Confirm the copy says “certified” and carries the clerk’s raised seal or signature. A plain photocopy, even a clean one, will be rejected.
Update Social Security First
Social Security has to come before anything else. To update your card, complete Form SS-5 (Application for a Social Security Card) and submit it with your certified marriage certificate and a proof of identity such as a U.S. driver’s license, state-issued ID, or U.S. passport. Change Your Louisiana Driver’s License or State ID

A name change is treated as a duplicate license, and the standard duplicate fee is $17. If your license is within 180 days of expiration, you can renew and change the name together and pay the renewal fee instead. One catch that trips people up: the OMV can only change the name on a valid credential. If your license is expired or suspended, you have to resolve that first.
Update Your U.S. Passport
Which form you use depends on how old your passport is. If it was issued less than a year ago and your name change happened within that same year, you can request a corrected passport at no cost using Form DS-5504. Tax Withholding and Employer Records
The IRS doesn’t have a separate name-change form; it uses whatever Social Security has, which is why you handled SSA first. Newlyweds are required to give their employer a new Form W-4 within 10 days. Banks, Insurance, and Voter Registration
Financial institutions generally want you to appear in person at a branch with your updated government-issued photo ID and the certified marriage certificate. On joint accounts, most banks require every owner to be present. Call ahead to confirm the process; some banks will accept changes by mail or phone if you can’t get to a branch.
From there, work through the rest of your financial life: credit cards, health and auto and home insurance, investment and retirement accounts, and any loans or mortgages. The paperwork combination is almost always the same, so batch these together.
