-100 business closure request, and the POA-1 power of attorney.6Indiana Department of Revenue. BMV Forms

The Bureau of Motor Vehicles has a separate repository for vehicle titles, driver’s licenses, permits, and ID cards.8Indiana Bureau of Motor Vehicles. Beyond the basics, you’ll find vision certification (State Form 22106), physician certification of medical impairment (State Form 50018), a log of supervised driving hours for new drivers (State Form 54706), and hardship waiver requests (State Form 28622). Certified driving records are requested on State Form 53789.11Indiana Bureau of Motor Vehicles. Driver’s License and Permit Forms

Business Filings

Business formation, registration, and ongoing compliance filings with the Secretary of State go through INBiz at inbiz.in.gov. New businesses can also register through INBiz for sales tax, withholding tax, food and beverage tax, and several other tax types.6Indiana Department of Revenue. Business Tax Forms

One filing to watch is the Business Entity Report. The first report is due two years after the business was formed or registered, and subsequent reports are due every two years in the same month the business was originally formed. Missing it leads to administrative dissolution for Indiana businesses or revocation for foreign entities.12INBiz. Business Entity Reports The filing fee is $32 online or $50 by paper.13INBiz. Vital Records

Birth certificates, death certificates, and related vital records are handled by the Indiana Department of Health’s Division of Vital Records. For birth certificates, you can order by mail using State Form 49607 (Application for Search and Certified Copy of Birth Record), order online through VitalChek, or go through the local health department where the birth occurred. Walk-in service is not available at the state Department of Health.14Indiana Department of Health. Order Certificates

A birth certificate costs $10 for the first copy and $4 for each additional copy ordered at the same time. Amendments or corrections cost $8, and delayed birth registrations cost $10. These fees are nonrefundable and do not include third-party processing charges from services like VitalChek.14Indiana Department of Health. Order Certificates

Court Forms

If you’re filing in an Indiana court without an attorney, the Indiana Judicial Branch’s Self-Service Legal Center points you to the Coalition for Court Access at indianalegalhelp.org, which hosts standardized forms for small claims, protective orders, name changes, family law matters, and other common proceedings.15Indiana Judicial Branch. Filling Out the Form Correctly

Gather your supporting documents before you start. Individual filings almost always need a Social Security number. Business filings need a Federal Employer Identification Number, and the DOR’s business tax application checklist also asks for the responsible officer’s Social Security number.17Indiana Department of Revenue. Business Tax Application Checklist Property-related forms often require the legal description from your deed or property tax statement. Having everything on the desk before you start spares you from stopping halfway through to dig through a filing cabinet.

For paper forms, use black or blue ink and write legibly. Fill in names, addresses, and identification numbers exactly as they appear on your source documents. One transposed digit in a Social Security number, or a name that doesn’t match your driver’s license, can get the whole thing bounced back.

If a form requires notarization, Indiana law leaves that determination to the specific statute governing the document. Neither the notary nor the Secretary of State’s office can tell you whether a particular document must be notarized; you need to check the law that requires the filing.18INBiz. Electronic Signatures

Indiana gives electronic signatures the same legal weight as handwritten ones. Under Indiana Code 26-2-8-106, a record or signature cannot be denied legal effect just because it’s in electronic form, and an electronic signature satisfies any law that requires a signature.19Indiana General Assembly. Indiana Code Title 26 Article 2 Chapter 8 Section 26-2-8-106 – Legal Recognition of Electronic Records, Electronic Signatures, and Electronic Contracts Even so, some forms still require a wet-ink signature or a digital certificate rather than a typed name. Check the form’s instructions before assuming an e-signature will fly.

Submitting and Paying

After you submit a form electronically, save the confirmation number or email receipt. That’s your proof of filing if the agency later says it never received anything. For paper, certified mail with a return receipt does the same job and creates a verifiable delivery date.
