Kentucky Medicare: Plans, Costs, and the Birthday Rule

Kentucky Medicare works the same way the federal program does everywhere else, with one meaningful addition: the state layers its own consumer protections and assistance programs on top. If you are turning 65 in Kentucky, your Initial Enrollment Period runs for seven months around your birthday, the standard Part B premium in 2026 is $202.90 per month, and you will choose between Original Medicare (often paired with a Medigap policy) and a Medicare Advantage plan sold by a private insurer. What follows is what you actually need to decide, in the order the decisions come up.

When You Can Enroll

Most people qualify for Medicare at 65. You can also qualify earlier after collecting Social Security Disability Insurance for 24 months, and sooner if you have end-stage renal disease or ALS.1Social Security Administration. Medicare Information You do not have to be retired or collecting Social Security to sign up.

Your Initial Enrollment Period is seven months long. It starts three months before the month you turn 65, includes your birthday month, and ends three months after.2Medicare.gov. When Does Medicare Coverage Start Enrolling in the first three months of that window gets your coverage started the soonest. Waiting until the back half can push your start date out by a month or two.

Miss the window entirely and your next chance is the General Enrollment Period from January 1 through March 31, with coverage starting the month after you sign up.2Medicare.gov. When Does Medicare Coverage Start That gap can leave you uninsured for months, and in most cases it also triggers a permanent late-enrollment penalty (see below).

If You’re Still Working at 65

You can usually delay Part B without penalty if you have health coverage through a current job (your own or a spouse’s). This exception only applies to coverage based on current employment. COBRA, retiree plans, VA benefits, and individual marketplace plans do not count.3Social Security Administration. How to Apply for Medicare Part B During Your Special Enrollment Period

When the job or the employer coverage ends, whichever happens first, you get an eight-month Special Enrollment Period to sign up for Part B without penalty.3Social Security Administration. How to Apply for Medicare Part B During Your Special Enrollment Period Miss that eight-month window and you are back to waiting for the General Enrollment Period and probably paying a permanent surcharge.

Most people should still enroll in premium-free Part A at 65 even if they delay Part B. One exception matters: if you are contributing to a Health Savings Account, enrolling in any part of Medicare ends your ability to make further HSA contributions.

What Medicare Covers and What It Costs in 2026

Medicare has four parts, and the first thing to understand is what each one is for.

Part A covers inpatient hospital stays, skilled nursing facility care after a qualifying hospital stay, hospice, and some home health care.4Medicare.gov. What Part A Covers Most people pay no premium because they or a spouse paid Medicare taxes for at least 10 years. You still owe cost-sharing when you use services: the inpatient hospital deductible is $1,736 per benefit period in 2026, and skilled nursing facility care is fully covered for 20 days after a qualifying three-day hospital stay, then runs $217 per day from day 21 through day 100. After day 100, Medicare pays nothing.5Centers for Medicare & Medicaid Services. 2026 Medicare Parts A and B Premiums and Deductibles

Part B covers outpatient care: doctor visits, lab tests, durable medical equipment, mental health services, ambulance transport, and limited outpatient drugs.6Medicare.gov. What Part B Covers The 2026 standard premium is $202.90 per month and the annual deductible is $283. After the deductible, you typically pay 20% of the Medicare-approved amount.5Centers for Medicare & Medicaid Services. 2026 Medicare Parts A and B Premiums and Deductibles A wide range of preventive services, including annual wellness visits, mammograms, colonoscopies, flu and COVID-19 vaccinations, and diabetes and cardiovascular screenings, carry no cost when you see a provider who accepts assignment.7Medicare.gov. Part C is Medicare Advantage, discussed in the next section.

Part D covers outpatient prescription drugs through private plans. Each plan has its own formulary, premium, and pharmacy network. The big change in recent years is a hard annual cap on out-of-pocket drug spending: in 2026 that cap is $2,100, after which you pay nothing more for covered drugs the rest of the year.

Original Medicare or Medicare Advantage

This is the real decision point. Both paths cover the same core services, but they work very differently.

Original Medicare (Parts A and B) lets you see any doctor or hospital in the country that accepts Medicare, with no referrals. That matters if you travel, split time between states, or see specialists outside your immediate area. The catch is that Original Medicare has no annual cap on your out-of-pocket costs.8Medicare.gov. Medicare Costs A long hospital stay or a serious illness can keep adding bills with no ceiling. That is why most people on Original Medicare also buy a Medigap policy (see below) and a standalone Part D plan.

Medicare Advantage plans are sold by private insurers approved by Medicare. They bundle Parts A and B, usually include Part D, and often add routine dental, vision, and hearing benefits.9Medicare.gov. Medicare Advantage and Other Health Plans Every Medicare Advantage plan must cap your yearly out-of-pocket spending on covered services. The trade-off is network restrictions: most plans are HMOs or PPOs, so if your doctor or hospital is out of network you may pay more or get no coverage. Rural Kentuckians should check network adequacy carefully, because plan networks can thin out beyond the Louisville, Lexington, and Northern Kentucky corridors.

Kentucky’s State Health Insurance Assistance Program (SHIP) will help you compare specific plans in your county against Original Medicare plus Medigap, at no cost and without selling anything.10Kentucky Department of Insurance. Medigap and Kentucky’s Birthday Rule

If you stay with Original Medicare, a Medigap policy from a private insurer covers most or all of the deductibles, copayments, and coinsurance that Medicare leaves to you. Medigap plans are standardized by letter (Plan A, Plan G, Plan N, and so on), so a Plan G from one company covers exactly the same benefits as a Plan G from another. The difference is price.

The Federal Open Enrollment Period

Your strongest window to buy Medigap is the one-time, six-month federal Open Enrollment Period that starts the first day of the month you are both 65 and enrolled in Part B.11Medicare.gov. When Can I Buy a Medigap Policy During those six months, any insurer selling Medigap in Kentucky must sell you any policy they offer at their best rate, no matter your health. Once this window closes, insurers in most situations can refuse coverage or charge more because of medical history.

Kentucky’s Birthday Rule

Kentucky adds a protection the federal rules do not. Under the Birthday Rule, effective January 1, 2024, you get a 60-day guaranteed-issue window starting on your birthday each year to switch your Medigap policy to the same lettered plan with a different insurance company, with no medical underwriting. It is strictly plan-for-plan: a Plan G can move to another Plan G, but you cannot use the Birthday Rule to jump to a different letter. If you have a “Select” or innovative variation, you can move to the standard version of that same letter, which can mean a lower premium.12Kentucky Department of Insurance. Medicare Supplement Update Consumer Alert

The practical effect is that you can shop around every year on your birthday without worrying that a new health condition will block the switch or push the rate up.

A Trial Right for Medicare Advantage

If you enroll in a Medicare Advantage plan when you first become eligible at 65 and decide it is not working, you have 12 months to switch back to Original Medicare and buy any Medigap policy in the state with guaranteed-issue rights. The same protection applies if you dropped a Medigap policy to try Medicare Advantage for the first time, though in that case you can return to your original policy or certain standard plans rather than any plan on the market. Apply for the Medigap policy within 63 days of your Medicare Advantage coverage ending.11Medicare.gov. When Can I Buy a Medigap Policy

High-Income Surcharges (IRMAA)

If your income is above certain thresholds, Social Security adds a surcharge to your Part B and Part D premiums called the Income-Related Monthly Adjustment Amount. It is based on the tax return from two years earlier, so your 2024 income sets your 2026 premiums.13Social Security Administration. The 2026 Part B brackets for individual filers (joint filers in parentheses) are:5Centers for Medicare & Medicaid Services. 2026 Medicare Parts A and B Premiums and Deductibles

  • $109,000 or less ($218,000 joint): standard $202.90
  • $109,001–$137,000 ($218,001–$274,000 joint): $284.10
  • $137,001–$171,000 ($274,001–$342,000 joint): $405.80
  • $171,001–$205,000 ($342,001–$410,000 joint): $527.50
  • $205,001–$499,999 ($410,001–$749,999 joint): $649.20
  • $500,000 or more ($750,000 or more joint): $689.90

Part D has its own IRMAA at the same income breakpoints, running from an extra $14.50 per month at the lowest surcharge tier up to $91.00 per month at the highest.14Medicare.gov. 2026 Medicare Costs

Late Enrollment Penalties Are Permanent

Medicare’s late-enrollment penalties stick with you for as long as you have coverage, so they compound into real money over retirement.

For Part B, your monthly premium goes up by 10% for every full 12-month period you could have had it but did not, and you pay that surcharge for the rest of your life.15Medicare.gov. Avoid Late Enrollment Penalties Three years without coverage means a 30% higher premium every month, forever. The main exceptions are a valid Special Enrollment Period through employer coverage or enrollment in a Medicare Savings Program.

For Part D, going 63 or more consecutive days without creditable drug coverage after your initial eligibility window adds 1% of the national base beneficiary premium for each uncovered month. The 2026 base is $38.99, so each month of delay adds roughly $0.39 to your permanent monthly penalty.15Medicare.gov. Avoid Late Enrollment Penalties The penalty is recalculated each year as the base changes.

Kentucky Programs That Reduce What You Pay

Kentucky administers several programs through the Cabinet for Health and Family Services that can cut Medicare costs substantially if your income is limited.

Medicare Savings Programs

Medicare Savings Programs use state Medicaid funds to pay some or all of your Medicare cost-sharing. Eligibility is based on income and asset limits tied to the federal poverty level and updated each year.16Medicare. Medicare Savings Programs There are three tiers:

  • Qualified Medicare Beneficiary (QMB) is the most comprehensive. It pays your Part A premium if any, your Part B premium, and all deductibles, coinsurance, and copayments for Medicare-covered services.
  • Specified Low-Income Medicare Beneficiary (SLMB) covers the Part B premium only. You must have both Part A and Part B.
  • Qualifying Individual (QI) also covers the Part B premium only, at a slightly higher income threshold than SLMB. You must have both Part A and Part B.

Qualifying for any of the three automatically enrolls you in Extra Help, the federal Low-Income Subsidy that sharply reduces Part D premiums, deductibles, and copayments.16Medicare. Medicare Savings Programs You can apply through a local Department for Community Based Services office or online through the state’s benefind system.17Kentucky Cabinet for Health and Family Services. PACE

The Program of All-Inclusive Care for the Elderly serves people 55 and older who need a nursing-home level of care but want to stay in the community. It bundles doctor visits, prescriptions, therapy, transportation, meals, and personal care into one program. In Kentucky, Horizon PACE operates centers in Bowling Green, Monticello, Richmond, and Somerset, covering 14 counties in the southern and eastern parts of the state. If you live outside that service area, PACE is not currently available to you.

SHIP Counseling

Kentucky’s State Health Insurance Assistance Program offers free one-on-one help from trained volunteers who do not sell insurance. SHIP counselors compare plans, work through enrollment paperwork, help with claims disputes, and screen you for Medicare Savings Programs and Extra Help. SHIP is reached through local Area Development Districts across Kentucky.
