Iowa DOT Form 430032 Vision Report: How to Fill Out and Submit

Iowa DOT Form 430032, the Driver’s Visual Acuity Report, is the form a licensed optometrist or ophthalmologist completes to tell the Iowa Department of Transportation whether your eyesight meets the state’s driving standards. You’ll need it if you failed the vision screening at a service center, if your license already carries a vision-related restriction due for renewal, or if the DOT has asked for an updated report after a concern was raised about your eyesight. The exam behind the form has to be recent: measurements must be taken within 30 days of the date you submit it.¹

When the DOT Will Ask for This Form

The most common trigger is a failed screening at a driver’s license service center. Under Iowa Administrative Code 761—604.7, if you can’t read the 20/40 line with at least one eye but can read the 20/70 line, the examiner stops the process and refers you to a vision specialist. The DOT will not issue or renew your license until it has Form 430032 (or an equivalent letter) on file.²

If your current license already has a vision-related restriction, the DOT typically mails you the form about a month before your renewal. A physician, family member, or law enforcement officer can also report concerns about a driver’s eyesight, which can prompt a review outside the normal renewal window.³

If your license is still valid when the referral happens, the DOT may issue a temporary driving permit with restrictions matching your current vision level so you can keep driving while you get the exam and turn the paperwork in.²

Where to Get the Form

You can download and print Form 430032 from the Iowa DOT’s website or complete it through the DOT’s online form portal.⁴ Hard copies are available at any driver’s license service center, and the DOT mails the form directly to drivers whose current restriction requires periodic vision reports.³

Filling In Your Part

The top section is for you. It asks for your full legal name, date of birth, and contact information. Make sure the name matches what the DOT already has on file, exactly. Even a missing middle initial can cause the form to be flagged as a mismatch and slow processing.

What the Vision Specialist Records

A licensed optometrist or ophthalmologist completes the clinical portion. The specialist records:

Visual acuity, with separate readings for the left eye, right eye, and both eyes together, both with and without corrective lenses.
Field of vision, measured in degrees for each eye and binocularly.
An opinion on whether the condition is stable or progressive. A progressive condition can lead the DOT to require a new report more often than the standard renewal cycle.
Whether new corrective lenses have been prescribed.

The specialist signs the form and enters the examination date and their professional license number. If any of those elements is missing, the DOT will reject the form.¹

The 30-Day Rule

The measurements on Form 430032 have to come from an exam taken within 30 days before you submit the form. An older exam won’t be accepted, so book the appointment with your submission timeline in mind.¹

If the Office Uses Its Own Letterhead

A signed letter on the specialist’s professional letterhead is acceptable in place of the form, as long as it includes everything Form 430032 asks for: acuity readings, field of vision measurements, the stability opinion, the license number, and the exam date.¹ Using the actual form avoids any argument later about whether the letter covered every field.

How to Submit It

Three channels are available:

In person at any Iowa DOT driver’s license service center. Staff will scan the form into your electronic file on the spot.
By fax to Driver Services at 515-239-1837. Your eye doctor’s office can send it directly.⁵
By mail to the Iowa DOT Driver and Identification Services office in Ankeny. Call Driver Services at 515-244-8725 first if you want to confirm the current mailing address.⁵

You can also email Driver.Services@iowadot.us to ask about the status of a submitted report.³ If your temporary permit is running short, fax or in-person delivery beats the mail.

What the DOT Does With It

The DOT’s medical review team measures the specialist’s findings against Iowa Administrative Code Chapter 604 and notifies you of the outcome by mail. Depending on the numbers, the result can be:

An unrestricted license, if acuity is 20/40 or better and field of vision is 140 degrees or wider.
A corrective lens restriction, requiring glasses or contacts while driving.
A daytime-only restriction (no driving when headlights are required), when acuity falls between 20/40 and 20/70.⁶
A requirement to have both left and right outside rearview mirrors, when the left eye is worse than 20/100 or field of vision is between 110 and 139 degrees binocularly.
A shortened renewal cycle if the specialist reports a progressive condition.
Denial, if acuity is below 20/70 in both eyes or field of vision is below 110 degrees binocularly (below 100 degrees monocular), subject to limited DOT discretion to grant an exception.⁷

A road test is also required the first time your acuity measures below 20/40.⁶

Avoiding Rejections and Delays

Most problems with Form 430032 are preventable. Before you leave the specialist’s office, check that the form is fully signed, dated, and legible, with the license number filled in. Confirm your legal name matches DOT records. Schedule the exam no more than a few weeks ahead of when you plan to submit, so the 30-day window doesn’t close on you. If you’re close to the edge, fax the form or drop it off in person rather than trusting the mail. Keep a copy before sending the original; if anything is lost, the copy makes it easy for the eye doctor’s office to resend.
