MC 210 Statement of Facts: How to Complete and Submit It

The MC 210 Statement of Facts is the questionnaire California’s Department of Health Care Services uses to decide whether you qualify for Medi-Cal.1Legal Information Institute. Filling it in accurately the first time is what prevents weeks of back-and-forth with your county worker.

When You Have to Fill One Out

State regulations formally title the document the “Statement of Facts,” though the copy the county sends you is often labeled “Medi-Cal Renewal Form.”2California Department of Health Care Services. You’ll be asked to complete one in three situations:

  • At annual renewal, if the county cannot verify your continued eligibility automatically from records it already has (the “ex parte” check), it will mail you a form to complete.   • When your income, household size, or living situation changes in a meaningful way, the county can require an updated Statement of Facts between renewals.

The form is especially important if you’re applying under a non-MAGI program, which covers people 65 or older, people with disabilities, and people needing nursing facility care. Those programs count assets in addition to income, so the property questions carry weight that they don’t carry for standard MAGI applicants.

What to Gather Before You Start

Missing documents are the most common reason applications and renewals stall, so pull everything together before you open the form.   • Government-issued identification for each household member listed.

  • Bank and credit union statements, mortgage and car loan statements, and current values for any real estate.   • Records of medical, shelter, or other expenses the county may factor in.

If you’re applying for or renewing long-term care coverage under a non-MAGI program, you’ll also need records of property transfers made within the past 30 months, because those transfers are reviewed for asset-limit purposes.5Department of Health Care Services. Filling In the Form Without Creating Delays

You can pick up the MC 210 at your county social services office or download it from the DHCS website. If a pre-filled renewal packet arrives in the mail, read every populated field carefully. Outdated information carries the same risks as wrong information, and it is your signature that certifies it.

A few habits keep files moving:

  • Don’t leave fields blank. Write “N/A” or “none” for questions that don’t apply. A blank looks like a skipped question and gets the form kicked back.
  • Make your numbers match your documents. The eligibility worker will compare your entries against attached pay stubs, bank statements, and benefit letters. Inconsistencies create delays.
  • Sign and date the form. An unsigned form cannot be processed, and the date establishes the time frame your information covers.

Keep the narrative sections factual. They are not the place to argue for eligibility or explain hardship; stick to numbers and let the worker apply the rules.

How and When to Submit

When you receive a renewal form in the mail, federal rules require the state to give you at least 30 days to complete and return it.6Medicaid.gov. Missing it can create a gap in your coverage or end it entirely.

You have several ways to submit:

  • Online through BenefitsCal. Log in, go to “Things to Do,” and follow the prompts to submit the form and upload documents. This is typically the fastest route.7BenefitsCal. Renew Medi-Cal Update
  • By mail. The packet includes a postage-paid return envelope. If you use your own, consider certified mail so you have proof of the mailing date.3Department of Health Care Services. Medi-Cal Annual Redetermination Form
  • In person at your county social services office. Ask for a date-stamped receipt; that receipt is your best proof of timely submission if a dispute arises later.

If you miss the deadline and your coverage lapses, you have 90 days from the date coverage ended to send in the form and any missing documents. Respond within that window and your coverage can be restored without a new application. Miss the 90 days and you start over.

What Happens After You Submit

The county has 45 days to process a standard Medi-Cal application. When the determination involves establishing a disability, the window stretches to 90 days. Even with documented good cause for further delay, total processing cannot exceed three months from your application date.

During review, the eligibility worker compares your form to your attached documents and may cross-check information through electronic databases. If something is missing or unclear, the county sends a Request for Information that spells out what’s needed and gives you a deadline to respond.2California Department of Health Care Services. Keep a complete copy of everything you submitted. When the county asks for clarification, you can compare their request against your records and respond with what’s actually missing, rather than reconstructing the filing from scratch.

If Someone Else Needs to Handle the Form for You

If illness, disability, or other circumstances make it hard to complete the MC 210 yourself, federal rules let you designate an authorized representative to act for you. That person can sign your application or renewal, submit documents, receive copies of your notices, and communicate with the county on your behalf.8eCFR. 42 CFR 435.923 – Authorized Representatives

To set it up, you provide a written designation with your signature. If someone already has legal authority over your affairs through a court-issued guardianship or a valid power of attorney, the county must accept that document as the designation.8eCFR. 42 CFR 435.923 – Authorized Representatives The designation remains in effect until you revoke it, the representative resigns, or the underlying legal authority changes. Your representative carries the same duty of accuracy you would and is legally bound to keep your personal and medical information confidential.

Accuracy Matters

Honest mistakes are not fraud. But knowingly providing false information to obtain Medi-Cal benefits is a crime under both California and federal law, and a conviction can carry jail time, fines, and up to a one-year suspension of Medi-Cal eligibility.9California Legislative Information. Cross-checking every entry against your documentation protects you on both fronts.

If the County Denies, Terminates, or Reduces Your Coverage

You have the right to challenge an adverse decision through a state hearing filed with the Department of Social Services. Generally you have 90 days from the date the notice of action is mailed to request one. One protection is worth knowing before you need it. If you request a hearing before the effective date of the county’s action, your Medi-Cal benefits continue at their existing level until the hearing decision is issued.12Medicaid.gov. You can request a hearing by mail, in person, or by phone.
