Iowa Medicaid prior authorization form 470-5595 is the state’s uniform outpatient PA request, used for both fee-for-service Medicaid and all three Iowa Health Link managed care plans. To get it approved, you fill in the member and provider identifiers, the CPT/HCPCS and ICD-10 codes for what you are requesting, a clinical justification that addresses the criteria in Iowa Administrative Code rule 441-78.28, and the treating physician’s signature. You then submit it to the member’s MCO or, for FFS members, to the Iowa Medicaid Enterprise.
Download the current version from the Iowa HHS provider forms page at hhs.iowa.gov/medicaid/provider-services/provider-forms. Use 470-5595 for outpatient services. Inpatient admissions use Form 470-5594. A handful of services have their own dedicated forms: 470-5473 for inpatient psychiatric hospital stays, 470-2145 for augmentative communication systems, 470-4210 for enteral nutrition certification, and 470-4767 for hearing aids. If you are not sure a given service needs PA at all, rule 441-78.28 is the controlling list, and the member’s MCO can confirm.
Completing Form 470-5595
Member and Provider Information
Enter the member’s full legal name and eight-digit Iowa Medicaid ID number exactly as they appear on the Medicaid card. On the provider side, include the treating provider’s National Provider Identifier and the practice’s federal tax identification number. Transposed digits in any of these fields are one of the fastest routes to an administrative denial that has nothing to do with the medical merits of the request, so verify them before you submit.
Codes and Units
Every request must list the CPT or HCPCS codes for the service, equipment, or supply being requested, together with the ICD-10 diagnosis codes that explain why the member needs it. Include the number of units or the duration of service. The procedure codes and the diagnosis codes have to line up with each other. A mismatch between what you are asking for and the condition you say justifies it is a common reason reviewers return requests.
Clinical Justification
This is where most approvals are won or lost. Rule 441-79.9 requires the request to address the specific criteria for the service, drug, or equipment in rule 441-78.28. Attach the recent clinical notes, lab results, and imaging that show why the service is necessary and why cheaper or less intensive alternatives will not work.
Reviewers are looking for a clear sequence: what you tried first, why it failed, and why what you are now requesting is the appropriate next step. A short narrative of the member’s treatment history, written in that order, strengthens the request considerably. For equipment requests, document the underlying functional impairment and the failure of simpler alternatives in concrete terms.
Physician Signature
The treating physician must sign the form to certify that the information is accurate and the service is medically necessary. Unsigned forms are returned without review.
Where to Send the Completed Form
Before you submit, confirm whether the member is in fee-for-service Medicaid or in one of the three Iowa Health Link MCOs. Sending the form to the wrong place delays the whole process.
Fee-for-Service Members
For FFS members, 470-5595 and all supporting documentation go to the Iowa Medicaid Enterprise. You have three main options:
- Fax to 515-725-1356, the designated PA line.
- Submit electronically using the ASC X12N 278 transaction. If the electronic request needs attachments, send the paper attachments separately with a Prior Authorization Attachment Control Form as the cover sheet, referencing the attachment control number from the 278.
- Use the Iowa Medicaid Portal Application (IMPA) at secureapp.dhs.state.ia.us/impa/.
Pharmacy PA requests follow a separate track. Fax them to 1-800-574-2515, or call the Pharmacy PA Helpdesk at 1-877-776-1567 (Monday through Friday, 8:00 a.m. to 5:00 p.m. Central). Inpatient psychiatric hospital requests on Form 470-5473 go by email to inptps@hhs.iowa.gov, and PMIC admissions to PMIC2@hhs.iowa.gov.
Managed Care Members
Each MCO uses the same state-issued form but has its own submission channel.
- Iowa Total Care accepts the uniform PA forms from the Iowa HHS site; provider resources are at iowatotalcare.com.
- Molina Healthcare of Iowa accepts PA requests through Availity Essentials (Patient Registration, then Authorizations). The utilization management fax number is (877) 319-6828.
- Wellpoint Iowa accepts requests by phone, fax, or Availity’s Interactive Care Reviewer. Fax numbers vary by service type: 1-844-442-8016 for inpatient behavioral health, 1-844-451-2826 for outpatient behavioral health, and 1-800-964-3627 for long-term services and supports.
Whatever method you use, save the fax confirmation or portal receipt. The system issues a tracking or reference number when it accepts the request. Record it right away, because you will need it for any status inquiry or follow-up.
How Long a Decision Takes
Starting January 1, 2026, standard MCO decisions must be issued within seven calendar days of receiving the request, down from the previous 14-day limit. If the standard timeframe could seriously jeopardize the member’s life, health, or ability to function, the provider can mark the request expedited and the MCO must decide within 72 hours.
Fee-for-service requests reviewed by Iowa Medicaid Enterprise have a different safety net. Under rule 441-79.9, if Iowa Medicaid has not issued a decision within 60 days of receiving the request, the authorization is automatically approved. It rarely comes to that, but it is worth knowing when a request seems to have disappeared.
The provider sees the decision first through the portal or MCO system, and the member receives a written notice by mail. A denial notice spells out the specific reasons for the rejection, which is the starting point for anything you do next.
If the Request Is Denied
Peer-to-Peer Review
Before filing a formal appeal, the treating physician can request a peer-to-peer discussion with the MCO’s medical director to walk through clinical information that was not in front of the original reviewer. This has to be requested within two business days of the denial. It is optional and does not have to happen before an appeal, but it sometimes resolves the issue faster than the formal process.
MCO Internal Appeal
For Iowa Health Link members, the first formal step is an internal appeal with the MCO, filed within 60 days of the date on the Notice of Action.
- Iowa Total Care: call 1-833-404-1061, fax 1-833-809-3868, or mail to Iowa Total Care, Attn: Appeals, 1080 Jordan Creek Parkway, Suite 100 South, West Des Moines, IA 50266.
- Molina Healthcare of Iowa: call 1-844-236-0894, fax 1-888-832-1922, or mail to Molina Healthcare of Iowa, P.O. Box 93010, Des Moines, IA 50393.
- Wellpoint Iowa: contact information is printed on the Notice of Action letter.
The MCO must decide standard appeals within 30 days and expedited appeals within 72 hours when delay could harm the member.
State Fair Hearing
If the MCO upholds the denial, the member can request a state fair hearing through Iowa HHS. The deadline is 120 days from the date on the letter saying the MCO’s first-level review has been exhausted. Hearings requested after 120 days will not be granted. When a provider or other representative files on the member’s behalf, the member must provide written consent using Form 470-5526 (Authorized Representative for Managed Care Appeals), submitted with the hearing request.
Fee-for-service members go straight to the HHS Appeals Section for a state fair hearing, because there is no MCO-level appeal to exhaust first. An administrative law judge conducts the hearing.
