The Katy ISD physical form is the UIL Pre-Participation Physical Evaluation, and your student in grades 7–12 cannot try out, practice, or compete until a provider has signed it, you have uploaded it to Rank One Sport, and you have completed the rest of the online participation paperwork. The exam must be dated on or after May 1 to cover the upcoming school year.
Use the UIL Form, Dated May 1 or Later
The UIL Pre-Participation Physical Evaluation is the only form Katy ISD accepts.1Katy Independent School District. Athletics-Physical It has two parts. Parents and the student fill out the medical history at home. A licensed physician, physician assistant, or advanced practice nurse completes the physical exam section in the office, checking vision, height, weight, blood pressure, cardiovascular health, and the student’s history of surgeries, allergies, chronic conditions, and medications. Finish the medical history before the appointment so the provider can review it with you.
Watch the date. A physical administered before April 30 may not be valid after August 1, so schedule the appointment for May 1 or later to cover the full school year.2Katy Independent School District. Participation Forms An exam done in March during spring registration can expire before fall practice starts, which is the single most common reason families end up redoing a physical.
Where to Get the Exam
Any qualified provider can complete the UIL form. Katy ISD also partners with Medical Colleagues of Texas to offer physicals at district high school gyms for $30, typically from late April through August, rotating among campuses.3Katy Independent School District. ECG and Physical Schedule The schedule is posted on individual school athletic pages and through the district athletics office. ECG screenings are offered at those same events in the hours before the physicals. The screening is not mandatory, and schools are not required to provide or pay for it, but you can request one through any appropriately licensed provider.4University Interscholastic League. Complete the Online Forms in Rank One Sport
Every required participation form must be submitted before a student takes part in any tryout, practice, athletic class, open gym, weight room session, or competition.2Katy Independent School District. Participation Forms Before you log in to Rank One Sport, have your student’s Katy ISD ID number, private insurance information, and emergency contact details on hand.
The electronic forms you will complete in the portal are:5Katy Independent School District. Physical and Online Paperwork Instructions
- Handbook Acknowledgement Form, confirming you have reviewed the Katy ISD athletic handbook.
- Birth Certificate Upload, a scanned copy of the student’s birth certificate.
- Consent to Treat, authorizing emergency medical treatment if a parent cannot be reached.
- UIL Forms Signature Page, bundling four UIL documents including the Concussion Acknowledgement Form, Sudden Cardiac Death Awareness Form, and Acknowledgement of Rules.6Katy Independent School District. 2025-2026 Katy ISD Athletic Handbook
- Residency Verification, confirming the student lives within Katy ISD boundaries.
High school athletes also sign the UIL Parent and Student Notification/Agreement Form on illegal steroid use and random testing, which both the parent and student must sign.7University Interscholastic League. Steroid Agreement Form Football players have one extra item: a Football Helmet Information Sheet and Inspection Checklist. The concussion and sudden cardiac forms must be completed every school year — last year’s signatures do not carry over.
Upload the Signed Physical
Once the provider has signed the UIL form, scan or photograph both pages (medical history and physical exam) into a clear PDF or image. Log in to Rank One Sport through the link on the Katy ISD athletics participation page.2Katy Independent School District. Participation Forms Attach the file to your student’s profile when prompted.
Before you submit, check that the physician’s signature, the date, and any clearance or restriction notes are legible in the uploaded image. A blurry signature is one of the most common reasons an upload is sent back. The portal will not let you finalize the submission if any required form is still incomplete, so a submission error usually means a form was started but not finished.
Clearance, Rejections, and Medical Flags
Campus athletic trainers review the uploaded physical and the completed forms to confirm every signature is present and the provider has cleared the student without restrictions. Students cannot join any team activity until clearance is complete.2Katy Independent School District. Participation Forms Log back into Rank One Sport to check status; a green indicator means the student is cleared.
If something is rejected, you will typically get an email explaining what to fix. Common reasons: an expired physical date, a missing physician signature, an unreadable upload, or an incomplete online form. Correct the issue and resubmit through the same portal. The faster you respond, the less practice time your student misses.
Not every exam ends in full clearance. If the provider flags a condition that needs further evaluation, such as a heart murmur, fainting during exercise, or uncontrolled asthma, the student may receive conditional clearance or a temporary hold until additional testing or a specialist visit is complete. The athletic trainer cannot override a provider’s restriction, so the student stays off the field until the follow-up is resolved and new documentation is uploaded. Cardiovascular abnormalities, concussion history with lingering symptoms, and sickle cell trait are among the conditions that most often trigger a referral.
