Medi-Cal covers Mounjaro when it is prescribed to treat Type 2 diabetes, and only then. The Medi-Cal Rx Contract Drugs List restricts tirzepatide to improving blood-sugar control in patients with that diagnosis, so a claim without a matching Type 2 diabetes code is rejected automatically at the pharmacy counter.1Medi-Cal Rx. Medi-Cal Contract Drugs List (CDL) – Section: Hormones: Hypoglycemics Even with a qualifying diagnosis, your provider must get prior authorization before the pharmacy can fill the prescription.
Who Qualifies
The Contract Drugs List places Mounjaro under a “Code I” diagnosis restriction for Type 2 diabetes. If that restriction is not met, the pharmacy claim denies with Reject Code 80, “Diagnosis Code Submitted Does Not Meet Drug Coverage Criteria.”2Medi-CalRx. GLP-1 Coverage Considerations In practice, your prescriber has to transmit the prescription with a Type 2 diabetes ICD-10 code such as E11.9.
Coverage also comes with quantity and product limits. Medi-Cal allows a maximum of one carton of four single-dose pens per fill, with no more than one fill every 28 days, and only the branded Eli Lilly product (identified by a specific manufacturer labeler code) is reimbursable.1Medi-Cal Rx. Weight Loss Is Not a Covered Use for Adults
Federal law lets states exclude weight-loss drugs from Medicaid coverage,3Office of the Law Revision Counsel. 42 US Code 1396r-8 – Payment for Covered Outpatient Drugs and California does. Effective January 1, 2026, GLP-1 medications prescribed for weight loss or weight-loss-related indications are not a Medi-Cal Rx benefit for adults, even for members who previously had an approved prior authorization for that purpose.4Medi-Cal Rx. Medi-Cal Contract Drugs List (CDL) – Section: GLP-1 Drugs for Weight Loss: Non-Benefit That exclusion covers Zepbound as well, which is the same active ingredient as Mounjaro (tirzepatide) but FDA-approved for chronic weight management rather than diabetes.
Mounjaro itself stays on the Contract Drugs List for its diabetes indication. If your provider submits a prior authorization for another FDA-approved, non-weight-loss use, Medi-Cal Rx will review it for medical necessity case by case.2Medi-CalRx. GLP-1 Coverage Considerations
Members Under 21
If you are younger than 21, the adult exclusion does not apply. GLP-1 drugs for weight loss may still be covered under the federal Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) benefit, which requires states to cover medically necessary treatments for children and young adults. Your prescriber can submit a prior authorization for a weight-loss indication, and Medi-Cal Rx will review it for medical necessity.5Medi-Cal Rx. What the Prior Authorization Needs
Your provider files the prior authorization, not you. But incomplete paperwork is the most common reason a request stalls, so it helps to know what Medi-Cal Rx expects to see in the record.
- A Type 2 diabetes ICD-10 code (such as E11.9, or a more specific code reflecting complications) that matches your medical record.6Department of Health Care Services. Medi-Cal Rx Prior Authorization Request (DHCS 6560)
- Recent HbA1c values showing how well your blood sugar is currently controlled.
- A treatment history listing medications already tried and the results. Medi-Cal generally expects documentation that first-line therapy (typically metformin) was tried with specific dosages and duration and did not achieve adequate control.7Department of Health Care Services (DHCS) / Medi-Cal Rx. - The exact tirzepatide dose strength, quantity, and days of supply being requested.

How the Request Is Submitted and How Long It Takes
Providers can submit through CoverMyMeds, the Medi-Cal Rx Secured Provider Portal, fax (1-800-869-4325), an NCPDP pharmacy point-of-sale transaction, or by mail to the Medi-Cal Rx Customer Service Center, P.O. Medi-Cal Rx CoverMyMeds How-To Guide
California law requires Medi-Cal Rx to respond within 24 hours of receiving a prior authorization request. Welfare and Institutions Code Section 14133.37 sets that deadline for a response “by telephone or other means of telecommunication.”10Department of Health Care Services. Medi-Cal Rx Frequently Asked Questions – Section: Prior Authorization/Utilization Management That is a response window, not a final-decision guarantee: if the submission is incomplete or needs additional clinical review by DHCS staff, a final determination can come later. All denials require a separate DHCS review before they become final. Once approved, the pharmacy can fill the prescription and bill Medi-Cal directly, and the approval will specify a coverage period for refills.
If the Request Is Denied
A denial generates a written Notice of Action explaining what was decided, why, and your appeal rights.11Cornell Law School. California Code of Regulations Tit 22, 50179 – Notice of Action – Medi-Cal-Only Determinations or Redeterminations Read it carefully. The reason matters: a missing diagnosis code, thin documentation, and a clinical finding that Mounjaro is not medically necessary each call for a different response.
When the problem is paperwork, your prescriber can usually fix it by resubmitting a new prior authorization with the missing information, such as updated labs, a fuller treatment history, or a letter of medical necessity. That is often faster than a formal appeal.
When you want to challenge the decision itself, you have 90 days from the date you receive the Notice of Action to request a state fair hearing. The back of the notice contains a hearing request form. You can file by mail, by fax at 833-281-0905, online, or by calling 800-743-8525.12Department of Health Care Services. Medi-Cal Fair Hearing
If you file quickly enough (by the effective date on the notice when 10-day advance notice was required, or within 10 days of the notice date otherwise), your existing benefits can continue while the hearing is pending. This “aid paid pending” rule prevents a treatment gap while your case is reviewed.12Department of Health Care Services. Medi-Cal Fair Hearing
